Use this <strong>checklist</strong> if:<br />

Your UNIVERSITY (as shown on your Student ID) or your PERMANENT RESIDENCE (as shown on your Drivers<br />

License) is located <strong>in</strong> Florida, Georgia or South Carol<strong>in</strong>a.<br />

You MUST submit your Student Visa application and documents listed <strong>in</strong> the <strong>checklist</strong> below, IN PERSON, to the Miami<br />

Consulate between MAY 20 and JUNE 14, 2013 as it may take four to six weeks for the <strong>consulate</strong> to process your <strong>visa</strong>.<br />

to the calendar <strong>of</strong> available appo<strong>in</strong>tments. Appo<strong>in</strong>tments fill up quickly, so please make your appo<strong>in</strong>tment EARLY!<br />

Once your <strong>visa</strong> has been processed, it will be mailed back to you us<strong>in</strong>g a prepaid USPS Express Mail Envelope (details<br />

<strong>in</strong>cluded <strong>in</strong> <strong>checklist</strong> below). Other express delivery services, certified or registered mail are not accepted.<br />

Important Notes:<br />

• If you are an Academic Year <strong>student</strong>, or plan on stay<strong>in</strong>g for another semester (180+ days) please see additional<br />

<strong>in</strong>structions (12-13) at the end <strong>of</strong> the <strong>checklist</strong>.<br />

• If you plan on travel<strong>in</strong>g outside the USA prior over the summer, please contact Lisa Anthony (lanthony@syr.edu)<br />

immediately for additional <strong>in</strong>structions.<br />

You must submit orig<strong>in</strong>als AND one photocopy <strong>of</strong> the follow<strong>in</strong>g <strong>checklist</strong> items:<br />

1. Orig<strong>in</strong>al Passport AND Photocopy <strong>of</strong> Passport ID page: valid until at least June 2014 (fall <strong>student</strong>s) and<br />

December 2014 (yearlong <strong>student</strong>s). Passports must have at least two blank pages to affix the <strong>visa</strong>. Passports<br />

must be signed!<br />

In addition to a national passport, Non-US citizens must present evidence <strong>of</strong> Immigration Status <strong>in</strong><br />

the US via the I-20 /IAP-66 or Alien Registration Card <strong>in</strong> and a permit to return to the United<br />

States <strong>of</strong> America <strong>in</strong> the form <strong>of</strong>: a resident card, or a <strong>student</strong>/work <strong>visa</strong> that is stamped on the<br />

passport and is valid for a period <strong>of</strong> three months beyond the expected length <strong>of</strong> stay <strong>in</strong> the<br />

Schengen area, or the period <strong>of</strong> validity <strong>of</strong> the <strong>visa</strong>, whichever is longer. (N/A TO US CITIZEN).<br />

*Photocopies required as well.<br />

*Before submitt<strong>in</strong>g your passport, make an extra copy for your own records! If your passport<br />

expires before June 2014 (fall <strong>student</strong>s) and December 2014 (yearlong <strong>student</strong>s), you will need to renew it<br />

us<strong>in</strong>g an expedited service. Details <strong>of</strong> this can be found here:<br />

2. Two National Application Forms (Orig<strong>in</strong>al AND photocopy). Forms must be fully completed and<br />

signed.


• Application Forms must be completed <strong>in</strong> blue or black <strong>in</strong>k, and <strong>in</strong> pr<strong>in</strong>t (not cursive). Before<br />

be<strong>in</strong>g copied, the application form must be signed on page 4 (please f<strong>in</strong>d application form <strong>in</strong>structions<br />

below).<br />

3. Pro<strong>of</strong> <strong>of</strong> residence <strong>in</strong> Miami Consulate´s jurisdiction (FL, GA or SC): Orig<strong>in</strong>al and photocopy <strong>of</strong><br />

College/University ID and <strong>of</strong> U.S. Driver’s License with current address.<br />

4. 2 color passport sized photos: Passport photos can be taken at any K<strong>in</strong>ko’s, Walgreens, CVS, etc.<br />

Photos must be RECENT with white background and pr<strong>of</strong>essionally taken. Photos must be GLUED to<br />

each application form <strong>in</strong> the top right corner. DO NOT STAPLE PHOTOS TO APPLICATION<br />

FORMS!<br />

5. Letter <strong>of</strong> acceptance as a full time <strong>student</strong> from Spa<strong>in</strong>'s University/School or US program: You MUST<br />

contact Lisa Anthony at lanthony@syr.edu for these letters, at least two weeks prior to your <strong>visa</strong><br />

appo<strong>in</strong>tment, so they may be sent to you.<br />

6. Information about the study abroad program. PLEASE contact Lisa Anthony for this <strong>in</strong>formation.<br />

7. Pro<strong>of</strong> <strong>of</strong> Health Insurance: Provide Orig<strong>in</strong>al Letter guarantee<strong>in</strong>g full International Health <strong>in</strong>surance at<br />

a m<strong>in</strong>imum <strong>of</strong> €30,000 Euros, <strong>in</strong>clud<strong>in</strong>g the necessary Repatriation Coverage for the total period <strong>of</strong><br />

stay <strong>in</strong> Spa<strong>in</strong>. Please contact Lisa Anthony, at least two weeks prior to your <strong>visa</strong> appo<strong>in</strong>tment, so<br />

<strong>SU</strong> <strong>Abroad</strong> can provide you with this letter.<br />

8. Pro<strong>of</strong> <strong>of</strong> f<strong>in</strong>ancial means dur<strong>in</strong>g your stay: <strong>SU</strong> <strong>Abroad</strong> will provide the follow<strong>in</strong>g:<br />

• Letter from the University or School <strong>in</strong> Spa<strong>in</strong> or <strong>in</strong> the USA assum<strong>in</strong>g full f<strong>in</strong>ancial responsibility<br />

dur<strong>in</strong>g your stay. Please contact Lisa Anthony for this letter, at least two weeks prior to your<br />

<strong>visa</strong> appo<strong>in</strong>tment, so they may be sent to you.<br />

9. Pro<strong>of</strong> <strong>of</strong> accommodation <strong>in</strong> the form <strong>of</strong>:<br />

• Letter from the study-abroad program specify<strong>in</strong>g the address where the <strong>student</strong> will be stay<strong>in</strong>g. Please<br />

contact Lisa Anthony for this <strong>in</strong>formation.<br />

10. US Postal Service Money Order payable to the “Consulate General <strong>of</strong> Spa<strong>in</strong>” for $160.00 (US citizens)<br />

or $78.60 (most non-US citizens). For nationals <strong>of</strong> Russia, Macedonia, (ARYM), Montenegro, Serbia,<br />

Albania and Moldavia the fee is $45.85. Do not submit cash, personal checks, or bank checks because<br />

the Consulate will not accept them. These can be purchased at any US Post Office.<br />

11. Pre-paid “US Express Mail” Envelope from the US Postal Service*, Self-Addressed with $19.95 <strong>in</strong> stamps. Once<br />

the <strong>visa</strong> is affixed to your passport (<strong>visa</strong> process<strong>in</strong>g time takes about 6 weeks), you can either go pick it up <strong>in</strong><br />

person or choose this option, where the Spanish Consulate will mail your passport back to you us<strong>in</strong>g this<br />

envelope. In the addressee section <strong>of</strong> your mail<strong>in</strong>g label, write the address where you expect to be over the<br />

summer. In the “Sender” portion <strong>of</strong> the label, please write:<br />

Academic Year Students (180+ days) ONLY:<br />

Medical Certificate (orig<strong>in</strong>al and photocopy): Doctor’s recent statement (no older than 3 months) <strong>in</strong> doctor’s<br />

letterhead or with medical center’s stamp, stat<strong>in</strong>g:<br />

“This health certificate verifies that Mr./Ms ...... is free <strong>of</strong> drug addiction, mental illness and does not<br />

suffer from any disease that could cause serious repercussions to public health accord<strong>in</strong>g to the<br />

specifications <strong>of</strong> the <strong>in</strong>ternational sanitary regulation <strong>of</strong> 2005.<br />

These contagious diseases <strong>in</strong>clude, but are not limited to smallpox, poliomyelitis by wild poliovirus, the<br />

human <strong>in</strong>fluenza caused by a new subtype <strong>of</strong> virus and the severe acute respiratory syndrome (SARS),<br />

cholera, pneumonic plague, yellow fever, viral hemorrhagic fevers (e.g.: Ebola, Lassa, Marburg), West<br />

Nile Virus and other illnesses <strong>of</strong> special importance nationally or regionally (e.g.: Dengue Fever, Rift<br />

Valley Fever and men<strong>in</strong>gococcal disease.)<br />

Ms./Mr. ______________ is a very healthy <strong>in</strong>dividual <strong>in</strong> all senses, she/he has no pre-exist<strong>in</strong>g medical<br />

*The orig<strong>in</strong>al must be translated <strong>in</strong>to Spanish and submitted along with a photocopy,<br />

12. Certification <strong>of</strong> Absence <strong>of</strong> Police Records (orig<strong>in</strong>al and photocopy): FBI or State Police Crim<strong>in</strong>al<br />

Background Check legalized with the “Apostille <strong>of</strong> the Hague Convention” from the U.S. A police<br />

record must be obta<strong>in</strong>ed (crim<strong>in</strong>al background check) from all non EU countries where you have lived<br />

for the past five years for more than 6 months, legalized by the Spanish Consulate or Embassy <strong>in</strong> the<br />

respective countries. If the Certificate is issued outside the USA it must be duly legalized by the<br />

correspond<strong>in</strong>g Consulate <strong>of</strong> Spa<strong>in</strong>. LOCAL POLICE BACKGROUND CHECK WILL NOT BE<br />

ACCEPTED. Orig<strong>in</strong>al translated <strong>in</strong>to Spanish and one copy will be needed (contact a legal translator).<br />

*The orig<strong>in</strong>al must be translated <strong>in</strong>to Spanish and submitted along with a photocopy,<br />

contact a legal translator.<br />

*FBI background checks can take 8-12 weeks to get back from the FBI.<br />

form and the tak<strong>in</strong>g <strong>of</strong> my photograph are mandatory for the exam<strong>in</strong>ation <strong>of</strong> the <strong>visa</strong> application.<br />

Any personal data concern<strong>in</strong>g me which appear on the <strong>visa</strong> application form, as well as my<br />

photograph, will be supplied to the relevant authorities and processed by those authorities, for the<br />

purposes <strong>of</strong> a decision on my <strong>visa</strong> application. Such data as well as data concern<strong>in</strong>g the decision<br />

taken on my application will be entered <strong>in</strong>to, and stored <strong>in</strong> a database. The controller is the Consular<br />

Post at which the <strong>visa</strong> was applied for. Data shall be accessible to the <strong>visa</strong> authorities and the<br />

authorities competent for carry<strong>in</strong>g out checks on <strong>visa</strong>s at external borders and <strong>in</strong> Spa<strong>in</strong>, immigration<br />

and asylum authorities for the purposes <strong>of</strong> verify<strong>in</strong>g whether the conditions for the legal entry <strong>in</strong>to,<br />

stay and residence on the territory <strong>of</strong> Spa<strong>in</strong> are fulfilled, <strong>of</strong> identify<strong>in</strong>g persons who do not or who no<br />

longer fulfil these conditions, <strong>of</strong> exam<strong>in</strong><strong>in</strong>g an asylum application and <strong>of</strong> determ<strong>in</strong><strong>in</strong>g responsibility<br />

for such exam<strong>in</strong>ation.<br />

I am aware that I have the rights <strong>of</strong> access, correction and deletion <strong>of</strong> personal data relat<strong>in</strong>g to me<br />

and the right to object to the process<strong>in</strong>g there<strong>of</strong>, pursuant to the provisions conta<strong>in</strong>ed <strong>in</strong> the Ley<br />

Orgánica 15/1999, de 13 de diciembre, de Protección de Datos de Carácter Personal, by address<strong>in</strong>g<br />

I declare that to the best <strong>of</strong> my knowledge all particulars supplied by me are correct and complete. I<br />

am aware that any false statements shall lead to my application be<strong>in</strong>g rejected or to the annulment <strong>of</strong><br />

a <strong>visa</strong> already granted.<br />

I am aware that once the <strong>visa</strong> request has been submitted, a copy <strong>of</strong> the form, sealed with the<br />

<strong>in</strong>dication <strong>of</strong> the date and place <strong>of</strong> reception, shall be returned to the applicant and that arrangements<br />

can be made with the applicant as to the channel to request corrections or the supply <strong>of</strong> documents or<br />

certificates, as well as to serve notice <strong>of</strong> summons and to notify the decision adopted <strong>in</strong> due course.<br />

Notices and requests shall be served by telephone or fax to the contact number provided by the<br />

<strong>in</strong>terested party or his/her legal representative. If they were to prove <strong>in</strong>effective, they shall be served<br />

<strong>in</strong> writ<strong>in</strong>g to the domicile set out <strong>in</strong> the application, which must be located with<strong>in</strong> the consular<br />

district.<br />

Summons and requests served must be complied with with<strong>in</strong> a period not exceed<strong>in</strong>g ten days, unless<br />

personal attendance is required <strong>in</strong> which case the deadl<strong>in</strong>e is fifteen days.<br />

Once all the possibilities <strong>of</strong> serv<strong>in</strong>g a notice have been exhausted, the notice shall be served by<br />

means <strong>of</strong> an announcement posted at the relevant notice board <strong>of</strong> the Embassy or Consulate dur<strong>in</strong>g<br />

ten days.<br />

If no reply is received to a request or summons, the applicant shall be deemed to have withdrawn his<br />

request and the resolution establish<strong>in</strong>g this to be the case shall be notified to him.<br />

Decisions on a <strong>visa</strong> application shall be notified with<strong>in</strong> one month <strong>of</strong> the date <strong>of</strong> the lodg<strong>in</strong>g <strong>of</strong> an<br />

application, except for residence <strong>visa</strong>s without work permit. In this case, decisions shall be notified<br />

with<strong>in</strong> three months, but the request for the residence authorisation that has to be issued by the<br />

competent Delegación or Subdelegación del Gobierno shall <strong>in</strong>terrupt the computation <strong>of</strong> this period<br />

<strong>of</strong> time until this authorisation is notified to the Embassy or Consulate.<br />

The <strong>visa</strong> once granted must be collected with<strong>in</strong> a month, unless it is a <strong>visa</strong> for family reunion or a<br />

<strong>visa</strong> for study purposes; <strong>in</strong> these cases, it must be collected before two months elapse. If a <strong>visa</strong> is not<br />

collected with<strong>in</strong> the aforesaid period, the applicant shall be deemed to have renounced to the <strong>visa</strong><br />

granted and the proceed<strong>in</strong>gs shall be filed.<br />

I am aware that the <strong>visa</strong> fee is not refunded if the <strong>visa</strong> is refused.<br />
