Use this checklist if:
Your UNIVERSITY (as shown on your Student ID) or your PERMANENT RESIDENCE (as shown on your Drivers
License) is located in Florida, Georgia or South Carolina.
You MUST submit your Student Visa application and documents listed in the checklist below, IN PERSON, to the Miami
Consulate between MAY 20 and JUNE 14, 2013 as it may take four to six weeks for the consulate to process your visa.

to the calendar of available appointments. Appointments fill up quickly, so please make your appointment EARLY!
Once your visa has been processed, it will be mailed back to you using a prepaid USPS Express Mail Envelope (details
included in checklist below). Other express delivery services, certified or registered mail are not accepted.
Important Notes:
• If you are an Academic Year student, or plan on staying for another semester (180+ days) please see additional
instructions (12-13) at the end of the checklist.
• If you plan on traveling outside the USA prior over the summer, please contact Lisa Anthony (lanthony@syr.edu)
immediately for additional instructions.
You must submit originals AND one photocopy of the following checklist items:
1. Original Passport AND Photocopy of Passport ID page: valid until at least June 2014 (fall students) and
December 2014 (yearlong students). Passports must have at least two blank pages to affix the visa. Passports
must be signed!
In addition to a national passport, Non-US citizens must present evidence of Immigration Status in
the US via the I-20 /IAP-66 or Alien Registration Card in and a permit to return to the United
States of America in the form of: a resident card, or a student/work visa that is stamped on the
passport and is valid for a period of three months beyond the expected length of stay in the
Schengen area, or the period of validity of the visa, whichever is longer. (N/A TO US CITIZEN).
*Photocopies required as well.
*Before submitting your passport, make an extra copy for your own records! If your passport
expires before June 2014 (fall students) and December 2014 (yearlong students), you will need to renew it
using an expedited service. Details of this can be found here:

2. Two National Application Forms (Original AND photocopy). Forms must be fully completed and
signed.

• Application Forms must be completed in blue or black ink, and in print (not cursive). Before
being copied, the application form must be signed on page 4 (please find application form instructions
below).
3. Proof of residence in Miami Consulate´s jurisdiction (FL, GA or SC): Original and photocopy of
College/University ID and of U.S. Driver’s License with current address.
4. 2 color passport sized photos: Passport photos can be taken at any Kinko’s, Walgreens, CVS, etc.
Photos must be RECENT with white background and professionally taken. Photos must be GLUED to
each application form in the top right corner. DO NOT STAPLE PHOTOS TO APPLICATION
FORMS!
5. Letter of acceptance as a full time student from Spain's University/School or US program: You MUST
contact Lisa Anthony at lanthony@syr.edu for these letters, at least two weeks prior to your visa
appointment, so they may be sent to you.
6. Information about the study abroad program. PLEASE contact Lisa Anthony for this information.
7. Proof of Health Insurance: Provide Original Letter guaranteeing full International Health insurance at
a minimum of €30,000 Euros, including the necessary Repatriation Coverage for the total period of
stay in Spain. Please contact Lisa Anthony, at least two weeks prior to your visa appointment, so
SU Abroad can provide you with this letter.
8. Proof of financial means during your stay: SU Abroad will provide the following:
• Letter from the University or School in Spain or in the USA assuming full financial responsibility
during your stay. Please contact Lisa Anthony for this letter, at least two weeks prior to your
visa appointment, so they may be sent to you.
9. Proof of accommodation in the form of:
• Letter from the study-abroad program specifying the address where the student will be staying. Please
contact Lisa Anthony for this information.
10. US Postal Service Money Order payable to the “Consulate General of Spain” for $160.00 (US citizens)
or $78.60 (most non-US citizens). For nationals of Russia, Macedonia, (ARYM), Montenegro, Serbia,
Albania and Moldavia the fee is $45.85. Do not submit cash, personal checks, or bank checks because
the Consulate will not accept them. These can be purchased at any US Post Office.
11. Pre-paid “US Express Mail” Envelope from the US Postal Service*, Self-Addressed with $19.95 in stamps. Once
the visa is affixed to your passport (visa processing time takes about 6 weeks), you can either go pick it up in
person or choose this option, where the Spanish Consulate will mail your passport back to you using this
envelope. In the addressee section of your mailing label, write the address where you expect to be over the
summer. In the “Sender” portion of the label, please write:

*Please, do not send any other type of envelope (e.g. US Priority, DHL, UPS, FedEx, etc.) as the
Spanish Consulate will not accept it.

Academic Year Students (180+ days) ONLY:
Medical Certificate (original and photocopy): Doctor’s recent statement (no older than 3 months) in doctor’s
letterhead or with medical center’s stamp, stating:
“This health certificate verifies that Mr./Ms ...... is free of drug addiction, mental illness and does not
suffer from any disease that could cause serious repercussions to public health according to the
specifications of the international sanitary regulation of 2005.
These contagious diseases include, but are not limited to smallpox, poliomyelitis by wild poliovirus, the
human influenza caused by a new subtype of virus and the severe acute respiratory syndrome (SARS),
cholera, pneumonic plague, yellow fever, viral hemorrhagic fevers (e.g.: Ebola, Lassa, Marburg), West
Nile Virus and other illnesses of special importance nationally or regionally (e.g.: Dengue Fever, Rift
Valley Fever and meningococcal disease.)
Ms./Mr. ______________ is a very healthy individual in all senses, she/he has no pre-existing medical
conditions, and she/he is capable of traveling abroad.”.
* The medical certificate must be signed by a M.D.
*The original must be translated into Spanish and submitted along with a photocopy,
contact a legal translator.
12. Certification of Absence of Police Records (original and photocopy): FBI or State Police Criminal
Background Check legalized with the “Apostille of the Hague Convention” from the U.S. A police
record must be obtained (criminal background check) from all non EU countries where you have lived
for the past five years for more than 6 months, legalized by the Spanish Consulate or Embassy in the
respective countries. If the Certificate is issued outside the USA it must be duly legalized by the
corresponding Consulate of Spain. LOCAL POLICE BACKGROUND CHECK WILL NOT BE
ACCEPTED. Original translated into Spanish and one copy will be needed (contact a legal translator).
*The original must be translated into Spanish and submitted along with a photocopy,
contact a legal translator.
*FBI background checks can take 8-12 weeks to get back from the FBI.

Any personal data concerning me which appear on the visa application form, as well as my
photograph, will be supplied to the relevant authorities and processed by those authorities, for the

I declare that to the best of my knowledge all particulars supplied by me are correct and complete. I
am aware that any false statements shall lead to my application being rejected or to the annulment of
a visa already granted.
I am aware that once the visa request has been submitted, a copy of the form, sealed with the
indication of the date and place of reception, shall be returned to the applicant and that arrangements
can be made with the applicant as to the channel to request corrections or the supply of documents or
certificates, as well as to serve notice of summons and to notify the decision adopted in due course.
Notices and requests shall be served by telephone or fax to the contact number provided by the
interested party or his/her legal representative. If they were to prove ineffective, they shall be served
in writing to the domicile set out in the application, which must be located within the consular
district.
Summons and requests served must be complied with within a period not exceeding ten days, unless
personal attendance is required in which case the deadline is fifteen days.
Once all the possibilities of serving a notice have been exhausted, the notice shall be served by
means of an announcement posted at the relevant notice board of the Embassy or Consulate during
ten days.
If no reply is received to a request or summons, the applicant shall be deemed to have withdrawn his
request and the resolution establishing this to be the case shall be notified to him.
Decisions on a visa application shall be notified within one month of the date of the lodging of an
application, except for residence visas without work permit. In this case, decisions shall be notified
within three months, but the request for the residence authorisation that has to be issued by the
competent Delegación or Subdelegación del Gobierno shall interrupt the computation of this period
of time until this authorisation is notified to the Embassy or Consulate.
The visa once granted must be collected within a month, unless it is a visa for family reunion or a
visa for study purposes; in these cases, it must be collected before two months elapse. If a visa is not
collected within the aforesaid period, the applicant shall be deemed to have renounced to the visa
granted and the proceedings shall be filed.
I am aware that the visa fee is not refunded if the visa is refused.
