<strong>The</strong> <strong>Voice</strong> <strong>of</strong> <strong>Nursing</strong> Winter/Spring 2013<br />
As you all know, <strong>St</strong>. Mary’s is on a journey to an Electronic<br />
Health Record (EHR) through the QUEST program. By<br />
implementing an EHR at <strong>St</strong>. Mary’s <strong>Medical</strong> <strong>Center</strong>, we are<br />
building upon and enhancing the high-quality care you provide<br />
to our patients each day. By deploying QUEST we are able to<br />
integrate more closely with <strong>St</strong>. Vincent Health and draw upon<br />
their experience and expertise with the program.<br />
This journey involves significant effort and transformation for<br />
all clinicians at <strong>St</strong>. Mary’s. <strong>The</strong> new QUEST functionality<br />
involves rethinking workflow and existing ways <strong>of</strong> caring for<br />
our patients. We are currently reviewing the workflow changes<br />
that are necessary as we adapt to the new system, and we need<br />
your help and input! As you review workflow in your workout<br />
teams, please <strong>of</strong>fer feedback to your Manager/Director.<br />
We want to hear from you on how we can approach this in a<br />
manner that will work for our clinicians.<br />
Also, please know that we understand the amount <strong>of</strong> time and<br />
focus that QUEST training will require <strong>of</strong> our Associates. It<br />
is with that in mind that we have engaged AMN Healthcare<br />
to provide us with travel nurses during the training period and<br />
go-live. We want to ensure you are able to focus on becoming<br />
comfortable in this new system and bringing that value back to<br />
your patients, physicians and colleagues.<br />
Implementing an EHR is no small task, and I would like to<br />
call special attention to Jeanne Braun, Executive Director<br />
<strong>of</strong> Clinical Informatics, and also our Clinical Informaticists,<br />
Melanie Daisey, Kristie French, Tricia Johnson and Ginger<br />
Kreyling for the vital role they each play in leading and<br />
preparing us for our successful implementation <strong>of</strong> QUEST. I<br />
want to thank them for their dedication to this project and for<br />
all their time, energy and efforts. In addition, I would like<br />
to recognize Tracy MacGregor, Patsy Kietzman, Carol<br />
Dobbins and our Clinical Nurse Specialists for the valuable<br />
support they continue to provide to our Clinical Informaticists.<br />
This is an exciting time for us at <strong>St</strong>. Mary’s, and I want to<br />
thank all <strong>of</strong> you for your time and commitment<br />
in working through this new system in a spirit <strong>of</strong><br />

PDQ: Delirium<br />
Joanna Emmons, MSN, RN, ACNS-BC, CCNS, CCRN<br />
<strong>The</strong> Adult Critical Care Units have adopted the<br />
Confusion Assessment Method for the ICU (CAM-ICU)<br />
tool to begin screening for delirium in all critically ill<br />
patients. Delirium is very prevalent in the critical care<br />
setting, with 50-80 percent <strong>of</strong> mechanically ventilated<br />
patients experiencing delirium. Additionally, it is<br />
documented that 20-50 percent <strong>of</strong> lower severity ICU<br />
patients will also develop delirium.<br />
Delirium leads to increased mortality, longer hospital<br />
stay, poorer recovery, higher healthcare costs and<br />
long-term neurocognitive problems. <strong>The</strong> literature<br />
demonstrates each day a patient spends delirious leads to<br />
a 10 percent increase risk <strong>of</strong> death.<br />
Dr. Anthony Uvieghara, Pulmonologist and Critical<br />
Care Medicine physician, has taken the lead on a<br />
Physician Driven Quality (PDQ) initiative to focus on<br />
assessment, prevention and management <strong>of</strong> delirium in<br />
the hospitalized setting.<br />
<strong>The</strong> PDQ Team has made several changes in the critical<br />
care setting to facilitate delirium prevention strategies.<br />
All critically ill patients will be screened for delirium at<br />
least once per shift. A bundled strategy has been adopted<br />
to help prevent and/or break the cycle <strong>of</strong> delirium.<br />
<strong>The</strong> ABCDE Bundle is an evidence-based, organizational<br />
approach to the management <strong>of</strong> acutely ill patients and<br />
includes the following components:<br />
ABC – Awakening and Breathing Trial Coordination<br />
D – Delirium Assessment and Management<br />
E – Early Exercise and Progressive Mobility<br />
Each piece <strong>of</strong> the ABCDE Bundle is supported by<br />
independent study and evidence-based practice. <strong>The</strong> goal<br />
<strong>of</strong> the bundle approach is to promote collaboration among<br />
members <strong>of</strong> a multidisciplinary team. This collaboration<br />
encourages ongoing communication, helps implement<br />
standardized care processes and breaks the cycle <strong>of</strong><br />
suboptimal sedation practices. Each component <strong>of</strong> the<br />
bundle includes assessments and interventions designed<br />
to prevent or minimize the unintended consequences <strong>of</strong><br />
our patient care.<br />

Spirituality in the Workplace<br />

As a Catholic faith-based organization, <strong>St</strong>. Mary’s Health<br />
is privileged to incorporate Spirituality in the Workplace as<br />
part <strong>of</strong> our Caring and Healing Environment. We see our<br />
work not just as a job, but as a ministry <strong>of</strong> care and healing<br />
for persons who are ill, injured and seeking to maintain<br />
good health. Our nurses and patient care techs are essential<br />
persons who live and share our ministry and spirituality with<br />
those we serve every day.<br />
As we incorporate Spirituality in the Workplace, inherited<br />
from our Founders the Daughters <strong>of</strong> Charity, we seek<br />
to sustain the culture and practices which support the<br />
spiritual, physical, emotional and mental well-being <strong>of</strong> our<br />
associates, physicians and volunteers, as well as our patients<br />
and visitors. We live and express our own individual<br />
spiritualities while embracing the corporate spirituality <strong>of</strong><br />
<strong>St</strong>. Mary’s Health.<br />
We respect and honor the faith traditions <strong>of</strong> all persons we<br />
serve and serve among. We begin all meetings and large<br />
group gatherings with prayer and reflection, asking God’s<br />
guidance and direction as we seek to serve with excellence<br />
and compassion. A brief Daily Reflection and Prayer, a<br />
Monthly Gathering Prayer with Reflection Questions and<br />
resources celebrating our history and heritage are sent by<br />
e-mail to all associates for use during staff meetings or for<br />
personal prayer as they desire. We provide for the spiritual<br />
and emotional needs <strong>of</strong> persons <strong>of</strong> all faith traditions, as<br />
well as those who may not be involved with any particular<br />
faith community as we serve them.<br />
We believe that Spirituality in the Workplace includes<br />
honoring and respecting the presence <strong>of</strong> God within<br />
oneself, all those we serve and each <strong>of</strong> our co-workers. <strong>The</strong><br />
customs, practices and resources mentioned here are just<br />
a few <strong>of</strong> the ways we incorporate and promote Spirituality<br />
in the Workplace in order to integrate a spirit <strong>of</strong> reverence<br />
and respect for all persons in our Caring and Healing<br />
Environment. I give thanks to all our nurses, patient care<br />
techs and all who share this spirit <strong>of</strong> reverence, respect and<br />
personal caring as you touch the ill and injured whom we<br />

Opportunities Recruitment<br />
Event at <strong>St</strong>. Mary’s Manor<br />
Auditorium for local<br />
graduating nursing students.<br />
Over 50 nursing students<br />
braved the wintry weather to<br />
attend. Attendees were from a variety <strong>of</strong> schools, including<br />
Henderson Community College, Ivy Tech Community<br />
College, the University <strong>of</strong> Evansville and the University <strong>of</strong><br />
Southern Indiana.<br />
<strong>The</strong> event provided students<br />
with the chance to speak<br />
with <strong>St</strong>. Mary’s Nurses<br />
about career opportunities at<br />
the hospital in an informal<br />
setting. An amazing dinner<br />
was provided by <strong>St</strong>. Mary’s<br />
Food Services & Catering<br />
staff for all in attendance.<br />
<strong>St</strong>aff Nurses, Charge Nurses, Directors, Executive Directors<br />
and our Chief <strong>Nursing</strong> Officer, Darcy Ellison, were all<br />
available to answer questions, as were members from our<br />
Human Resources & Development <strong>Nursing</strong> Recruitment<br />
Team. Fabulous door prizes were awarded to students,<br />
including a Wii console, an Amazon Kindle Fire, a Keurig<br />
C<strong>of</strong>feemaker, Apple iPods and a flat screen TV! Each<br />
student received a <strong>St</strong>. Mary’s long-sleeved t-shirt and many<br />
won <strong>St</strong>. Mary’s hooded sweatshirts in the drawings.<br />

<strong>Medical</strong> Services Bedside Shift Report American Idol Contest<br />
Billie Silkwood, RN<br />
In October, there was a discussion among the 6South/2West staff <strong>of</strong> ways to<br />
improve Bedside Shift Report. Most felt an education class was not appropriate<br />
as they were all taught the same way and knew how to give the report. However,<br />
emphasis did need to be placed on certain aspects <strong>of</strong> the report, including updating<br />
the white board, including the patient in the report, asking if the patient has any<br />
questions, avoiding reporting “normals,” reporting core measure needs and being<br />
concise and organized. To address these areas, Billie Silkwood, RN, came up<br />
with an American Idol Contest idea in November. She devised a score sheet that<br />
included all <strong>of</strong> the areas <strong>of</strong> Bedside Shift Report with a point system for each.<br />
Beginning late November, all <strong>of</strong> the 6 South/2 West RNs were given the Bedside<br />
Shift Report Competency Form to review and practice. In addition to Billie,<br />
three other judges, made up <strong>of</strong> staff nurses, were chosen. For a two-week period<br />
beginning early December, the four judges observed and scored each <strong>of</strong> the RN<br />
contestants.<br />

On December 19, the winners were announced. Our first place winner, Angie Gores, RN, received a gift certificate for a<br />
smoked ham. Second place winner, Chardae Chambers, RN, BSN, received a gift certificate for barbequed ribs. Our third<br />
place winner, Kim Hughes, RN, and fourth place winner, Shane Hayes, RN, each received a pen light.<br />
We would like to thank all <strong>of</strong> the nurses on 6 South/2 West for participating in this activity. We immediately saw an<br />
improvement in our bedside shift reports! This was an activity initiated, judged and carried out by our RNs on 6 South/<br />
2 West. This is a wonderful example <strong>of</strong> our nurses working together to improve an important aspect <strong>of</strong> their role.<br />
<strong>Nursing</strong> Clinical Ladder Committee Hosts<br />
“Heart to Heart” Help Sessions<br />

<strong>The</strong> <strong>Nursing</strong> Clinical Ladder Committee spent Valentine’s Day spreading<br />
love and knowledge about <strong>St</strong>. Mary’s <strong>Nursing</strong> Clinical Ladder (NCL). <strong>The</strong><br />
NCL “Heart to Heart” Help Sessions were held in the GIFT Conference<br />
on February 14, 2013. <strong>Nursing</strong> Clinical Ladder Committee members<br />
answered questions, provided ideas and gave support to nurses interested<br />
in participating in the NCL Program. <strong>The</strong> <strong>Nursing</strong> Clinical Ladder,<br />
formerly known as CAP: Career Advancement Program, provides flexibility<br />
and formal recognition for Registered Nurses interested in developing<br />
their pr<strong>of</strong>essional nursing practice by <strong>of</strong>fering three levels <strong>of</strong> monetary<br />
recognition. This program provides a formal process that promotes the<br />
pr<strong>of</strong>essional development <strong>of</strong> registered nurses through evidence-based<br />
nursing care.<br />

DAISY Award Winner!<br />
Nurses’ passion, dedication and skills are vital to the patients and families<br />
that spend any length <strong>of</strong> time at <strong>St</strong>. Mary’s. This is why we are committed<br />
to recognizing those who inspire this commitment. <strong>The</strong> Recruitment,<br />
Retention and Recognition Council is continuously searching for additional<br />
ways to recognize exceptional <strong>St</strong>. Mary’s Nurses. In this search, the council<br />
discovered the DAISY Award.<br />
<strong>The</strong> DAISY Award was established by the DAISY Foundation in<br />
memory <strong>of</strong> J. Patrick Barnes who died at the age <strong>of</strong> 33 <strong>of</strong> idiopathic<br />
thrombocytopenia, an auto-immune disease. <strong>The</strong> Barnes Family was<br />
awestruck by the clinical skills, caring and compassion <strong>of</strong> the nurses who<br />
cared for Patrick, and created this national award to say thank you to nurses<br />
everywhere. DAISY stands for Diseases Attacking the Immune SYstem.<br />
<strong>St</strong>. Mary’s is proud to recognize this quarter’s DAISY Award recipient, Lillybeth Cassel, BSN, RN. Lillybeth is a nurse on<br />
6 West: Renal/Diabetes. Lilly was nominated by numerous fellow associates. Her nominators stated,<br />
“Lilly is a great example <strong>of</strong> a nurse who collaborates with other team members and provides quality care to her<br />
patients. I have never worked on the same floor as Lilly but have seen her compassion and dedication first hand. She<br />
not only strives for great patient care, she also genuinely cares for her co-workers and other associates within the<br />
hospital. She always has a smile on her face and is always positive. One example is after a particularly hard day on<br />
my unit, Lilly stopped by after her shift (on another unit) to tell me she appreciates us and all we do. She works for<br />
the better good <strong>of</strong> the Hospital. Lilly is the ultimate example <strong>of</strong> a role model, advocate and outstanding employee.”<br />
“Lilly is the epitome <strong>of</strong> what every <strong>St</strong>. Mary’s Nurse should strive to be. I have seen her go the extra three miles to<br />
get the patient the care they need. I have seen her use her bilingual talents to calm a patient when they didn’t trust<br />
the blue phone translator’s voice. I have so much respect for her. She brings love, caring and the ability to carry out<br />
all nursing duties with respect for the patients and staff she works with.”<br />

We were fortunate to have Dr. Timothy Girard, Assistant Pr<strong>of</strong>essor <strong>of</strong> Medicine in the Division <strong>of</strong> Allergy, Pulmonary and<br />
Critical Care Medicine at the <strong>Center</strong> for Health Services Research at Vanderbilt University School <strong>of</strong> Medicine, present at<br />
our most recent Grand Rounds. His presentation was video taped and can be viewed on distance education.<br />

<strong>St</strong>. Mary’s <strong>Nursing</strong> is continually looking for ways to retain and recognize our nurses. One method that we use to gauge<br />
associate satisfaction is the RN Satisfaction Survey. This September, over 82 percent <strong>of</strong> <strong>St</strong>. Mary’s nurses took an online<br />
survey that focused primarily on their Practice Environment. Based on the results <strong>of</strong> the survey, members <strong>of</strong> <strong>Nursing</strong><br />
Leadership work with bedside nurses to create action plans to improve the RN experience. Each year, <strong>Nursing</strong> Excellence<br />
recognizes the departments with the best and most improved scores. Please congratulate the following departments on their<br />

<strong>The</strong> “6 East Transfuses Love” board at the 6 East nurses station was created by our weekend-option <strong>St</strong>udent Nurse/Unit<br />
Assistant, Bethany Kramer. She changes the board for every season and/or holiday. At Christmas, the board was a snow<br />
man made out <strong>of</strong> cotton balls that said, “<strong>The</strong>re’s snow place like 6 East.” She is very creative and comes up with some great<br />
ideas. She also had a fall-themed board. Her theme for February (Valentine’s Day) is perfect because <strong>of</strong> the love we impart<br />
to our patients and families as many <strong>of</strong> our patients receive transfusions.<br />
Bethany also coordinated the Christmas family donations for 6 East Oncology this year. Bethany collected money from staff<br />
which we were able to use to get every family member several gifts from local department and grocery stores. She gathered<br />
and wrapped a lot <strong>of</strong> gifts - at least 50 boxes! After wrapping, Bethany packaged all the gifts and got them ready to be<br />
shipped. She truly exercised Relationship-Based Care by taking on this task for this family.<br />
Bethany is creative, thoughtful and always thinks <strong>of</strong> others. Because <strong>of</strong> her creativity, she keeps staff motivated and smiling.<br />
<strong>St</strong>aff, patients and visitors constantly comment on her “6 East Transfuses Love” board. She also keeps crayons and coloring<br />
paper at the nurses’ station for anyone to color “love-themed” pictures to post. Bethany is an outstanding STAR!!!<br />

Larissa Mendieta<br />
Nurses’ passion, dedication and skills are vital to the patients and families that spend<br />
any length <strong>of</strong> time at <strong>St</strong>. Mary’s. This is why we are committed to recognizing those<br />
who inspire this commitment. <strong>The</strong> Recruitment, Retention and Recognition Council is<br />
continuously searching for additional ways to recognize exceptional <strong>St</strong>. Mary’s nurses.<br />
In this search, the council discovered the DAISY Award.<br />
<strong>The</strong> DAISY Award was established by the DAISY Foundation in memory <strong>of</strong> J. Patrick<br />
Barnes who died at the age <strong>of</strong> 33 <strong>of</strong> idiopathic thrombocytopenia, an auto-immune<br />
disease. <strong>The</strong> Barnes Family was awestruck by the clinical skills, caring and compassion <strong>of</strong> the nurses who cared for Patrick,<br />
and created this national award to say thank you to nurses everywhere. DAISY stands for Diseases Attacking the Immune<br />
SYstem. <strong>St</strong>. Mary’s is proud to recognize this quarter’s DAISY Award recipient, Larissa Mendieta, BSN, RN. Larissa is a<br />
nurse in the Emergency Department. Larissa was nominated by a peer. Her nominator stated:<br />
“Larissa has a mothering heart for her patients and her coworkers. Speaking <strong>of</strong> one specific occasion when Larissa<br />
was really shining, she had a group <strong>of</strong> children as her patients. She called other departments to find clothing for them,<br />
and colored and played with them. Larissa even took the young girl and gave her a bath and washed her hair all<br />
while waiting for the foster parents to be ready for the children. She held the little girl and cared for them as her own,<br />
providing them with food and even giving them some <strong>of</strong> her own to care for them at this extremely difficult time in<br />
their young lives. By the time it was time for them to leave, the children didn’t want to leave, especially the youngest<br />
that had become quite attached to Larissa after three hours with her. After carrying the child to the car, strapping her<br />
in and telling them both goodbye, she returned and sat at the nurse’s station tearful at the situation the children were<br />
dealing with. It was very sensitive for her. She treats all her patients with this kindness.”<br />

United Kingdom <strong>Nursing</strong> <strong>St</strong>udents Visit <strong>St</strong>. Mary’s <strong>Medical</strong> <strong>Center</strong><br />

On March 19, four nursing students from Queen Centre Hospital in Nottingham,<br />
England spent the day at <strong>St</strong>. Mary’s to learn about nursing care delivery at our facility.<br />
<strong>The</strong> students are participating in a month-long visit to the University <strong>of</strong> Evansville’s<br />
<strong>Nursing</strong> Program. During the program, these third year students will observe clinical<br />
rotations, tour local facilities as well as attend nursing classes and conferences.<br />
<strong>The</strong> students’ day at <strong>St</strong>. Mary’s began with a Welcome and Invocation by Darcy<br />
Ellison, MSN, RN, NEA-BC, Chief <strong>Nursing</strong> Officer/Senior Vice President <strong>of</strong> Clinical<br />
Operations. Sheila Hauck, DNP, RN, NEA-BC, Executive Director <strong>of</strong> Pr<strong>of</strong>essional<br />
Practice & Oncology, gave the group an Introduction to <strong>Nursing</strong> Excellence, Magnet<br />
Designation and Relationship-Based Care. <strong>The</strong> students were then led on a guided tour <strong>of</strong> Women & Children’s Services by<br />
Ashly Fisher, BSN, RN, CLS. Ashly is the Women & Children’s <strong>Nursing</strong> Pr<strong>of</strong>essional Development Specialist. During<br />
lunch, the nursing students participated in discussions with an expert panel that included nurses from Pr<strong>of</strong>essional Practice,<br />
<strong>Medical</strong>/Surgical Services, Cardiovascular Services and Critical Care. In addition, the students took part in a tour <strong>of</strong><br />
<strong>St</strong>. Mary’s campus, complete with a performance by Cyndi Bickel on the harp and our wonderful Zither ensemble. This<br />
