“A RESOLUTION AUTHORIZING THE CITY MAYOR, HON. REY T. UY TO SIGN FOR AND IN BEHALF OF THE
CITY GOVERNMENT OF TAGUM, A CONTRACT OF TRAINING AND AFFILIATION WITH SAINT MARY’S
COLLEGE TAGUM CITY”
WHEREAS, Ms. Aida C. Hangad , RN, MAN, Program Head Nursing of Saint Mary’s College
has endorsed the contract of Training and Affiliation between the City of Government of Tagum
and Saint Mary’s College – Tagum City to this August Body dated August 30, 2008.
WHEREAS, there is a need to expose nursing students in the promotion, maintenance and
delivery of health-related services in as much as our health offices are directly engaged in
rendering health-related services to the community;
WHEREAS, this august body sees the need for such contract of affiliation in order to help
nursing students gain enough knowledge and experiences in the field of medical services and to
help address the need of our constituents;
NOW THEREFORE, BE IT RESOLVED BY THE Sangguniang Panlungsod of Tagum to authorize the
City Mayor, Hon. Rey T. Uy to sign for and in behalf of the City Government of Tagum a contract of
training and affiliation with Saint Mary’s College Tagum City;
RESOLVED FURTHER, that the full-text of the contract of Affiliation is as follows, to wit:
KNOW ALL MEN BY THESE PRESENTS:
CONTRACT OF AFFILIATION
This CONTRACT OF AFFILIATION, made and entered into in Tagum City, Philippines, this ______
day of ________________, _____ by and between:
The CITY GOVERNMENT OF TAGUM, duly represented by HON. REY T. UY herein referred to on
the PARTY OF THE FIRST PART;
and
ST. MARY’S COLLEGE School/College/ University with business and postal address at National
Highway, Tagum City, a duly recognized institution by the Commission on Higher Education
represented by S. MA. FE D. GERODIAS, RVM as authorized by the school’s Board of Trustees per
Corporate Resolution No. 01, s-2001, hereinafter referred to as the PARTY OF THE SECOND PART.

1. The PARTY OF THE FIRST PART agrees and allows the affiliation and training of Nursing
students of the PARTY OF THE SECOND PART subject to the Policies, Procedures and
Guidelines Governing Affiliation and Training of Students in all accredited DOH hospitals
and government health facilities or also known as Administrative Order No. 5-A series of
1996. Likewise, the PARTY OF THE SECOND PART shall strictly comply with the provisions of
the above Administrative Order and any violation thereof shall be legitimate ground for
cancellation of the contract.
WHEREAS, Sec. 2 of the above Administrative Order states that the Department of Health is
making available its hospitals and accredited government community health resources for the
learning and training of students from health professionals schools.
WHEREAS, Sec. 9 of the above Administrative Order provides the Standard Rates of
Affiliation Fees in connection with the aforementioned undertaking as follows;
9.17 Nursing P60.00/student for 50-80 hours
P40.00/student for 30-49 hours
P30.00/student for 10-29 hours
P20.00/student for 1-9 hours
WHEREAS, such collection of affiliation fees shall be remitted to the City Treasurer’s Office
which the same shall be recorded as affiliation fees income under miscellaneous and operating
services of the general fund;
WHEREAS, in reference with Section 10 of Administrative Order No. 5-A, the maintenance
and operating expenses such as but not limited to meetings, supplies and materials,
honorarium/incentives for health personnel/committee members involved in the program shall be
prioritized and charged to this affiliation fees income;
WHEREAS, in the event of conflicts regarding affiliation training of students the City Health
Officer has the authority to resolve the issue within his/her level in accordance with the
Administrative Order;
WHEREAS, Sec. 12 of the above Administrative Order likewise cited the Special Provision as
follows:
“Other government-owned or controlled schools or colleges shall be required to pay the
affiliation fees to the Rural Health Units/Health Centers except the University of the Philippines.”
2. The term of this contract is from June 1 , 20 07 to July 31, 20 09.
