The program provides opportunities for improving the health and well-being of the children served, and supporting permanency planning. In the program children are served in the least restrictive, most-like setting possible, involving those in the caregiving network - foster family, birth family, and adoptive family members. The WSP will provide a variety of support services to children and their family including counseling, intensive in-home supports, planned or crisis respite (for developmentally disabled children), crisis avoidance and management, vocational training and employment placement for youth, skill building, community advocacy, day habilitation and immediate crisis response. The WSP will attend meeting to discuss the creation of an Individual Health Plan(IHP) for each participant.
