Local Health Department Clinic now called Community Health and Wellness Center The Health Department Clinic is now the Original Community e-mail Health and Wellness Center. The center provides comprehensive primary and preventive care. The Health and Wellness Center will operate out of the Wood County Health District at 1840 East Gypsy Lane Road, in Bowling Green. Satellite locations in North Baltimore, Northwood, Perrysburg, and Bradner also are available. Services include primary care, preventive health services, immunizations, prenatal care, women’s health care, sexually transmitted infection testing, pediatrics, senior and men’s health care. The center employs two nurse practitioners with physician oversight, three skilled nurses and four clinic assistants. Patients also have access as needed to a pharmacist and social worker. The facility is open to residents of all ages, regardless of their ability to pay or health insurance status. Fees are based upon income. Effective November 1, a $10 service fee is requested from all patients to help offset the cost of care. The center is accepting new patients including those with or without insurance, Medicaid and Medicare recipients. Obituary •LUCILLE KOLASINSKI Lucille J. (Voland) Kolasinski, 96, lifelong resident of Perrysburg Township, died Thursday, December 13, 2012, in her home. She was born in Perrysburg, on June 21, 1916, to Conrad and Ada (Lober) Voland. She married Daniel Kolasinski on June 7, 1942, in Toledo. She was a 1934 graduate of Perrysburg High School and graduated in 1939 from the Riverside School of Nursing. She worked for more than 50 years as a private duty registered nurse, retiring at the age of 72. Ms. Kolasinski is survived by her children, James “Mike” (Janet) Kolasinski of Rossford, Martin (Karen) Kolasinski of Walbridge, Kristine (William) Miller of Perrysburg, Anna Mae Hemingway of Perrysburg, Rose E.L. Noland of Des Moines, Iowa, and Sally Meier of Bowling Green, Ohio; 21 grandchildren and 18 great-grandchildren; brother, John Voland of Perrysburg; several nieces and nephews. She was preceded in death by her husband, Daniel, on April 19, 1989. She also was preceded in death by her brother, Christian “Bub” Voland; sisters, Anna Mae Kohl, Helen Clark, Elizabeth Woolley and Alice Vanarsdall, and son-in-law, Richard Meier. Private graveside services will be held in Fort Meigs Cemetery. There will be no visitation. Arrangements were made by the Witzler-Shank Funeral Home, Perrysburg. Area residents create Manor House display honoring Dionne Quintuplets Perrysburg residents Louise Brubaker and Julie Overy decorated the Cedar Closet in the Manor House at Wildwood Park. They are both members of the Toledo Mothers Of Twins Club and Ms. Brubaker also is a member of the Dionne Quintuplet Collectors Club. The display, “A Dionne Quintuplet Christmas” was provided by Ms. Brubaker’s personal collection. Two other Twins Club members, Joann Miller-Davis and Beverly Burzynski, also help with decorating this area. The Dionnes are the world’s only set of all identical quintuplets in the world and they are now 78 years old. Two of them, Cecile and Annette, are still living near Montreal, Canada. Emilie died at age 20, Marie at age 36, and Yvonne at age 67. Before the quints’ births, their mother had given birth to six children, then had the quints, which made her a mother of 11 by the age of 25. She gave birth to three other children following the quints. The family lived in a farm house which had no insulation, no electricity and no plumbing. Large boxes may be dropped off at the Department of Public Service, 11980 Roachton Road or 211 East Boundary during regular business hours of Monday through Friday, 7:00 a.m. – 3:30 p.m. (excluding holidays). • Special Cardboard Recycling Christmas Hours—will be available at the Department of Public Service, 11980 Roachton Road from 9 a.m.-12 noon on Saturday, December 29, 2012. • Cell phones—may be recycled at Pak Mail, Way Public Library and cell phone stores. • Christmas Tree Curbside Recycling Collection—will be Wednesday, January 2 through Friday, January 11. During the week, residents must check in at the Department of Public Service and on Saturdays go directly to the City Compost Site. • Christmas tree lights—may be recycled at the Department of Public Service Building located at 11980 Roachton Road during regular business hours of Monday through Friday 7:00 a.m. – 3:30 p.m. (excluding holidays) during the months of December and January. • Computers—may be recycled (working or nonworking computers and parts) year round at the Goodwill Donation Station located off North Dixie Highway in the plaza across from Walt Churchill’s Market. • Gift Wrapping Paper—may be recycled curbside. Only paper gift wrap may be recycled. Paper gift bags can also be recycled. PLEASE do not place gift wrap paper in plastic bags. You may place gift wrapping paper in cardboard boxes or paper bags. NO ribbon, bows, foil or Mylar paper can be recycled. • Greeting Cards and Envelopes—may be placed in a paper bag, set out for curbside recycling with gift wrapping paper, newspapers, magazines, catalogs, telephone books and junk mail. • Packing Peanuts—may be dropped off at Pak Mail in Perry’s Junction, the UPS store on Route 20 by Taco Bell and, at the e-Bay Store in Commodore Way on Louisiana Avenue. • Plastic Toys—may be recycled by participating in the Levis Commons “Play it Forward” program. On Saturday, January 5, 2013, take clean used plastic toys to the movie theatre entrance to be donated to local agencies. Large toys like ride on toys, play houses, outside play equipment are welcome. No stuffed animals or books are accepted.
