<strong>Replacement</strong> <strong>of</strong> <strong>Testamur</strong> / <strong>Certificate</strong><br />
My testamur has been lost or damaged. How do I go about getting a<br />
new/replacement testamur<br />
Download the <strong>Replacement</strong> <strong>Testamur</strong> Request Application form from the Student<br />
Administration Forms website.<br />
The form will outline the required documents and the cost applicable to your<br />
application.<br />
You must also submit a Statutory Declaration from Commissioner <strong>of</strong> Oath, a police<br />
report on the loss <strong>of</strong> your <strong>Testamur</strong> and payment.<br />
If damaged, you must bring along a Statutory Declaration from Commissioner <strong>of</strong><br />
Oath, a police report, return your damaged <strong>Testamur</strong> / <strong>Certificate</strong> and payment along<br />
with your request.<br />
The Statutory Declaration must state your name, student ID, address, phone contact,<br />
title <strong>of</strong> course for the required testamur. You must also outline the circumstances as<br />
to why you need a replacement testamur.<br />
I have changed my name since I graduated. Can I get a new testamur showing<br />
my new name<br />
Yes. You will need to send Awards, Ceremonies and Examinations the following<br />
documents:<br />
• A completed <strong>Replacement</strong> <strong>Testamur</strong> Request Application form, found on<br />
the Student Administration Forms website.<br />
• A certified statutory declaration, detailing the reason that a replacement<br />
testamur is to be issued.<br />
• A certified copy <strong>of</strong> your change <strong>of</strong> name certification, eg change <strong>of</strong> name<br />
certificate, wedding certificate, Identity Card, etc.<br />
• The required payment<br />
• A short letter outlining any other information not covered by the statutory<br />
declaration and requesting a replacement testamur due to change <strong>of</strong> name.<br />
Please note the replacement testamur will have a statement on the bottom that<br />
shows it is a replacement certificate along with the date that the original was issued<br />
and will show the signatures <strong>of</strong> the current <strong>University</strong> senior <strong>of</strong>fice bearers.
