support to assist frail elderly and<br />
younger people with disabilities and<br />
their carers. The services provided<br />
depend on the needs of the client<br />
or carer ranging from assistance<br />
with daily living, general household<br />
tasks, transport, in home respite,<br />
personal care, social support, allied<br />
health including counselling, social<br />
work and client education and other<br />
tasks in response to individual<br />
needs. Eligibility: Persons in the<br />
target group at risk of premature<br />
or inappropriate placement in<br />
institutional care – screened using<br />
the HACC ONI Tool.<br />
