How We’re Doing: Heart Failure At SSM Health Care, our Mission is at the heart of everything we do. We say that “through our exceptional health care services, we reveal the healing presence of God.” But how can we tell how well we are living our Mission We measure three things: exceptional patient care, exceptional commitment from our employees and physicians, and exceptional financial performance and growth. In each issue of OptimiSSM, we’ll highlight the results from one of these areas to let you know how we’re doing. At SSM, we focus on results in four key clinical areas to determine how well we are accomplishing our Mission of “exceptional patient care.” These are: heart failure (HF), acute myocardial infarction (AMI), pneumonia (PN), and surgical care improvement project (SCIP). These are also the quality of care measures that are shown on Hospital Compare, a Web site that was developed by both the Centers for Medicare & Medicaid Services (CMS) and Hospital Quality Alliance: Improving Care Through Information (HQA). This online tool shows how often hospitals give the recommended treatments known to get the best results and allows patients to compare hospitals. Research has shown that these treatments provide the best results for most patients with those conditions and are an important part of the patients' overall care. Collaborative teams at each of our hospitals make sure that we use this research to continually improve the care we provide to our patients. For example, in the area of Heart Failure, we know that patients and their families need very specific discharge instructions about their care before they can go home from the hospital. Our caregivers strive to make sure that every patient understands his or her dietary restrictions, recommended activity levels, what prescription medications have been prescribed, and signs and symptoms of worsening heart failure. Consistent delivery of discharge instructions is just one of many specific care processes for heart failure patients that collaborative teams throughout SSM Health Care focus on constantly.
