loved ones and educate healthcare professionals about the benefits of end-of-life care. Each
of our programs is available to all who need them, regardless of their ability to pay.
At Haven, we believe hospice is about living and that no one
should suffer.

support, our goal is to help our patients and their families focus on living. We are dedicated to
providing the care and support that meets the needs of our patients and their families.

At Haven Hospice, our staff and volunteers are passionate for
what we do and how we do it. Our staff and volunteers are
committed to meeting the needs of those in our communities
who are faced with life-limiting illness and loss. Our staff and
volunteers live each day with the belief that everyone deserves
comfort, care and compassion.
Haven has been doing that every day over the last 33 years
by providing a variety of programs, services and support to the
individuals and families for which we have been privileged to
provide care.
But our staff and volunteers have not done it alone. Without
you, our generous donors, we would not have the resources to
provide the scope of programs and services that we do. I cannot
begin to express how much we appreciate what you all have

PRIVILEGED TO SERVE
In 2011, Haven provided direct care to 4,854 individuals facing life-limiting illness and
supported their loved ones through this process. In addition, we provided counseling and
grief support services to 3,339 individuals and families in the communities we serve.
• We provided significant relief of pain and other symptoms.
• We gave our patients’ families the support and resources they needed.
• We provided counseling and support groups for grieving individuals and families.
• We served children who experienced or needed to prepare for the loss of a loved one
through our Camp Safe Haven and SASHA’s Friends.
• We provided assistance to seriously ill patients and their caregivers through our Haven
Transitions program.
• We educated health-care professionals about the benefits of end-of-life and palliative care.
• We improved access to hospice care for those in rural communities, veterans, minorities
and others who may not be aware of their benefits under hospice care.

• We staffed our teams with physicians, nurses, licensed massage therapists, pharmacist and

COMMITMENT
TO THE COMMUNITY
As a community-based, not-for-profit hospice serving North Florida since 1979, Haven Hospice
serves 18 counties over 13,000 square miles. Haven has always been committed to providing
our programs and services regardless of an individual’s ability to pay. Many of the communities
we serve are largely rural and the families we care for are struggling to make ends meet. One
hundred percent of the gifts and donations made to Haven remains in the communities we serve
and helps provide care for patients unable to pay, as well as the many unfunded programs and
services we offer. In 2011, Haven spent more than $3.1 million on unreimbursed patient care,
family services and programs, community and health professional education and other community
support.

Commitment To The Community
Patient and Family Care
Everyone deserves companionship through
life’s journeys. That is one of the core beliefs
at Haven Hospice. Our dedicated staff and
volunteers are here to make sure patients
and families have support, comfort and dignity
each day. We do that by serving them in the
setting where they are most comfortable and
providing relief from pain and symptoms. Our
patient and family care offers them choices
and honors their wishes. Donations helped
offset $1,562,579 in unreimbursed patient care
expenses in 2011.
Healing Hearts
All those we serve have access to our
grief support services at no cost. They are
offered above and beyond our patient
services to enhance the care we provide. In
addition to those we have served in hospice,
Haven makes these grief support services
available to anyone in our communities who
has experienced a loss. For many people
in our community, Healing Hearts is their
first experience with our comfort, care and
compassion. In order to honor the lives of
family, friends and loved ones, we host semiannual
memorial services known as Love and
Remembrance services in May and November.
Last year, 3,339 individuals benefitted from the
services provided by Haven’s Healing Hearts
program. Your gifts in 2011 helped support
$195,533 in unreimbursed expenses for
bereavement services and children’s programs.
That does not include the cost of our chaplains
and social workers who spend thousands of
hours providing supportive counseling for our
patients and their families.

Commitment To The Community
Haven Transitions
When education and support are needed, families
need look no further than Haven Transitions – a
program that can make a world of difference for
someone facing a serious illness. This support
includes phone calls and volunteer assistance for
those who can benefit from companion visits.
For someone wanting to learn about living with
a chronic illness, preparing advance directives
and much more, Transitions has the answers.
Transitions provided services to 779 individuals
and their families last year. More than $47,298 of
the unreimbursed expenses of those services was
covered by donations in 2011.
Children’s Programs:
Camp Safe Haven and SASHA’s Friends
No two people grieve in the same way. For
children, the process is even more challenging.
There is nothing more important than providing
that special support for children who have either
recently experienced a loss or are anticipating
one. Our grief support services include Camp
Safe Haven, a unique program for children who
have lost a loved one and SASHA’s Friends,
a support group for young people who are
anticipating the loss of someone close to them.
Haven’s Healing Hearts hosts Camp Safe Haven
annually for children, ages 6 - 12 and teens,
ages 13 - 17. With the support of Haven staff and
volunteers, these children participate in fun,
safe and therapeutic activities that allow them to
work through their grief. They also find comfort
in sharing their loss with their peers who may
also be grieving. Camp Safe Haven and SASHA’s
Friends served 77 children in 2011.

Palliative Massage Therapy
Comfort and compassionate care is part of the treatment for Haven Hospice patients,
and nothing is more comfortable than the touch by Haven’s specially trained licensed
massage therapists. Palliative massage therapy enhances our patient care by helping
patients relax, relieving their pain, stress and anxiety, increasing their physical
comfort and improving sleep. All together, we were able to provide more than 6,084
patient visits by our massage therapists. Your support of Haven in 2011 helped offset
$341,664 in unreimbursed expenses for this program.

Commitment To The Community
Volunteer Services
Volunteering for Haven Hospice does more than
enhance the lives of patients and their families. It
is inspirational for the volunteers themselves. Our
specially-trained volunteers work hand-in-hand with
patient care teams to help provide vitally needed
support to patients and families. Volunteers also
reach out to grieving families, help staff our Attic
Resale stores, provide administrative and offce
support, perform community outreach and serve
on special event committees. In 2011, donations
helped offset $534,060 in unreimbursed expenses
required to recruit, educate, train and manage
volunteers.
Community & Health
Professional Education
Haven Hospice’s commitment to all its
communities extends far beyond providing handson
clinical, emotional and spiritual care. Providing
education and information on a variety of topics,
including living wills and advance directives,
coping with grief, caring for the caregiver and
much more are all part of the services we provide.
Haven Hospice also provided clinical education
to future health professionals from area colleges,
universities and community colleges by providing
students with internships, clinical rotations,
course instruction and lectures. Donations from
you helped offset $439,999 in unreimbursed
expenses for this program.
There’s even more as Haven Hospice sees its
role as including clinical education programs to
physicians, nurses, social workers and nursing
assistants in area hospitals, long-term care
facilities and doctor’s offces in order to help them
better understand and appreciate all the unique
issues surrounding end-of-life and palliative care.
Additionally, we train medical professionals in how
to help care for hospice patients in their facilities.

New Care Center Under Construction
There’s much excitement in the air in Orange Park with the new Homer J. Sr. & Fern O. Custead
Care Center scheduled to open in the summer of 2012.
Ground was broken on the 18-bed inpatient hospice facility, located at 745 Blanding Blvd., in
September. The care center is set to serve residents of Orange Park and Clay County, as well as
in nearby Duval, Baker, Nassau and St. Johns counties.
It all started with a $1.3 million gift from the Custead family that provided the impetus needed to
make the care center a reality.
The Custeads see this as a wonderful way to support Haven and the community.
“My father moved to the area 65 years ago and worked hard to make a living and provide for our
family,” said Richard Custead, the donor’s son. “My mother thought this would be a wonderful way
to honor him in the community he loved.”
While the Custeads got things started, there are still a number of opportunities to donate to the
construction in a rather unique fashion.
A website, www.careforclay.org, has been established where donors can select specific building
materials to buy or find a variety of naming opportunities. That’s all part of the effort to truly make
this care center part of the community.
“We are committed to Orange Park and Clay County,” said Haven President Tim Bowen. “The gift
from the Custead family will allow Haven to better provide care for patients and families in the
Orange Park and Clay County area.”
This will be the fifth Haven Hospice care
center, with others in Chiefland, Gainesville,
Lake City and Palatka. Each provides
specialized care in the communities we serve
and is conveniently located so patients and
families can access them without having to
travel a distance or forego inpatient hospice
care altogether. Each care center is ready
to deal with short-term inpatient needs
and, as a result, offers families an option
other than admission to a hospital. It also
provides respite care support for families and
caregivers who need a break.
Plans call for employing 23 fulltime staff
Ground breaking of the Homer J. Sr. & Fern O. Custead Care Center
employees who live in Orange Park and its
neighboring communities. They will include registered nurses, licensed practical nurses, nurses’
aides, an administrator, secretary, medical director, social worker, cooks and housekeepers.

THE COMMUNITIES’ SUPPORT
Donors make it possible for us to provide all our services. In 2011, donors gave gifts to Haven
Hospice in honor or memory of a loved one, attended or sponsored a special event, responded
to an annual appeal such as Light Up a Life or Forget-Me-Not, donated items to or shopped at
our Haven Attic resale stores or remembered Haven in their wills. Their generous contributions
provided $1,866,946 in total support. These donations are vital to Haven and allow us to
provide many programs and services to our patients and their families that otherwise would not
be available.

Special Events, Annual Appeals, Planned Giving, the Haven Hospice Attic Resale Stores and

The Community’s Support
Special Events
As they always have, Haven supporters
embraced the opportunity to uphold our
mission through fundraising and volunteering
in 2011. Many supported our events through
their attendance and participation. As we have
strengthened the partnerships in each of the
communities we serve, organizations in those
communities have stepped up to host special
events. These events, vital to our fundraising
efforts throughout North Florida, raised
$332,100 in cash to benefit Haven in 2011.
Annual Appeals
Thanks to the generosity of our donors, Haven
Hospice received $107,606 in 2011 through our
spring Forget-Me-Not appeal, our fall Light Up
a Life appeal, our In Touch newsletters and in
response to our Annual Community Report.
Honor/Memorial Gifts
Haven received $305,797 in gifts and pledges
in honor or memory of a loved one in 2011.
These donations are a primary source of support
for unfunded and unreimbursed programs and
services offered by Haven. Many families and
friends request that donations be made to
Haven Hospice in lieu of flowers, or they decide
to memorialize their loved ones along the
Memorial Walkways located at our care centers.
The Memorial Walkways are dedicated to your
loved ones. A permanent legacy is created
through the walkway donors. Bricks, granite
pieces and benches along the Memorial
Walkways are inscribed to honor loved ones
according to your wishes.

The Community’s Support
Planned Giving
Planned giving is a significant way to provide
major support for Haven Hospice. We received
$221,449 from donors who named us in their
wills, estates or other planned giving vehicles in
2011. These donations are vital to the patients
and families who benefit from the many unfunded
programs and services Haven Hospice offers.
Planned giving enables donors to arrange
charitable contributions to Haven Hospice in
ways that maximize their personal objectives
while minimizing the after-tax cost. Depending
on the asset given to Haven Hospice and the
gift arrangement selected, a donor can generally
expect to obtain some or all of the following
benefits:
• Fulfill philanthropic goals
• Reduce income tax through a charitable
deduction
• Avoid capital-gains tax on gifts of long-term
appreciated property
• Retain a stream of income for life for yourself
and/or other beneficiaries
• Increase spendable income
• Eliminate potential federal estate tax on
property that passes to Haven Hospice
• Reduce costs and time in estate settlement
Endowment Fund
The Haven Hospice Endowment is a separate
account of funds designed to provide future longterm
support to Haven Hospice. These funds are
used to provide for the unfunded patient care and
our many community and family support programs
and services. The Haven Hospice Board of
Directors maintains oversight of funds associated
with the endowment. As of Dec. 31, 2011, the
balance of the endowment was $1,534,886.
Sustainers: Making It Simple
to Provide Your Annual Gift to
Haven Hospice
You can now make your annual gift to Haven
Hospice even easier by becoming a Haven
Sustainer. The new Haven Sustainers program
allows donors to make convenient, automatic,
and ongoing monthly contributions that can be
changed or stopped at any time.
Haven Sustainers have chosen to contribute at
least $5 per month through automatic deductions
from their credit cards. This provides a consistent
stream of income to help Haven provide critical
programs and services to patients and families
regardless of their ability to pay. Membership in
the Haven Sustainers program is automatically
renewed each year until you choose to make a
change in your giving level or your participation.
It is very easy to become a Haven Sustainer. You
can call 1-800-727-1889 and ask to speak with
someone in the Development Department. Or you
can go online to www.havenhospice.org, fill out
our online donation form and click on the monthly
donation option.
However you choose to make your donation to
Haven Hospice, we want you to know how much it
is appreciated. Without you we cannot do most of
what we do for families impacted by the passing of
loved ones.

Haven Hospice Attic Stores
You can support Haven’s four Attic stores
in Gainesville, Lake City, Orange Park and
Chiefland in three different ways.
Donate
The generous donations brought to the stores
create the inventory we need. We appreciate
our generous donors who bring their new
or gently used treasures to us. Without them
we would have a great challenge helping the
families and communities we serve. Sales
from these donations benefit the patients and
families who cannot pay for care, along with
those served by programs such as Camp Safe
Haven for children who have experienced a
loss. Countless individuals, businesses and
organizations generously donate items ranging
from housewares and furniture to clothing and
children’s toys.
At the Attics, we also partner with community
organizations to provide much needed clothing
and housewares to the homeless, displaced
homemakers, victims of fire and many others
in need. We are also proud of our partnership
with Kids Closet that provides new or like new
coats to children in need. We host Kids’ Night
in December to help children shop for family
members who otherwise could not afford
to exchange gifts. Thanks to your generous
donations, the Attic stores combined net
revenue in 2011
was $559,183.
Volunteer
It is the Haven Attic volunteers who make the
stores a success. They spend time sorting
donations, stocking the floor and making friends.
But beyond that, they help people throughout
the Haven Hospice community — patients and
families, as well as people who have challenging
economic circumstances and need an affordable
place to shop. We are always seeking energetic,
conscientious volunteers who are mindful of the
Haven mission. We could not do what we do
without our volunteers.
Another way to support the Attic is hosting a
donation drive or bringing the Rollin’ Attic truck
to your place of business or neighborhood. To

Haven Hospice Attic Stores for
Orange Park and Chiefland
The newest Attic Resale Stores opened in
March 2012 in Chiefland and March 2011 in
Orange Park. The Chiefland store is located at
112 Rodgers Blvd. next to the Save-A-Lot and
the Orange Park store is located at 1801 Wells
Rd. across from the Orange Park Mall. The
showrooms are filled with wonderful treasures
and have hard-working volunteers waiting
to serve you. The residents of Chiefland and
Orange Park have enthusiastically responded to
the new stores.
Shop
We are thankful for our customers who love
shopping for all those unexpected treasures
at our Attic stores. We restock the gently used
items daily.

DeLAND
Volunteer: Brenda Gormley
Brenda Gormley goes above and beyond, and
because of that she was the recipient of the
Johana Knox Community Service Award.
The award was created in 2008 by the Florida
Hospital Fish Memorial Foundation. It is
intended to recognize, celebrate and encourage
outstanding volunteerism in Volusia County.
Brenda creates, sews and individualizes every
Memory Bear for the DeLand Haven Hospice
offce. Haven Hospice volunteers offer each
family of a deceased patient the special gift of
two Memory Bears, free of charge. In return,
interested family members respond with favorite
articles of clothing from their loved one. This
could be a shirt, blouse or any article of clothing
which was special to the patient. After receiving
the articles Brenda contacts the family to
determine some special interests of the loved
one so that she can embellish the Memory Bear
with small mementos symbolizing the cherished
loved one’s journey through life. It usually takes
Brenda six to eight hours to complete one
Memory Bear.
Community Campaigns
In 2011, Haven Hospice received $95,803
from generous individuals throughout our
communities that designated Haven as their
charity of choice in the associated employee
campaign. Donors to the University of Florida
Campaign for Charities (UFCC), the Combined
Federal Campaign (CFC) and the Florida State
Employee Community Campaign (FSECC) have
designated their gifts to Haven. Additionally,
even though Haven is not a United Way agency,
anyone participating in a United Way employee
campaign can name Haven as a designated
agency of choice.
The Memory Bear project has not only impacted
Brenda’s life and the lives of Haven Hospice
family members, but it has provided a means
for the families to have the memories and the
connection to their loved ones through the
huggable, loveable Memory Bears.
CHIEFLAND
Volunteer: Harry Coleman
Haven Hospice helped his family in 1994, and
since 2003 Harry Coleman has been doing
what he calls payback – volunteering for Haven
Hospice in Chiefland.
If you are in the Chiefland area, most likely you
will cross paths with Mr. Harry as he is fondly

known at Haven. His caring is obvious when
you consider he spent more than 750 hours in
2010 and more than 1000 hours in 2011 helping
with and promoting the many programs of
Haven Hospice. He finds time almost every
day to receive and organize donations at the
Haven Hospice Attic donation trailer located
at Scoggins Chevrolet. As a result of his hard
work and dedication, and the support of the
community, Haven opened its newest Haven
Hospice Attic Resale Store in Chiefland in
March 2012.
At other times, Mr. Harry will be energetically
working with the current fundraising event.
When it comes to the popular yearly TV
giveaway by Badcock Home Furniture & More,
it is very likely Harry will sell the most tickets. He
also worked hard in 2011 on a new fundraising
event hosted by the Chiefland Rotary Club for
Haven, which was highly successful due in large
part to his efforts. The yearly Fall at the Ranch
event is one of his favorites, as well as Haven’s
annual ViVA! fundraiser.

staff’s candy jar stocked at all times. His goal is
to make everyone’s job easier and help where
needed. He’s also been known to anonymously
purchase memory bricks for the memorial
walkway in the name of former patients.
All of this because of in his words, “the way
Haven Hospice took care of my family.”
Fall at the Ranch
Fall at the Ranch at Rock Bluff Ranch in Bell was,
as always, a big success for Haven Hospice
in 2011. The event raised more than $55,000
to support unfunded programs, including
Camp Safe Haven. More than 400 guests
and volunteers attended the casual, fun-filled
evening on Oct. 15.
Guests enjoyed food from the Ivy House, drinks,
live music by Dottie South and the Slackers,
a live and silent auction as well as interactive
cowboy experiences. Silent auction items
included a signed guitar by Trenton local Easton
Corbin, a trip for two to New York including a
stay at the Hyatt, a delicious catered dinner at
your home by the Ivy House including a live
singer and a hunting rifle from Pawn Pro.
Mr. Harry has been helping and supporting the
Haven Hospice Tri-Counties Care Center in
Chiefland since before it was built. Not only did
he sponsor two rooms at the Haven care center,
he is still very involved with the operation of the
facility, including: donating roses for patients
when they pass away; providing personal

GAINESVILLE
Gary Perry: Living Life to the Fullest
Gary Perry is going to die, but until he does, he
is living life to the fullest – thanks to
Haven Hospice.
Gary suffers from Gardner Syndrome, also
known as familial colorectal polyposis. It leads to
rectal cancer and tumors or polyps in the body.
In his abdomen alone, Gary has 40 small tumors.
Additionally, the gene that causes the disease
is passed on to children. Before he was aware
he had Gardner syndrome, Gary and his wife
had three children. They lost their 20-year-old
daughter, and today, their 19-year-old son and
5-year-old grandson both have the disease.
There is currently no cure. In its advanced
forms, which Gary has, it is generally considered
terminal with a life expectancy of 35 to 45 years.
Gary celebrated his 47th birthday in January.
“When I was 40 and found out about this, I did
not think I was going to see 47,” he said.
But not only has Gary seen 47, he was able to
celebrate his birthday in January – something
he would not have thought possible only six
months ago.
Last year, Gary made the decision to put aside
the aggressive disease treatments and start
to manage the symptoms with the help of
Haven Hospice. The change was not easy at
first, but with the help of Haven’s Dr. Geraldine
Bichier and the staff of the Haven Hospice E.T.
York Care Center, Gary’s pain and symptoms
subsided. Before, I was having seizures every
five minutes,” he said. “But after working with the
Haven team, the seizures stopped, I could walk,
and I got normal sleep.”
Gary felt well enough on Dec. 23 to do what
millions of other people were doing that day –

going Christmas shopping. He called his mother
to come and get him. “As soon as the paperwork
was done, we left and went to the Oaks Mall
to go shopping. Later that evening, I took my
parents to Red Lobster for dinner.”
But that was far from enough for Gary. The next
morning, Christmas Eve, he called his mother at
7 a.m. to go shopping again.
“After all,” he said, “it was Christmas Eve, and it
was the first time I could go shopping in years.”
Since then he has been indulging in some of
his favorite activities – driving, fishing and riding
his bike.
moment I can. I know this has not changed my
disease process, but for now I have a better
quality of life. This is like a gift, and I will take it
for as long as it will last.”
Gary remembers what it was like when the
doctors first talked about hospice care.
“When my mother first heard about hospice, she
started crying,” he said. “But now we understand
things differently. It’s working, and I am enjoying
life to the fullest. There’s no stop sign on life
when you get to Haven Hospice. You don’t have
to check out. Keep living. Keep enjoying the gift
of life.”
“I have to make myself slow down, or I will wear
out my legs,” he said. “I know I have to limit
myself, but I am awake, and I can function.”
That’s important to Gary.
“I have kids, and I want to be around for them as
long as I can,” he said. “I believe that God has
sent me back to them, and I want to live every

Run for Haven
More than 300 runners and walkers were
treated to perfect weather at the second-annual
Run for Haven, which took place in March at the
Tioga Town Center and the Town of Tioga. The
event featured a twilight 5K and 10K run/walk
followed by a great party featuring food, drinks,
live music and other family-friendly activities.
In addition to the runners and walkers, more
than 40 volunteers and 23 sponsors and
vendors participated. More than $17,000 was
raised from the event and went to support
Haven’s care and services to patients
and families.

ViVA! 2011—Polynesian Bash
A good time was had by all at the seventhannual
ViVA! Polynesian Bash, a benefit
for Haven Hospice held in April at the Rembert
Farm in Alachua.
More than 600 guests enjoyed an evening full
of true Polynesian cuisine catered by Embers
Wood Grill, along with lively music and dancing
from Island Vision Productions, a gaming tent,
a cigar roller, live and silent auctions and
much more.
ViVA! 2011 was a tremendous success. With a
wonderful location and perfect weather, more
attendees were able to enjoy the night than at
any of the previous six ViVA! events.
ViVA! 2011 raised an estimated $345,000
to support patient care, services and the
many unfunded programs provided by
Haven Hospice.
Tenth Annual Styles For Life
Fashion Show
With dignitaries in attendance, models displayed
some high fashion in the 11th annual Haven
Hospice Alliance Styles for Life Fashion Show.
The November event, at the Paramount Plaza
Hotel and Conference Center raised more than
$15,000 for unfunded programs offered by
Haven Hospice to patients and families.
Among those in attendance were Florida Sen.
Steve Oelrich and his wife, Rose Mary, School
Board member April Griffn, and Melanie Shore.
The WKTK’s Storm Roberts served as emcee.
The show featured local fashions from Belk,
Coldwater Creek, Rapid City Clothiers (RCC
Western Stores), Stein Mart and the Haven
Hospice Attic. There were also door prizes and
a silent auction.
Lynn Newman chaired the show. Sponsors
included Granny Nannies, Gainesville Healthcare
Centers, Comfort Keepers, Forest Meadows
Funeral Home, D’Lites, Barnes Health Care
Centers and Morgan Stanley Smith Barney.
The event was hosted by Haven Hospice
Alliance, a group of volunteers who promote
and support Haven through fundraising, special
events, volunteerism and community education.

The Community’s Support
JACKSONVILLE
Lisa Norvelle
When Lisa Norvelle married Rob Lazano on Dec.
10, she walked down the aisle wearing a wedding
dress she bought for $39.99. The bargain also
included two bridesmaid dresses.
The bride was not being cheap. She was
supporting one of her favorite charities – Haven
Hospice.
But she did more than find a bargain wedding
dress. She encouraged her guests to forego
traditional wedding gifts. Instead, she and Rob
asked their guests to donate to the construction
of the Homer J. Sr. & Fern O. Custead Care
Center by going to the Care for Clay website,
www.careforclay.org. There, they could contribute
to the fund set up to buy the building materials
necessary to build the care center.
Some might think the gesture a little unusual,
but not for Lisa, who has been supporting the
Orange Park Hospice Attic since the day it
opened.
In the beginning she would bring meals for
the volunteers, and, while she was there, do a
little shopping. Spending time at The Orange
Park Attic helped occupy her time while Rob,
a doctor, was called to active duty in the coast
guard last fall.
Like many people who volunteer or donate to
Haven Hospice, Lisa and Rob had a connection.
“Haven Hospice is a wonderful organization,”
she said. “They helped with Rob’s mother a few
years ago, my grandmother in 2002 and my
godson’s father three years ago.”
So, while she was there one day, she spotted
her wedding dress, and the idea came to her.
“Haven Hospice has done so much for so many
people,” she said. “It was the perfect way to say
‘thank you.’ And I love shopping at the Attic. This
is the best place.”
And right after the wedding, the dress went
right back to the attic. That way, someone else
can buy it and make another donation to Haven
Hospice.v

Joes v Pros Family Extravaganza
Sunday, September 18 was a beautiful day for
a softball game as Haven Hospice and the
Baseball Grounds of Jacksonville hosted the
Second Annual Joes v Pros Family Extravaganza
in Jacksonville.
The event featured the University of North
Florida’s baseball team as the “Pros” for the
event. The “Joes” consisted of children from
the Jacksonville area. The UNF baseball team
hosted a mini-training camp with training
stations. Festivities also included food and
drinks, a kid’s zone with face-painting, craft
activities and inflatable attractions, a viewing
area to watch the Jaguars game, music and a
grand finale softball game.
Care for Clay 5K
On Sunday, October 23rd, around 150 runners
participated in Run for Haven: Care for Clay 5K,
a twilight run at the Orange Park Mall. The run
raised more than $5,000 to support Care for Clay,
an effort to help Haven Hospice offset the cost of
building its Custead Care Center in Orange Park.
Participants were invited to donate to Care for
Clay by buying a $1 “Memory Balloon” which they
could write the name of a lost loved one on. The
Sunjammer Band provided entertainment and a
post-race party was held at the end of the race that
included food, drinks and an awards ceremony.

LAKE CITY
Veteran’s Day
The second-annual Veteran’s Day program
in Lake City, held in the Suwannee Valley
Community Room, drew a packed house and left
not a dry eye among those in attendance.
Speaker Deborah Grassman, ARNP, shared
her inspiring message about the special
grief of Veterans. Grassman works for the
U.S. Department of Veteran Affairs as a
hospice nurse.
The audience included hardened veterans
from World War II, Korea and Vietnam. Despite
the tough crowd, one person in attendance
suggested that there was healing happening
in the audience. Her message was very
therapeutic, and people were overwhelmed with
her stories about veterans.
The author of Peace At Last: Stories of Hope and
Healing for Veterans and their Families, she has
been sharing the lessons she has learned over
30 years as a hospice nurse. Her book normally
sells for $20, but she gave one to every veteran
in attendance.
The result was a wonderful experience for all
involved, and it would not have been possible
without the sponsorship of Power Country 102,
and Ms. Wezzie’s Haircuts and a host
of volunteers.
It will be hard to top this event in 2012, but it is
important to remember our veterans each year.

Volunteer: Bettye Zowarka
Hospice Volunteer and Keystone Heights
resident Bettye Zowarka received one of the
highest honors possible for a hospice volunteer
in 2011 – the Volunteers are the Foundation of
Hospice Award for Organizational Support.
Bettye received the national award from
the National Hospice and Palliative Care
Organization (NHPCO) at its annual conference
in San Diego in October.
Bettye has been a Haven volunteer since 1984.
It is estimated that she has given more than
27,000 hours of her time to help members of her
community with end-of-life care. In fact, without
Bettye, Haven Hospice might not exist in the
Keystone Heights and Melrose area. She was
the motivation and inspiration behind hospice
care expansion into these communities.
Bettye’s desire to bring Haven Hospice to the
area came from personal experience. Her father
died of cancer in Tennessee in 1984. She was so
impressed with the level of care and compassion
he received; she became determined to bring
hospice services to her own community in
Clay County.
Bettye worked tirelessly to convince Haven
Hospice that her small rural community needed
their services. Established in Keystone Heights
and Melrose, the service area was later
expanded to include Clay County.
Haven began to provide care to patients and
families there and Zowarka formed a group of
30 volunteers to help in the new area. In order
to make the care a reality, community volunteers,
led by Bettye, held fundraisers and gained more
than $10,000 in support.
Over time, Bettye has served in many volunteer
roles with Haven Hospice, including working
with patients and their families, bereavement
and fundraising, as well as public speaking and
volunteer training. Today she is a Volunteer
Liaison who supports Haven in four counties –
Alachua, Bradford, Clay and Putnam.

Event: NEFAR Charity
Bass Tournament
The Northeast Florida Association of Realtors
(NEFAR) has been a long-time supporter of
Haven Hospice. The annual NEFAR Charity
Bass Tournament in Palatka has raised
approximately $145,000 over the last eight
years (including $20,500 in 2011) to support
Haven patients and their families in the
Putnam County area.
ST. AUGUSTINE
Art Showcase
For the last year, there have been three gatherings
at Haven Hospice in St. Augustine that are a little
different. Those would be the openings of the Art
Showcases designed to feature and promote the
work of local artists.
In 2011, NEFAR also generously donated
$25,000 for the purchase of a box truck for
the new Haven Hospice Attic Resale Store
in Orange Park. The truck is used to pick up
donations throughout northeast Florida and
is a tremendous asset for the organization.
The Showcases have been a highly successful
vehicle to connect Haven Hospice to the St.
Augustine community.
Hundreds of people have come to the openings.
Attendees have included patients and their families,
friends of the artists, members of the healthcare
professional community and others interested in art.
Originally created simply as a networking opportunity,
they have become much more. The openings have
also served as a way for people to learn more
about Haven Hospice’s mission and eliminate
misconceptions about what hospice care is all about.
Haven uses the openings, which are free and open
to the public, as a way to revamp artwork in its offce.
Staff encourages the community to come out and
see emerging talent in the community with each
showcase.
With the success of 2011, the Art Showcases will
continue in 2012. Plans include having a theme for
each show to help market them more effectively.
