APPLICATION FORM<br />
SPONSORED AND ADMINISTERED BY THE <strong>CWB</strong> GROUP<br />
The R. M. Gooderham Bursary was established by the Board of Directors of the <strong>CWB</strong> <strong>Group</strong><br />
to honour Mr. R. M. Gooderham, the first General Manager of the Canadian Welding Bureau<br />
(a division of the <strong>CWB</strong> <strong>Group</strong>).<br />
A maximum of ten (10) bursaries valued at $2,000.00 each are awarded annually to<br />
qualifying students. The Bursary is granted to successful candidates with the intent to further<br />
the welding and joining industry in Canada.<br />
INSTRUCTIONS:<br />
1. Complete and sign the Application Fo<strong>rm</strong> and e-mail to the Program Coordinator at<br />
your College along with your digital photograph and Letter of Introduction.<br />
2. Instruct the Program Coordinator to include a copy of your most recent academic<br />
transcript and a recommendation from a staff member with your submission.<br />
3. Submissions are to be sent by your Program Coordinator to the <strong>CWB</strong> <strong>Group</strong> before<br />
November 26 th , 2010 by e-mail to: hilary.barton@cwbgroup.org<br />
4. Submissions to include the completed Application Fo<strong>rm</strong>, Personal Letter of<br />
Introduction, Digital photograph, copy of academic transcript, and a recommendation<br />
from a staff member.<br />
5. Applications will be evaluated using the following weighting:<br />
Letter of Introduction Content: 60%<br />
Academic Standing: 40%

SECTION 4 (Letter of Introduction)<br />
Submit a Personal Letter of Introduction, which outlines your goals and ambitions related to your career in the<br />
welding industry.<br />
- Submit Letter of Introduction with the Application Fo<strong>rm</strong> -<br />
SECTION 5 (Staff Submission)<br />
Include a brief submission from a staff member of your choice from the Institution relating to your activities in<br />
welding and related courses. The staff member should provide the reason they think the individual is worthy of<br />
consideration for the Bursary.<br />
- Submit recommendation with the Application Fo<strong>rm</strong> -<br />
SECTION 6 (Photograph)<br />
Submit a digital Photograph of yourself (size 4” x 6” – full front view of the face and shoulders) that can be used<br />
in publications announcing winners of the Bursaries.<br />
- Submit a digital Photograph of yourself with the Application Fo<strong>rm</strong> -<br />
SECTION 7 (Declaration)<br />
I certify that this <strong>application</strong> fo<strong>rm</strong> has been completed to the best of my knowledge and belief, and that the Letter<br />
of Introduction is my own and unassisted. I also consent to the use of my name, institution, photograph and<br />
Letter of Introduction in promotional material if I am awarded one of the Bursaries. (All other personal<br />
info<strong>rm</strong>ation collected will be used only for the <strong>bursary</strong> <strong>application</strong>.)<br />

Your <strong>application</strong> package must be e-mailed and received by the <strong>CWB</strong> <strong>Group</strong> by November 26 th ,<br />
2010. Applications received after this date will not be accepted.<br />
