Purchase any product* in our catalogue for the<br />

first time and if you are not completely satisfied,<br />

call us within 30 days of your purchase date<br />

and we will exchange the product for one that’s<br />

ideally suited to your clinical technique. If you’re<br />

still not completely satisfied, we’ll provide you<br />

with a refund. Please note, after 30 days, these<br />

products may not be exchanged or returned.<br />

NEW CUSTOMER:<br />

Welcome to Bisco Dental Products Canada Inc.<br />

To set up a new customer account, we require the<br />

following information:<br />

• BILL TO – Name, Address, Phone Number<br />

• SHIP TO - Name, Address, Phone Number<br />

• Email Address & Fax Number<br />

• Method of Payment (Visa or Mastercard)<br />

Pre-Payment is required for your first order to establish a new<br />

customer account. For all subsequent orders, you can choose<br />

your preferred method of payment from the payment terms*<br />

PAYMENT TERMS*<br />

CREDIT CARD PAYMENT: We accept VISA or<br />

MASTERCARD. At your request, we will keep your<br />

credit card number and expiry date on file. Please<br />

let us know if you would like all of your orders<br />

automatically processed on your credit card.<br />

NET 30 DAYS: Existing customer accounts are<br />

payable within 30 days of the invoice date. As monthly<br />

statements are not automatically issued, please<br />

calculate the amount due from the invoice enclosed<br />

with your parcel. Overdue NET 30 accounts are<br />

subject to a finance charge.<br />

ACCOUNT STATEMENTS: Account statements are<br />

available by request. Statements are only issued<br />

automatically for overdue account balances.<br />

NON-SUFFICIENT FUNDS: If your payment by<br />

cheque is not honoured or is returned, we will charge<br />

your account with a $20.00 Non-Sufficient Funds fee.<br />

PRODUCT WARRANTY:<br />

At Bisco Dental Products Canada, we recognize<br />

our responsibility in ensuring your satisfaction with<br />

the products we supply. We will replace, at no extra<br />

charge to you, any products that are proven to be<br />

defective. Bisco Dental Products Canada does not,<br />

however, accept liability for any damage or loss,<br />

either direct or consequential, stemming from the<br />

use or misuse of any product we distribute. It is the<br />

responsibility of the user to determine the suitability<br />

of the product for an intended procedure prior to use.<br />

The use assumes all ri<strong>sk</strong> and liability in connection<br />

therewith.<br />

PRODUCT DELIVERY<br />

All orders placed by 1:30 pm PST will be shipped the<br />

same day. Subsequent orders will be shipped the<br />

following day. Typically, most orders are delivered next<br />

day to every major metropolitan city within Canada.<br />

Please be aware that some destinations cannot be<br />

reached by the next business day and may take<br />

between 48-72 hours. Product delivery charges vary<br />

by region and for urgent product deliveries surcharges<br />

will apply.<br />

PRODUCT RETURNS<br />

We understand the time and frustration associated with<br />

product returns and we want to help make the process<br />

as efficient as possible. Our 30-DAY Guarantee<br />

applies to each time a product in our catalogue is<br />

purchased for the first time. Our standard return policy<br />

applies to all products which have been purchased<br />

more than once on your account.<br />

If you must return a product, please follow the<br />

guidelines below. All product returns must be<br />

preauthorized. Products sent to us without<br />

authorization will be refused.<br />

STANDARD RETURN POLICY<br />

Product returns will be authorized if the following<br />

guidelines are met:<br />

1. The product is returned within 30 days of the<br />

invoice date.<br />

2. The product is unopened, unused, and in the<br />

original packaging.<br />

3. The product is damaged or defective (determined<br />

by Bisco representative).<br />

Product returns will not be authorized if:<br />

1. It has been 30+ days since the product was<br />

purchased (date on invoice).<br />

2. The product is opened or used (excluding<br />

damaged or defective products).<br />

3. It is a special order item (excluding damaged or<br />

defective products).<br />

Please call a Bisco customer service rep at 1-800-667-<br />

8811 for authorization and return instructions. Please<br />

have the following information available when you call:<br />

1. Your customer number and/or office phone number<br />

2. Original invoice number of product(s) being<br />

returned.<br />

3. Description of product(s) and reason for return.<br />

Include lot # and expiry date, if applicable.<br />

Because this information may have to be verified in<br />

your file, we may need to call you back with a return<br />

authorization or refusal. Returned products may be<br />

subject to a minimum restocking charge of 15% or a<br />

partial credit.<br />

Please ensure that all returning products (once<br />

authorized) are placed in protective packaging, are<br />

properly addressed to Bisco Canada’s Head Office,<br />

and include your account information.<br />
