locate this and lots of other information about appraisal on the Education Centre pages of the RUH
website.
SECTION 2: Evidence for Appraisal
When putting evidence in your folder it is a good idea to write in the top right
hand corner which dimensions and level this refers. On the following pages are
templates you could use to record various forms of evidence.
If you need help to do this speak to your manager, appraiser, colleagues,
mentor, etc. or call the Workforce Development Department on ext. 5502 who
will be able to give advice.
CONFIDENTIALITY:
Any evidence containing patient information, staff and financial details and anything else regarded as
confidential by the Trust should only be included in your folder if it has been made anonymous, i.e.
deleted names, numbers, figures - anything that could be traced back to the Trust, patient or staff
member.
The KSF Folder will be securely kept by the staff member.
The staff member and Manager/Appraiser will view and discuss the content of the KSF Folder at appraisal
and development reviews only.
