SF-36), a comprehensive self-administered short form with only 36 questions<br />

designed to measure health status <strong>and</strong> outcomes from the patient's<br />

The SF-36 yields a profile of eight health scores:<br />

1. Limitations in physical activities because of health problems.<br />

2. Limitations in usual role activities because of physical health problems.<br />

3. Bodily pain.<br />

4. General health perceptions.<br />

5. Vitality energy <strong>and</strong> fatigue).<br />

6. Limitations in social activities because of physical or emotional problems.<br />

8. Mental health psychological distress <strong>and</strong> well-being).<br />

The SF-12 [100], an even shorter survey form published in 1995, has been<br />

shown to yield summary physical <strong>and</strong> mental health outcome scores that<br />

are interchangeable with those from the SF-36 in both general <strong>and</strong> specific<br />

populations.<br />

The instruments have been translated into more than 40 languages. The<br />

SF-36 can be used in all kinds of surveys <strong>and</strong> has been proved useful in<br />

monitoring general <strong>and</strong> specific populations, as documented in more than<br />
