INSTRUCTIONS
TO THE APPLICANT

I. Cash reward is available to both Gallantry and Non-Gallantry Award winners.
2. This application form duly filled in is to be submitted to Rajya Sainik Board,
Orissa under a covering letter addressed to the Principal Secretary to Government
Home Department, Orissa (Through Rajya Sainik Boar:d)
3. The Government of India Gazette Notificatio.n, Citation,(Certificate and the
Residential Certificate shall be produced at appropriate time in original before the
Secretary Rajya Sainik Board, Orissa f~r attestation. of the Xerox copies enclosed
with the Application Form. .Release Certificate and Pension Payment Order (in
case of retired personnel) also shall be produced in original before .the Secretary ,
Rajya Sainik Board for attestation of Xerox copy.
4.. In case of se-rVing personnel, the service certificate issued by the Unit shall be
enclosed in original with the application form.
5. Portions not applicable'shall be scored out, wherever the choice exists. Tick mark
the portion to indicate your choice.
6. Incomplete applications shall be rejected without any intimation to applicant.
7. The Rajya Sainik Board shall not be responsible for late receipt of application due
to postal or other delay and ~uch application may be rejected.
8. In case of deceased personnel, the cash rewards shall be available to next-of -kin
(NOK). In such c~s,
the applicant may submit this Form and Affidavit under
his/her signature disclosing his/her relationship with the deceased at serial
number-I of the application along with copy of Dea~h Certificate and Legal Heir
Certificate.
