Everyone wants a beautiful smile and every one should<br />

have a <strong>healthy</strong> one too.<br />

The orthodontist’s goal is to achieve both <strong>for</strong> his<br />

patients.<br />

What causes Orthodontic problem<br />

An orthodontic problem is called a malocclusion,<br />

meaning “bad bite”. Crowded teeth, extra teeth,<br />

missing teeth or jaws that are out of alignment are all<br />

various <strong>for</strong>ms of malocclusion. Most malocclusions are<br />

inherited, although some can be acquired. Acquired<br />

malocclusion can be caused by accidents, early or late<br />

loss of baby/milk teeth, or sucking of the thumb or<br />

fingers <strong>for</strong> a prolonged period of time.<br />

Why is Orthodontic important<br />

People who have straight, well-aligned teeth have<br />

been found to have higher levels of confidence and<br />

self-esteem than those who have crooked teeth.<br />

Crooked teeth are difficult to reach with <strong>you</strong>r tooth<br />

brush and cannot be kept clean, leading to cavities &<br />

gum diseases, thus decreasing the life span of <strong>you</strong>r<br />

teeth.<br />

Remember that straight teeth are not only beautiful<br />

but also <strong>healthy</strong>.<br />

At what age can Orthodontic Treatment be<br />

undertaken<br />

Both children and adults can benefit from<br />

orthodontics. However, the treatment in adults may<br />

take a little longer as the bones would have become<br />

stronger. It is recommended that every child receive<br />

an orthodontic evaluation between 8-12 years.<br />

How long does a standard treatment using fixed<br />

braces take<br />

Though the treatment varies from a case-to-case basis,<br />

the average treatment duration is 24 months. Things to<br />

be kept in mind are the severity of the problem, health<br />

of the teeth, gums, supporting bone and how closely<br />

the patient follows the instructions.<br />

teeth, without bands, brackets or wires. The clear<br />

aligners are removable, allowing <strong>you</strong> to eat <strong>you</strong>r<br />

favourite foods while brushing and flossing<br />

normally to keep <strong>you</strong>r teeth and gums <strong>healthy</strong>.<br />

Why is it important to treat Gum Disease<br />

Recent studies show that those suffering from gum<br />

disease are at high risk <strong>for</strong> heart attacks, diabetes,<br />

stroke, rheumatoid arthritis and respiratory infection.<br />

Pregnant women with gum disease are also more<br />

prone to having pre-term low birth weight babies.<br />

Treating gum disease helps towards improving <strong>you</strong>r<br />

general health.<br />

What is Gum (periodontal) Disease<br />

It is the second commonest disease in the mouth after<br />

tooth decay. It affects the tissues supporting the teeth<br />

namely the bone, ligaments and gingiva (gum). It is<br />

caused by plaque (bacteria) present in the mouth in<br />

susceptible individuals.<br />

What are the different types of Periodontal Disease<br />

There are two main types: Gingivitis and<br />

Periodontitis. Gingivitis is the inflammation of the<br />

gums. It is the reversible condition which resolves<br />

with good oral hygiene. Periodontitis is the condition<br />

characterised by destruction of the bone and ligaments<br />

supporting the teeth.<br />

What are the signs of Gingivitis<br />

Gingivitis is characterised by bleeding from the gums<br />

especially on brushing. The gums appear red, smooth<br />

and swollen.<br />

How is Gingivitis treated<br />

It can be treated by<br />

brushing effectively along<br />

the gum margin and<br />

by flossing in between<br />

the teeth regularly. Regular<br />

professional cleaning of teeth will also help to remove<br />

all hard deposits (tartar).<br />

What are the signs of Periodontitis<br />

Periodontitis is characterised by recurrent swelling<br />

(abscess) involving the gums in different parts of<br />

the mouth. There may be associated pus discharge<br />

and a dull ache. The teeth may be loose and begin to<br />

move resulting in spaces appearing between<br />

the teeth. The gums may also bleed on brushing<br />

and there may be bad breath. In advanced cases, the<br />

teeth may fall out due to loss of bone support around<br />

the teeth.<br />

How is Periodontitis treated<br />

There are 3 main stages in<br />

treatment: non-surgical therapy,<br />

surgical therapy and the<br />

maintenance phase. In<br />

non-surgical therapy, the<br />

patient is taught how to maintain good plaque<br />

control (oral hygiene). Professional cleaning of teeth<br />

(scaling) is carried out above and below the gums.<br />

If the patient has moderate to advanced disease,<br />

he may require periodontal surgery. In selected<br />

cases, surgery may be carried out to regenerate<br />

bone around teeth. Once surgical treatment is<br />

complete, the patient is placed on a regular recall<br />

(maintenance) to monitor their progress. Patients<br />

not requiring surgery may proceed to the<br />

maintenance phase after non-surgical therapy.<br />

Can Gum Disease be cured<br />

The outcome is good if detected early when there<br />

is minimal bone loss around the teeth. Periodontal<br />

disease is a chronic condition. There is currently no<br />

cure <strong>for</strong> this condition similar to diabetes. It has to be<br />

treated and stabilised. Professional treatment can<br />

help to arrest the condition, except in about 10%<br />

who continue to decline in spite of professional<br />

intervention.<br />

How can I detect this condition early<br />

Visit <strong>you</strong>r dentist at least once a year and request<br />

him/her to screen <strong>you</strong> <strong>for</strong> periodontal disease and to<br />

refer <strong>you</strong> to the periodontist if necessary. Periodontists<br />

are those who specialise in treating gum disease.<br />

Losing a front tooth can be embarrassing because it<br />

affects <strong>you</strong>r looks and <strong>you</strong>r speech.<br />

Losing a back tooth can affect how well <strong>you</strong> chew<br />

<strong>you</strong>r food.<br />

Apart from these, losing any tooth can adversely<br />

affect the health of <strong>you</strong>r gums and teeth.<br />

What happens when a tooth is lost<br />

The loss of tooth affects <strong>you</strong>r health in several ways.<br />

The diagram below shows an example of what can<br />

happen when a lower back tooth (molar) is lost. The<br />

teeth on either side of the empty space tilt towards<br />

the space (small arrows). The tooth in the upper jaw<br />

will continue to grow downwards or over-erupt<br />

(big arrow) because it no longer has a tooth to bite on.<br />

As a result of this tilting and over-eruption, the teeth<br />

are no longer in alignment and there are uneven<br />

contacts between the teeth. The tilting of teeth also<br />

creates areas between the teeth where food debris and<br />

bacteria collect. These areas are especially difficult to<br />

clean, so tooth decay and gum disease often develop<br />

in these areas.<br />

All these problems can be prevented by replacing the<br />

lost tooth. Replacing the lost tooth after the tilting and<br />

over-eruption have occurred, can be quite difficult.<br />

Methods of Replacing Teeth<br />

Dentures<br />

Dentures are removable artificial teeth. They should<br />

be removed after every meal and cleaned. They should<br />

also be removed when <strong>you</strong> go to sleep. Dentures are<br />

most commonly used when several teeth need to be<br />

replaced. Dentures are also the cheapest way of<br />

replacing lost teeth.<br />

Normally after an extraction, one must wait a few<br />

months <strong>for</strong> the gums and bone to heal be<strong>for</strong>e dentures<br />

can be made.<br />

In some cases, an immediate denture can be<br />

made be<strong>for</strong>e the extraction so that it can be worn<br />

immediately after the tooth is extracted. Such<br />

dentures are temporary and should be replaced<br />

within 6 months. During this period, the denture<br />

would need to be adjusted a few times to fit the<br />

healing gums and bone.<br />

Bridges<br />

Bridges are fixed artificial teeth, i.e., they are rigid<br />

and cannot be removed. A bridge can only replace a<br />

few teeth and requires a good, strong natural tooth on<br />

either side of the empty space to support the artificial<br />

teeth.<br />

Implants<br />

Implants are the latest technology available to help<br />

replace missing teeth. An implant is a device that is<br />

inserted directly into the bone at the site of the missing<br />

teeth. This requires surgery. After about 4-6 months,<br />

when the bone around the implant has healed,<br />

artificial teeth can be constructed over the implants.<br />

As implants involve surgery, extra time and new<br />

technology, they are the most expensive method of<br />

replacing teeth.<br />

Every method has its advantages and disadvantages.<br />

Your dentist is the best person to discuss these with<br />

<strong>you</strong> and help <strong>you</strong> decide the best way to replace <strong>you</strong>r<br />
