RETRACTION OF THE FORESKIN AND DEALING WITH ADHESIONS<br />
This step is common to all the methods of circumcision described<br />
below. After effective local anaesthesia has been achieved, the<br />
<strong>for</strong>eskin should be fully retracted. If the opening of the <strong>for</strong>eskin is tight,<br />
it may be necessary to dilate it with a pair of artery <strong>for</strong>ceps (Fig. 5.15),<br />
but this is not usually necessary in adults and adolescents. Care must<br />
be taken to stretch just the aperture of the <strong>for</strong>eskin and not to push the<br />
<strong>for</strong>ceps in too far, because there is a risk of dilating the urethra and<br />
causing injury to the urethra and glans.<br />
