Step 3. Close the device, crushing the <strong>for</strong>eskin. Leave in the<br />
closed position <strong>for</strong> 3–5 minutes, to reduce the risk of bleeding.<br />
If the device is left too long it may be difficult to separate the<br />
<strong>for</strong>eskin to reveal the glans after the device is removed.<br />
Step 4. Cut off the <strong>for</strong>eskin on the outer side of the clamp with<br />
a scalpel (Fig 6.17). Open the device and remove.<br />

Step 5. Manipulate the penis, using gentle pressure from the<br />
side, to allow the glans to emerge from <strong>under</strong> the crushed<br />
<strong>for</strong>eskin (Fig 6.18). This is an important step to ensure the<br />
<strong>for</strong>eskin heals below the level of the corona. In older infants<br />
(>60 days) it may be necessary to place some 5-0 simple<br />
sutures to approximate the edges.<br />
