1. EMLA cream should be applied to the penis only. The<br />

maximum areas shown are those above which toxicity is<br />

likely to occur if larger areas are coated inadvertently.<br />

2. EMLA cream will be removed when the penis is cleaned<br />

and prepared <strong>for</strong> surgery.<br />

In addition to the other agents described, oral sucrose<br />

administration (sugar water) in the amount of 1-2 mls has been<br />

reported to ameliorate the pain of circumcision. d<br />

In many developed countries vitamin K is routinely given to<br />

babies to prevent vitamin K deficiency bleeding in the newborn.<br />

Vitamin K at a dose of 1 mg intramuscularly given shortly after<br />

birth has been shown in studies in the USA to reduce bleeding<br />

after neonatal circumcision. There is a need <strong>for</strong> evaluation of<br />

oral or injectable Vitamin K in the context of neonatal<br />

circumcision programmes in developing countries.<br />

SKIN PREPARATION AND DRAPING<br />

The penis and lower abdomen should be cleaned with<br />

povidone iodine solution. If local anaesthetic injections are<br />

being used, the skin preparation should be done be<strong>for</strong>e the<br />

anaesthetic is injected. If EMLA cream is being used, skin<br />

preparation should be done 1–2 hours after the EMLA cream is<br />

applied, just be<strong>for</strong>e the procedure starts. The lower abdominal<br />

and thigh area should then be covered with a sterile operative<br />

drape with a hole to allow the penis through. The drape should<br />

not cover the baby’s face.<br />
