IMPORTANT INFORMATION<br />

1. Eligibility :<br />

Before filling the Application Form, the candidate should<br />

ascertain whether he/she fulfils all the requirements and is eligible<br />

for admission to M.Sc. Nursing Course as on the last date of<br />

submission of application. The conditions of eligibility are mentioned<br />

in the notification by Govt. of Chhattisgarh.<br />

Candidate’s Responsibility: Candidates who appear in the test<br />

would do so at their own risk and responsibility so far as their<br />

eligibility is concerned. Hence, candidates are advised to ascertain<br />

their eligibility before applying.<br />

2. Examination City :<br />

The test will be conducted at Raipur only.<br />

UNIVERSITY, AYURVEDIC COLLEGE CAMPUS,<br />

G. E. ROAD, RAIPUR (C.G.)<br />

3. Availability of Application Form :<br />

(a) The Application Form for the test is part of this posting. The two pages of the<br />

application form should be downloaded and printed on A-4 Paper by the<br />

candidates. The completed application for along with enclosures should be<br />

submitted at the office of C.G. Ayush and Health Sciences University<br />

Raipur. It should be accompanied by demand draft for Rs. 800/- (For<br />

SC/ST/OBC) or 1000/- (For all others) as the case may be in the<br />

name of “Registrar, C.G. Ayush & Health Sciences University, Raipur”<br />

payable at Raipur OR paid in cash at the University Counter.<br />

4. Last date for submission of Application Form :<br />

Duly filled application form must be sent by registered post/speed post<br />

or hand delivered so as to reach C.G. Ayush and Health Sciences<br />

University Raipur or submitted in person at C.G. Ayush and Health<br />

Sciences University Raipur on or before 05.00 PM on 13-10-2011.<br />

C.G. Ayush and Health Sciences University Raipur shall not be<br />

responsible for any postal or other delay in receipt of application form.<br />

Application forms received after due date and time will be summarily<br />

rejected.<br />

5. In-Service candidates too shall have to submit their application<br />

forms as mentioned at S. No. 4 after getting it forwarded through<br />

proper channel.<br />

PRE – P.G. (NURSING) ENTRANCE EXAMINATION -2011<br />

IMPORTANT<br />

INSTRUCTIONS AND RULES FOR CONDUCT OF TEST<br />

The information given in the Application Form will be verified<br />

at the time of counseling/admission from the original documents<br />

by the counseling/admitting authority. If it is found later that a<br />

successful applicant has provided incorrect or false information or<br />

withheld some relevant information while filling the Application<br />

Form, the applicant shall be debarred from admission or her/his<br />

admission may be cancelled at any time during the course of<br />

his/her studies, by the head of the Institution/Admitting<br />

Authority/ C.G. Government, as the case may be without giving<br />

him any notice.<br />

1 NO DOCUMENTS TO BE ENCLOSED ALONG WITH THE<br />

APPLICATION FORM<br />

2 PROCEDURE FOR FILLING APPLICATION FORM-<br />

(i) Please fill the Application Form using blue/black ball point<br />

pen only.<br />

(ii) Please paste firmly 3.5cm x 4.5 cm colored photographs of good<br />

quality with white background, carrying the name & date of<br />

birth of candidate at the two specified places in the Application<br />

form. Do not staple the photograph. Do not get it attested.<br />

Do not sign on the photograph.<br />

(iii) Make all entries in the boxes ( ) with blue/black ball point<br />

pen only. All entries must be made in English Capital letters by<br />

writing one alphabet in each box leaving one box blank between<br />

two words.<br />

3 ALL APPLICANTS WILL BE ALLOWED TO APPEAR IN T HE<br />

EXAMINATION. SCRUTINY/REJECTION OF APPLICATION<br />

FORMS WILL BE DONE AT THE TIME OF COUNSELLING.<br />

4 MATERIALS TO BE CARRIED IN THE EXAMINATION HALL<br />

(i) Test Admit Card (TAC)<br />

(ii) Blue/Black ball point pen<br />

No other material such as cellular/mobile phones,<br />

calculators, log tables, stray papers etc. should be found in the<br />

possession of the candidate in the test hall, otherwise the<br />

candidate is liable to be registered under Unfair means (UFM) and<br />

may be refused the chance to appear in the test.<br />

5 UNFAIR MEANS (UFM)<br />

Candidates are advised not to bring mobile phones to the<br />

examination center.<br />

If any candidate is found using unfair means during the<br />

examination, which includes cellular/mobile phones, calculators,<br />

log tables, stray papers etc., referring to a book/note book/loose<br />

sheets, talking, giving assistance or seeking/receiving help from<br />

any source, indulging in any malpractice or misbehavior in any<br />

manner in the test hall, harassing or doing harm to other<br />

candidates or invigilator or supervisory staff or if any action of the<br />

candidate is interpreted by the Observer/Centre Superintendent/<br />

invigilator as amounting to adopting unfair means, a case will be<br />

registered under unfair means and shall be legally dealt with<br />

accordingly. The answer sheet of the candidate booked under UFM<br />

shall not be valued and his/her candidature shall be cancelled.<br />

Additionally, any case of use of unfair means on the part of the<br />

candidate may be handed over to the police. Criminal proceedings<br />

shall be initiated against such candidates.<br />

6 NATURE OF TEST<br />

The entrance test shall be of objective type questions only,<br />

where four answers/responses will be provided for each question,<br />

out of which only one will be correct. The candidate is required to<br />

select correct answer and mark the corresponding option (A or B or<br />

C or D) on the OMR answer sheet using Blue/Black ball point<br />

pen only.<br />

7 MARKS AND TIME ALLOTTED FOR QUESTION PAPER<br />

The Question paper will contain 100 questions. Total<br />

duration of the test will be of 2 hours 15 minutes, out of which the<br />

first fifteen minutes are allotted for making the entries in the OMR<br />

answer sheet and inspecting the question booklet for any printing<br />

or other errors etc. The candidates must ensure that the question<br />

paper is complete with 100 questions.<br />

8 SYSTEM OF VALUATION<br />

(i) In objective type questions, two marks will be awarded for each<br />

correct answer. No mark (zero mark) will be awarded for an<br />

incorrect response or multiple marking or for an unanswered<br />

question. There shall be no negative marking.<br />

(ii) Marking will be done through electronic means. There is no<br />

provision of retotalling/revaluation or scrutiny.<br />

9 DEFECTIVE QUESTION, THEIR CANCELLATION AND AWARD<br />

OF MARKS IN LIEU OF CANCELLED QUESTIONS<br />

University gets each question of the paper examined by a<br />

group of subject-experts along with the answer suggested by the<br />

paper-setter and moderator. If any question is found defective by<br />

these experts, the question may be cancelled due to:-<br />

(i)<br />

(ii)<br />

(iii)<br />

(iv)<br />

Wrong structuring;<br />

More than one option being correct;<br />

None of the options being correct;<br />

Any printing error leading to a situation where more than<br />

one answer may be correct or none of the answers may be<br />

correct.<br />

The decision of the expert panel regarding the question and<br />

the correct answer will be final.<br />

In lieu of the cancelled questions, every candidate may be<br />

awarded marks in proportion to the marks obtained by him/her,<br />

irrespective of the fact whether the candidate has attempted such<br />

cancelled questions or not. For example if 2 questions out of total<br />

100 questions are cancelled in a particular question paper and if<br />

each correct answer carries 02 marks, then after evaluation it is<br />

found that the candidate has secured 90 marks from the<br />

remaining 98 questions considered for evaluation purpose, then<br />

his/her marks for 100 question are computed as under.<br />

90 x 200 / (200-4) = 91.83<br />

Note: All calculations shall be up to two decimal places and<br />

will not be rounded off.<br />

10 DECLARATION OF RESULTS<br />

University will prepare the merit list as per the admission<br />

rules mentioned in notification of Government of Chhattisgarh.<br />

The results shall be available on the website of the University<br />

www.cghealthuniv.com<br />

11 MARK SHEET<br />

University will not send individual mark sheets to the<br />

candidates. A candidate is required to download it from the<br />

website of the University www.cghealthuniv.com using his/her Roll<br />

Number.<br />

12 DUPLICATE MARK SHEET<br />

There is no provision for issue of duplicate mark sheet. A<br />

candidate can obtain any number of copies as described in #11.<br />

13 CANCELLATION OF ADMISSION<br />

If it is found that a candidate has succeeded in obtaining<br />

admission on the basis of false/incorrect information of<br />

withholding relevant facts, during counseling or at any time after<br />

admission, the admission so granted to the candidate shall be<br />

cancelled without any notice, at any time even during the course of<br />

his/her studies, by the Head of the Institute/Admitting Authority<br />

concerned or Govt. of Chhattisgarh.<br />

14 The responsibility of C.G. Ayush and Health Sciences University,<br />

Raipur is confined to the extent of conducting the Entrance Test,<br />

declaring the merit list and forwarding it to the Director, Medical<br />

Education. The C.G. Ayush and Health Sciences University, Raipur<br />

shall be final authority for deciding all policies regarding conduct<br />

of the Test. If any question relating to the interpretation of “Rules<br />

& Instructions” for the conduct of Pre P.G. (Nursing) -2011 arises,<br />

it shall be referred to the Government of Chhattisgarh whose<br />

decision shall be final and binding. The University reserves the<br />

right to amend any rule/procedure for the conduct of the Entrance<br />

Test and any modification so made shall be binding.<br />

15 Any kind of correction in information submitted in the Application<br />

form for any reason is not allowed/ accepted after the examination<br />

results have been declared.<br />

16 JURISDICTION<br />

In case of any legal disputes, the jurisdiction shall be limited to the<br />

city of Raipur only.<br />

NOTE:<br />

• Any attempt on the part of the candidate to seek favour, including<br />

canvassing will lead to disqualification and cancellation of his/her<br />

candidature.<br />

• A candidate entering the examination hall after 15 minutes from the<br />

commencement of the test i.e. after 11:15 AM shall not be permitted<br />

to take the Test.<br />

• Candidates must hand over the OMR answer sheet and the question<br />

paper to the invigilator at the end of examination, before leaving the<br />

examination hall failing which such cases shall be registered under<br />

UFM clause. The candidates are allowed only to carry away the<br />

carbon copy of the OMR answer sheet with them.<br />

• Use of any kind of unfair means (UFM) (see IMPORTANT<br />

INFORMATION # 5) is strictly prohibited.<br />
