1. DSS-5015 Questions
What is the proper way to fill out the education field (page 2, item 10) on the 5015
Post secondary years should not be added to the high school years. Put 2 years for an Associate degree
or 4 years for a four year college degree.
If a sibling group is placed together and it goes over the capacity, do we need to change the
capacity
The action you are requesting is a change in capacity. If the capacity change goes over the maximum
number of children allowed according to the administrative rules, a waiver is required. (See also the
response below under the “Waivers” category.)
Do we hand write our 5015 for a transfer
Yes, the agency receiving the foster home will need to type or write a new 5015. If you don’t have the
capacity to type it, then write legibly. Many people are able to type in the form on the computer—it can
not be saved, however.
2. DSS-5157 Questions
Do you have to complete a 5157 if you are transferring agencies
Typically, no. You only need to submit a 5157 with a transfer if there is a change as well.
