Glossary PRIMIS Facilitator Handbook Glossary Adult Learning Approach A training approach which takes account of the learner’s prior knowledge and experience and involves them in planning their own training programme. Beta testing Operational testing of query sets by facilitators in live general practice sites. CHART Care and Health Analysis in Real Time. A software program developed by PRIMIS and based on Excel that enables the quick and easy viewing of data from clinical queries. Designed to be used in-house by general practice staff. Choose and Book A programme to allow patients, in partnership with health and care professionals, to book first outpatient appointments at the most appropriate date, time and place for the patient. Chronic Disease Registers Lists of patients who have been diagnosed with a particular chronic disease, such as asthma or diabetes. Clinical Governance A framework through which NHS organisations are accountable for continually improving the quality of their services and safeguarding high standards of care by creating an environment in which excellence in clinical care will flourish. Clusters A group of strategic health authorities working together by region to implement the new technology and information systems of the National Programme for IT. EPR Electronic Patient Records. A record containing a patient’s personal details, plus information about their diagnosis, condition and treatment in an electronic format; also sometimes called EHR (Electronic Health Records). ETP Electronic Transmission of Prescriptions. A scheme to enable GPs to send prescriptions electronically to pharmacies. GMS General Medical Services.
