4.7 Safety culture and its promotion should be considered as part of contractor<br />

evaluation and monitored during the construction stage.. Monitoring and evaluation should<br />

cover not only contractors‟ organizations but also staff.<br />

4.8 To support the safety culture principles, there should be a process <strong>for</strong> reporting safety<br />

concerns directly to management and the regulatory body. This process should include the<br />

capability of anonymously reporting a non-con<strong>for</strong>mance or concern.<br />

APPLICATION OF GRADED APPROACH<br />

4.9 A graded approach based on the relative importance to safety of each item, service or<br />

process should be used during all construction activities. The graded approach should reflect a<br />

planned and recognized difference in the application of specific quality assurance<br />

requirements.<br />

4.10 The grading process should determine the extent of the application of the<br />

management system requirements to the construction activities.<br />

4.11 In developing the grading approach, the following should also be considered:<br />

(a)<br />

The qualification of special construction processes such as welding or<br />

non-destructive testing and the qualification of the personnel that will carry<br />

them out;<br />

(b)<br />

The necessary level of detail and the need <strong>for</strong> inspection and test plans;<br />

(c)<br />

The safety significance of equipment, materials, procedures, records and<br />

other documents;<br />

(d)<br />

The level of in-process controls and the need <strong>for</strong> hold or witness points;<br />
