The Best of Both Worlds
OB care by family physicians combines old-fashioned
personal care with state-of-the-art support for delivery
I
n her 26 years as a family practice physician in the
Wisconsin Dells/Lake Delton area, Dr. Maureen Murphy has
seen some of her young patients grow up and start families of
their own. And she is proud to help deliver that next generation.
What her patients seem to
like most about obstetrics care
from their family physician
is “that knowledge of them
as a person, as part of this
community, and as part of a
particular family,” Dr. Murphy
says. “It’s hard to put into words what that means for people.
But for me, it feels like being more than their doctor—almost
part of the family.”
Dr. Christina Hook says assisting in the birth of future patients
is among the favorite parts of her work. “It’s just a joy to be
there as your patients see their babies and hold them for the
first time,” says the Dean Care physician who is based at the
Dells Clinic. “That’s a real privilege.”
Drs. Murphy and Hook are among a group of primary care
physicians in Baraboo, Wisconsin Dells, and Lake Delton whose
family practice encompasses obstetrics and deliveries at
St. Clare’s Childbirth Center.
The biggest advantage to patients of family doctors who
provide obstetrics is the continuity of care, Dr. Hook suggests.
“They like that I can be their doctor before pregnancy, during
the pregnancy, and after the pregnancy, and they like that I can
be their children’s doctor as well.”
“It’s hard to put into words what that
means for people. But for me, it feels
like being more than their doctor—
almost part of the family.”
“Also, we try our best on call for our OB
patients 24/7 and to be there for the delivery”
instead of rotating after-hours OB calls among
partners, she adds.
In fact, on the Wednesday before Thanksgiving,
Dr. Sharilyn Munneke, a family physician with
Medical Associates in Baraboo, canceled a planned
afternoon off as one of her long-time patients
went into labor. “I’ve known her since she was in
high school—I take care of her entire family,” Dr.
Munneke says. “It’s
such a joy to be here
for her as she has her
first baby.”
When a physician
with one of the clinics
is away, his or her
colleagues are available
to assist in deliveries. “But we all really enjoy OB,
and we want to be there for our patients whenever
we can,” she adds.
Among the family physicians who provide
obstetrical care are Drs. Hook, David Jarvis, and
Thomas Stark at the Dells Clinic; Drs. Murphy and
Mary Ellen Sabourin at the Lake Delton Integrative
Medicine Center; and Dr. Munneke and her
colleagues at Medical Associates in Baraboo. OB
patients have the convenient option of scheduling
some of their appointments with family physicians at
any of those clinics.
At a time when many family physicians are
dropping obstetrics from their practice, women
in this area are fortunate to have local access to
family doctors who still provide OB care, says Dr.
James Damos, a family physician with Medical
Associates in Baraboo. Dr. Damos is one of several
family physicians at Medical Associates who deliver
babies at St. Clare; he also supervises deliveries by
family practice residents in his role as director of the
UW Department of Family Medicine’s Rural Physician
Training Program. Several area physicians, including
Dr. Hook, completed their medical training through
that program, which emphasizes maternity care and
medical/surgical procedures.
Dr. Murphy recalls a comment from her mentor,
Dr. Harold Conley, who practiced family medicine
in the area for many years: “OB is 90% sheer joy
“I’ve known her since she was in high
school—I take care of her entire family,”
Dr. Munneke says. “It’s such a joy to be
here for her as she has her first baby.”
and 10% sheer terror.” Actually, the risk of serious
complications is much lower than that, she adds,
“but when there is a problem, you want to be able
to move quickly” in a facility that supports medical
emergencies, and St. Clare’s Childbirth Center fills
that bill.
The center’s nurses are trained in advanced life
support for mothers and infants, and they work
well with the surgical staff when an emergency
cesarean section is needed, she notes. In addition,
St. Mary’s Neonatal Intensive Care unit is just
a phone call away, and
transport of infants who
require specialized care is
handled swiftly and safely
by an expert team that
includes a physician and/or
nurse practitioner.
St. Clare’s Childbirth
Center features private rooms for labor, delivery,
recovery, and postpartum care and specially
designed rooms for mothers who’ve undergone
cesarean section.
Pregnant women with a history of preterm labor
or other risk factors such as preexisting diabetes,
high blood pressure, or multiple births may
want to consult with Dr. Joseph Fok, a Dean Care
obstetrician who sees patients at Medical Associates.
“We have a lot of great options for women who want
to keep their OB care local and have their babies
close to home,” Dr. Hook adds.
If
you need to consult a medical
specialist to head off heart problems,
seek treatment for a neurological
condition, or get some relief from achy
joints or feet, that health care is now
closer to home. Residents in Wisconsin Dells
and Lake Delton and points north have access
to specialized medical care from preventive
cardiology to podiatry at the expanded Dells Clinic.
After focus groups in Mauston, Adams-
Friendship, and the Dells identified preventive
cardiology and neurology as the most important
medical specialists for locate locally, St. Clare
Hospital and Health Services set out to find ways to
make that possible, says President Sandy Anderson.
A new, bigger building for the Dells Clinic,
1310 Broadway, is making room for seven
specialists, who also include a general surgeon,
endocrinologist and orthopedic surgeon,
alongside facilities for primary care physicians
and St. Clare’s Urgent Care Center. St. Clare is
partnering with physicians of Dean Clinic and
the Baraboo and Dells area to expand options
for medical care to residents in a region that
encompasses Wisconsin Dells, Adams-Friendship,
Montello, Oxford, Briggsville, and Endeavor.
“We’re working on keeping health care local.
That’s important to us because it’s important
to our patients,” Anderson says. “And we will
continue to look for opportunities to expand local
access to the care area residents need.”
Keeping pace with
advances in treatment
Among the new specialists at the Dells clinic is
Dr. Jane Pearson, an internist with Dean Health
Systems who is trained in preventive cardiology
and consults with and conducts specialized blood
tests for patients who have high cholesterol or
high blood pressure and/or whose family history
puts them at risk for heart problems.
Dr. Pearson’s approach to treatment, which
includes medical therapy and advice on diet,
exercise, and lifestyle changes, reflects a
“paradigm shift” in treating heart disease and the
evolving view of heart attacks as a preventable
complication of atherosclerosis.
“We used to think of heart disease as a ‘liming
up’ of the arteries,” she explains. “But we now
know that it’s actually an inflammation of the
arteries that leads to a build-up of plaque. So we
take a much more progressive approach to manage
and reduce the risk of future heart problems.”
A variety of medications are available to help
exercise alone are not sufficient. “Every person is different, so
there’s a bit of an art to figuring out the medication combinations
that will work best for each
patient,” Dr. Pearson says.
“Also, a lot of medications
cause side effects that can be
hard to manage, and I have a lot
of experience in sorting out the
proper medication and dose to
minimize those side effects.”
Dr. Pearson treats patients
referred by their primary care physicians or cardiologists, who
then continue the course of recommended treatment. Oftentimes,
she needs only one or two appointments with patients.
Convenient access for
patients and physicians
Other medical specialists will likely have a longer term
relationship with patients. For example, Dr. Kristen Wells, a
general surgeon, sees cancer patients in regular follow-up exams
for years after the disease has been cured. Thus, her Tuesday
mornings at the Dells Clinic offer easy access for residents in
St. Clare’s northern service region.
“Everyone is pressed for time, and gas certainly isn’t cheap,” notes
Dr. Wells, who has been seeing patients in Wisconsin Dells since
January. “It’s just more convenient for patients to be able to schedule
pre-surgical and follow-up appointments at the Dells Clinic.”
Having regular hours at the Dells Clinic also makes it easier
for the primary care providers based there to consult with her,
she adds.
Podiatrist Borys Kusyk is a familiar face at the Dells Clinic,
where he has been seeing patients for the past decade. He says
to our patients. And we will continue to look
for opportunities to expand local access to
~ Sandy Anderson, St. Clare President
Dr. Bassel Kazkaz,
Neurology
the addition of new specialists, including orthopedic surgeon
Christopher Dale, endocrinologist Raul Reber and neurologists
Teresa Mangin and Bassel Kazkaz, and the expansion of the clinic
mark major improvements in
convenience and accessibility
for area residents.
“The new clinic is beautiful,
spacious, and conveniently laid
out for patients, and it’s nice
that the traveling specialists have
our own area,” says Dr. Kusyk,
who joined Baraboo Medical
Specialists in 1997 and has been treating patients in southern and
central Wisconsin for 25 years. “And the more specialists who come
here, the more convenient it is for patients. Why travel if you don’t
have to?”
Some of Dr. Kusyk’s patients are referred by their primary
physicians, but he also gets many referrals from other patients
who’ve experienced relief from foot problems after seeking
treatment from the podiatrist. “You don’t need a referral from your
physician every time you want to come see me,” he notes.
For Dr. Kusyk’s patients who require surgery, those procedures
are conducted at St. Clare. But initial and follow-up appointments
can be scheduled at the Dells Clinic. The same is true for
orthopedic, oncology, and other types of surgery.
The medical specialists seeing patients at the Dells Clinic have
schedules on different days, allowing for an arrangement similar
to a “time-share environment” to make the most efficient use of
facilities, Anderson notes.
St. Clare plans a formal open house at the Dells Clinic in April so
area residents can tour the facility and meet the physicians based
there. In the meantime if you have questions about the specialized
care now available, talk with your family physician or call
the clinic at 608-253-1171 and request a brochure.
Professionally Staffed Medical Spa Offers a Variety of Alternative Treatments
“NASA uses laser technology for injury and
“Pro sports teams are also using lasers to
help their players return to the playing field
in a much quicker time frame.”
Jess Yngsdal and Lynn Crawford, RN
St. Clare Hospital’s Healthy Living Center is more than a place to
relax, it’s a medical spa offering clients a variety of alternative
healing treatments in a peaceful, relaxing environment staffed
by medical professionals. The Healthy Living Center opened
its doors one year ago with its first program: Medical
Weight Loss Management.
“We use a team approach” explained Debi Rodriguez,
NP, Coordinator of the Healthy Living Center. “A nurse
practitioner does the initial consultation, and our
registered dieticians work to develop a healthy eating
plan appropriate for each individual.”
Each client meets with an exercise physiologist
for an individualized exercise program. The weight
loss program stresses a healthy diet, exercise, group
sessions and monitoring over a 2 1/2 year period.
“Everyone has different expectations,” said Rodriguez.
“So our program is not one size fits all.”
The Healthy Living Center has expanded its offerings
to include aromatherapy and M technique with hot
and cold stones, Reiki, and
laser therapy.
The aromatherapy program
utilizes essential oils that
are specific to the clients’
needs. “Many people think
that you choose the oil based
on the scent that you like,”
explained Rodriguez. “We
listen to our clients and when
they tell us about a condition
that they would like to correct or alter, we are able to
choose an oil that will produce a specific outcome.”
Aromatherapy is particularly useful to alleviate stress,
encourage relaxation, and lessen pain.
Used in conjunction with aromatherapy, the
M technique of touch massage can provide rapid
and prolonged relaxation. The M technique follows
a set structure. Each movement and sequence is
done in a distinct pattern and with a set pressure
that is never modified.
Reiki (pronounced ray-key) is a simple and
safe method of energy work. The word reiki is a
combination of two Japanese words: rei which means
“universal,” and ki which is “energy.”
Reiki is effective in helping with relaxation, and
also works well in conjunction with other medical or
therapeutic processes to promote recovery, alleviate pain,
and encourage overall well-being. The practice of reiki
is based on
the belief that
there is a universal
energy that supports the body’s
innate healing abilities. Reiki practitioners
access this energy to facilitate healing of the body.
Laser therapy is the newest program at the Healthy
Living Center and is growing in popularity. Typically,
patients come in for eight treatments to alleviate pain
from shingles and plantar fasciitis to repetitive stress
injuries and migraines. Treatments work at the cellular
level to promote healing. Therapeutic lasers decrease
inflammation, decrease pain and healing time.
“NASA uses laser technology for injury and wound
healing in space,” said Rodriguez. “Pro sports teams
are also using lasers to help their players return to the
playing field in a much quicker time frame.”
St. Clare Receives
New Certification by
Joint Commission
After a two-year-long process, St. Clare Hospital has received Joint
Commission certification for their Inpatient Diabetes Program.
“We are the first hospital in Wisconsin to receive this distinction,” said
Laura Jelle, Vice President of Operations and Organizational Effectiveness.
“And we are only the 24th hospital in the nation to have been recognized.”
The Joint Commission is
an accreditation agency that
certifies more than 18,000
health care organizations
in the United States. Joint
Commission certification
“We are the first hospital
in Wisconsin to receive
this distinction”
~Laura Jelle
and accreditation is recognized as a symbol of quality that reflects an
organization’s commitment to meeting and maintaining certain standards.
“St. Clare sees a lot of patients who have diabetes as their primary or secondary
diagnosis,” said Barb Bair, Director of Clinical Resource Management. “Going
through the process to meet the standards forced us to look at how we were
treating diabetes. It brought about improvements in so many areas.”
Among the areas affected were lab work, the timing of insulin, checking
blood sugars, and improved treatment and management of hypoglycemia.
“This was a team effort,” explained Bair. “We pulled together different
disciplines; pharmacists, dieticians, nurses and statistics professionals, to
make it happen.”
Jelle and Bair met monthly with Melanie Mielke, Diabetes Educator at St. Clare.
“Our goal was to provide patients the best possible care while in the hospital,
and also to make sure that they left the hospital with the skills necessary to take
care of their diabetes,” explained Mielke. “We knew that we had improved our
program, but we were very honored to receive the certification.”
According to Mielke, Laura Jelle had the vision, and Barb Bair did a
wonderful job of organization. “St. Clare is truly a leader,” said Mielke. “We’ve
already had other hospitals call and want to talk to us about the process.”
St. Clare received Joint Commission certification for its Outpatient
Diabetes program last spring, and was recently granted recertification for its
Congestive Heart Failure (CHF) program.
Laura Jelle, Barb Bair and Melanie Mielke
Larry Skwor and Greg Halvorson
This year St. Clare Hospital will say goodbye to two of its long-time and valued
employees, Larry Skwor, Director of Medical Imaging, and Greg Halvorson,
Clinical Coordinator, Pharmacy.
Larry Skwor has been at St. Clare for 38 years. “Larry has been instrumental in
developing a variety of service line modalities at St. Clare,” said Laura Jelle, Vice
President of Operations/Organizational Effectiveness. “He is a cornerstone of St. Clare
Hospital and will be missed by the entire hospital and medical staff.”
Skwor remembered when the imaging department had three x-ray rooms and a very
small waiting room. “It was very basic,” recalled Skwor. “The biggest change to come
about in the medical imaging department has been the implementation of the women’s
program, which now includes digital mammography and stereotatic breast biopsies.”
“I’ll miss the people the most,” said Skwor. “The x-ray technicians, physicians, and
most of all, the patients, many of whom recognize me by name.”
Greg Halvorson has called St. Clare home for over 36 years. “We went from being a
hospital with little information to a hospital with the most advanced clinical electronic
health record available,” said Halvorson.
Halvorson has held several different positions while at St. Clare Hospital, including
pharmacist, staff pharmacist, director of pharmacy, and director of pharmacy materials
management. Halvorson returned to clinical pharmacy in 1999.
“When I was looking for a position, I wanted to find a place that concentrated on
patient care,” explained Halvorson. “The Sisters lived their mission; they had an innate
ability to put together people who could fulfill the mission of the Sisters of St. Mary’s.
Though there are few Sisters around St. Clare today, Halvorson says that the St. Clare
administration has maintained the mission, and that it is a value-driven business.
“When you care about patients and people, you naturally draw others who want to
practice in that type of environment,” said Halvorson.
“It’s been a pleasure to know Greg Halvorson for over 30 years,” said Peter Vrabec,
Director of Pharmacy. “And a greater pleasure to have worked for and with him. He
is a true professional pharmacist, and has been an incredible role model for student
pharmacists who had done clinical rotations at our hospital as pharmacy students,
and are now practicing pharmacists in locations all over the country. But most importantly,
he has always placed the care and treatment of our patients first, working
many long hours and many long days, taking calls in the middle of the night, coming
back to the hospital, if needed when an emergency arose, and has done so while
continuing to maintain a high level of clinical expertise throughout all these
years. He’s one of a kind, a true friend, an honorable colleague, and will be
deeply missed by me and all in our department and at our hospital.”
