OB care by family physicians combines old-fashioned<br />
personal care with state-<strong>of</strong>-the-art support for delivery<br />
I<br />
n her 26 years as a family practice physician in the<br />
Wisconsin Dells/Lake Delton area, Dr. Maureen Murphy has<br />
seen some <strong>of</strong> her young patients grow up and start families <strong>of</strong><br />
their own. And she is proud to help deliver that next generation.<br />
What her patients seem to<br />
like most about obstetrics care<br />
from their family physician<br />
is “that knowledge <strong>of</strong> them<br />
as a person, as part <strong>of</strong> this<br />
community, and as part <strong>of</strong> a<br />
particular family,” Dr. Murphy<br />
says. “It’s hard to put into words what that means for people.<br />
But for me, it feels like being more than their doctor—almost<br />
part <strong>of</strong> the family.”<br />
Dr. Christina Hook says assisting in the birth <strong>of</strong> future patients<br />
is among the favorite parts <strong>of</strong> her work. “It’s just a joy to be<br />
there as your patients see their babies and hold them for the<br />
first time,” says the Dean <strong>Care</strong> physician who is based at the<br />
Dells Clinic. “That’s a real privilege.”<br />
Drs. Murphy and Hook are among a group <strong>of</strong> primary care<br />
physicians in Baraboo, Wisconsin Dells, and Lake Delton whose<br />
family practice encompasses obstetrics and deliveries at<br />
St. Clare’s Childbirth Center.<br />
<strong>The</strong> biggest advantage to patients <strong>of</strong> family doctors who<br />
provide obstetrics is the continuity <strong>of</strong> care, Dr. Hook suggests.<br />
“<strong>The</strong>y like that I can be their doctor before pregnancy, during<br />
the pregnancy, and after the pregnancy, and they like that I can<br />
be their children’s doctor as well.”<br />

instead <strong>of</strong> rotating after-hours OB calls among<br />
partners, she adds.<br />
In fact, on the Wednesday before Thanksgiving,<br />
Dr. Sharilyn Munneke, a family physician with<br />
Medical Associates in Baraboo, canceled a planned<br />
afternoon <strong>of</strong>f as one <strong>of</strong> her long-time patients<br />
went into labor. “I’ve known her since she was in<br />
high school—I take care <strong>of</strong> her entire family,” Dr.<br />
Munneke says. “It’s<br />
such a joy to be here<br />
for her as she has her<br />
first baby.”<br />
When a physician<br />
with one <strong>of</strong> the clinics<br />
is away, his or her<br />
colleagues are available<br />
to assist in deliveries. “But we all really enjoy OB,<br />
and we want to be there for our patients whenever<br />
we can,” she adds.<br />
Among the family physicians who provide<br />
obstetrical care are Drs. Hook, David Jarvis, and<br />
Thomas Stark at the Dells Clinic; Drs. Murphy and<br />
Mary Ellen Sabourin at the Lake Delton Integrative<br />
Medicine Center; and Dr. Munneke and her<br />
colleagues at Medical Associates in Baraboo. OB<br />
patients have the convenient option <strong>of</strong> scheduling<br />
some <strong>of</strong> their appointments with family physicians at<br />
any <strong>of</strong> those clinics.<br />
At a time when many family physicians are<br />
dropping obstetrics from their practice, women<br />
in this area are fortunate to have local access to<br />
family doctors who still provide OB care, says Dr.<br />
James Damos, a family physician with Medical<br />
Associates in Baraboo. Dr. Damos is one <strong>of</strong> several<br />
family physicians at Medical Associates who deliver<br />
babies at St. Clare; he also supervises deliveries by<br />
family practice residents in his role as director <strong>of</strong> the<br />
UW Department <strong>of</strong> Family Medicine’s Rural Physician<br />
Training Program. Several area physicians, including<br />
Dr. Hook, completed their medical training through<br />
that program, which emphasizes maternity care and<br />
medical/surgical procedures.<br />
Dr. Murphy recalls a comment from her mentor,<br />
Dr. Harold Conley, who practiced family medicine<br />
in the area for many years: “OB is 90% sheer joy<br />

Dr. Munneke says. “It’s such a joy to be<br />
here for her as she has her first baby.”<br />

and 10% sheer terror.” Actually, the risk <strong>of</strong> serious<br />
complications is much lower than that, she adds,<br />
“but when there is a problem, you want to be able<br />
to move quickly” in a facility that supports medical<br />
emergencies, and St. Clare’s Childbirth Center fills<br />
that bill.<br />
<strong>The</strong> center’s nurses are trained in advanced life<br />
support for mothers and infants, and they work<br />
well with the surgical staff when an emergency<br />
cesarean section is needed, she notes. In addition,<br />
St. Mary’s Neonatal Intensive <strong>Care</strong> unit is just<br />
a phone call away, and<br />
transport <strong>of</strong> infants who<br />
require specialized care is<br />
handled swiftly and safely<br />
by an expert team that<br />
includes a physician and/or<br />
nurse practitioner.<br />
St. Clare’s Childbirth<br />
Center features private rooms for labor, delivery,<br />
recovery, and postpartum care and specially<br />
designed rooms for mothers who’ve undergone<br />
cesarean section.<br />
Pregnant women with a history <strong>of</strong> preterm labor<br />
or other risk factors such as preexisting diabetes,<br />
high blood pressure, or multiple births may<br />
want to consult with Dr. Joseph Fok, a Dean <strong>Care</strong><br />
obstetrician who sees patients at Medical Associates.<br />
“We have a lot <strong>of</strong> great options for women who want<br />
to keep their OB care local and have their babies<br />
close to home,” Dr. Hook adds.<br />
If<br />
you need to consult a medical<br />
specialist to head <strong>of</strong>f heart problems,<br />
seek treatment for a neurological<br />
condition, or get some relief from achy<br />
joints or feet, that health care is now<br />
closer to home. Residents in Wisconsin Dells<br />
and Lake Delton and points north have access<br />
to specialized medical care from preventive<br />
cardiology to podiatry at the expanded Dells Clinic.<br />
After focus groups in Mauston, Adams-<br />
Friendship, and the Dells identified preventive<br />
cardiology and neurology as the most important<br />
medical specialists for locate locally, St. Clare<br />
Hospital and <strong>Health</strong> Services set out to find ways to<br />
make that possible, says President Sandy Anderson.<br />
A new, bigger building for the Dells Clinic,<br />
1310 Broadway, is making room for seven<br />
specialists, who also include a general surgeon,<br />
endocrinologist and orthopedic surgeon,<br />
alongside facilities for primary care physicians<br />
and St. Clare’s Urgent <strong>Care</strong> Center. St. Clare is<br />
partnering with physicians <strong>of</strong> Dean Clinic and<br />
the Baraboo and Dells area to expand options<br />
for medical care to residents in a region that<br />
encompasses Wisconsin Dells, Adams-Friendship,<br />
Montello, Oxford, Briggsville, and Endeavor.<br />
“We’re working on keeping health care local.<br />
That’s important to us because it’s important<br />
to our patients,” Anderson says. “And we will<br />
continue to look for opportunities to expand local<br />
access to the care area residents need.”<br />
Keeping pace with<br />
advances in treatment<br />
Among the new specialists at the Dells clinic is<br />
Dr. Jane Pearson, an internist with Dean <strong>Health</strong><br />
Systems who is trained in preventive cardiology<br />
and consults with and conducts specialized blood<br />
tests for patients who have high cholesterol or<br />
high blood pressure and/or whose family history<br />
puts them at risk for heart problems.<br />
Dr. Pearson’s approach to treatment, which<br />
includes medical therapy and advice on diet,<br />
exercise, and lifestyle changes, reflects a<br />
“paradigm shift” in treating heart disease and the<br />
evolving view <strong>of</strong> heart attacks as a preventable<br />
complication <strong>of</strong> atherosclerosis.<br />
“We used to think <strong>of</strong> heart disease as a ‘liming<br />
up’ <strong>of</strong> the arteries,” she explains. “But we now<br />
know that it’s actually an inflammation <strong>of</strong> the<br />
arteries that leads to a build-up <strong>of</strong> plaque. So we<br />
take a much more progressive approach to manage<br />
and reduce the risk <strong>of</strong> future heart problems.”<br />

lower cholesterol levels and high blood pressure when diet and<br />
exercise alone are not sufficient. “Every person is different, so<br />
there’s a bit <strong>of</strong> an art to figuring out the medication combinations<br />
that will work best for each<br />
patient,” Dr. Pearson says.<br />
“Also, a lot <strong>of</strong> medications<br />
cause side effects that can be<br />
hard to manage, and I have a lot<br />
<strong>of</strong> experience in sorting out the<br />
proper medication and dose to<br />
minimize those side effects.”<br />
Dr. Pearson treats patients<br />
referred by their primary care physicians or cardiologists, who<br />
then continue the course <strong>of</strong> recommended treatment. Oftentimes,<br />
she needs only one or two appointments with patients.<br />
Convenient access for<br />
patients and physicians<br />
Other medical specialists will likely have a longer term<br />
relationship with patients. For example, Dr. Kristen Wells, a<br />
general surgeon, sees cancer patients in regular follow-up exams<br />
for years after the disease has been cured. Thus, her Tuesday<br />
mornings at the Dells Clinic <strong>of</strong>fer easy access for residents in<br />
St. Clare’s northern service region.<br />
“Everyone is pressed for time, and gas certainly isn’t cheap,” notes<br />
Dr. Wells, who has been seeing patients in Wisconsin Dells since<br />
January. “It’s just more convenient for patients to be able to schedule<br />
pre-surgical and follow-up appointments at the Dells Clinic.”<br />
Having regular hours at the Dells Clinic also makes it easier<br />
for the primary care providers based there to consult with her,<br />
she adds.<br />

where he has been seeing patients for the past decade. He says<br />

Christopher Dale, endocrinologist Raul Reber and neurologists<br />
Teresa Mangin and Bassel Kazkaz, and the expansion <strong>of</strong> the clinic<br />
mark major improvements in<br />
convenience and accessibility<br />
for area residents.<br />
“<strong>The</strong> new clinic is beautiful,<br />
spacious, and conveniently laid<br />
out for patients, and it’s nice<br />
that the traveling specialists have<br />
our own area,” says Dr. Kusyk,<br />
who joined Baraboo Medical<br />
Specialists in 1997 and has been treating patients in southern and<br />
central Wisconsin for 25 years. “And the more specialists who come<br />
here, the more convenient it is for patients. Why travel if you don’t<br />
have to?”<br />
Some <strong>of</strong> Dr. Kusyk’s patients are referred by their primary<br />
physicians, but he also gets many referrals from other patients<br />
who’ve experienced relief from foot problems after seeking<br />
treatment from the podiatrist. “You don’t need a referral from your<br />
physician every time you want to come see me,” he notes.<br />
For Dr. Kusyk’s patients who require surgery, those procedures<br />
are conducted at St. Clare. But initial and follow-up appointments<br />
can be scheduled at the Dells Clinic. <strong>The</strong> same is true for<br />
orthopedic, oncology, and other types <strong>of</strong> surgery.<br />
<strong>The</strong> medical specialists seeing patients at the Dells Clinic have<br />
schedules on different days, allowing for an arrangement similar<br />
to a “time-share environment” to make the most efficient use <strong>of</strong><br />
facilities, Anderson notes.<br />
St. Clare plans a formal open house at the Dells Clinic in April so<br />
area residents can tour the facility and meet the physicians based<br />

St. Clare Hospital’s <strong>Health</strong>y Living Center is more than a place to<br />
relax, it’s a medical spa <strong>of</strong>fering clients a variety <strong>of</strong> alternative<br />
healing treatments in a peaceful, relaxing environment staffed<br />
by medical pr<strong>of</strong>essionals. <strong>The</strong> <strong>Health</strong>y Living Center opened<br />
its doors one year ago with its first program: Medical<br />
Weight Loss Management.<br />
“We use a team approach” explained Debi Rodriguez,<br />
NP, Coordinator <strong>of</strong> the <strong>Health</strong>y Living Center. “A nurse<br />
practitioner does the initial consultation, and our<br />
registered dieticians work to develop a healthy eating<br />
plan appropriate for each individual.”<br />
Each client meets with an exercise physiologist<br />
for an individualized exercise program. <strong>The</strong> weight<br />
loss program stresses a healthy diet, exercise, group<br />
sessions and monitoring over a 2 1/2 year period.<br />
“Everyone has different expectations,” said Rodriguez.<br />
“So our program is not one size fits all.”<br />
<strong>The</strong> <strong>Health</strong>y Living Center has expanded its <strong>of</strong>ferings<br />
to include aromatherapy and M technique with hot<br />
and cold stones, Reiki, and<br />
laser therapy.<br />
<strong>The</strong> aromatherapy program<br />
utilizes essential oils that<br />
are specific to the clients’<br />
needs. “Many people think<br />
that you choose the oil based<br />
on the scent that you like,”<br />
explained Rodriguez. “We<br />
listen to our clients and when<br />
they tell us about a condition<br />
that they would like to correct or alter, we are able to<br />
choose an oil that will produce a specific outcome.”<br />
Aromatherapy is particularly useful to alleviate stress,<br />
encourage relaxation, and lessen pain.<br />
Used in conjunction with aromatherapy, the<br />
M technique <strong>of</strong> touch massage can provide rapid<br />
and prolonged relaxation. <strong>The</strong> M technique follows<br />
a set structure. Each movement and sequence is<br />
done in a distinct pattern and with a set pressure<br />
that is never modified.<br />
Reiki (pronounced ray-key) is a simple and<br />
safe method <strong>of</strong> energy work. <strong>The</strong> word reiki is a<br />
combination <strong>of</strong> two Japanese words: rei which means<br />
“universal,” and ki which is “energy.”<br />
Reiki is effective in helping with relaxation, and<br />
also works well in conjunction with other medical or<br />
therapeutic processes to promote recovery, alleviate pain,<br />
and encourage overall well-being. <strong>The</strong> practice <strong>of</strong> reiki<br />
is based on<br />
the belief that<br />
there is a universal<br />
energy that supports the body’s<br />
innate healing abilities. Reiki practitioners<br />
access this energy to facilitate healing <strong>of</strong> the body.<br />
Laser therapy is the newest program at the <strong>Health</strong>y<br />
Living Center and is growing in popularity. Typically,<br />
patients come in for eight treatments to alleviate pain<br />
from shingles and plantar fasciitis to repetitive stress<br />
injuries and migraines. Treatments work at the cellular<br />
level to promote healing. <strong>The</strong>rapeutic lasers decrease<br />
inflammation, decrease pain and healing time.<br />

New Certification by<br />
Joint Commission<br />
After a two-year-long process, St. Clare Hospital has received Joint<br />
Commission certification for their Inpatient Diabetes Program.<br />
“We are the first hospital in Wisconsin to receive this distinction,” said<br />
Laura Jelle, Vice President <strong>of</strong> Operations and Organizational Effectiveness.<br />
“And we are only the 24th hospital in the nation to have been recognized.”<br />

and accreditation is recognized as a symbol <strong>of</strong> quality that reflects an<br />
organization’s commitment to meeting and maintaining certain standards.<br />
“St. Clare sees a lot <strong>of</strong> patients who have diabetes as their primary or secondary<br />
diagnosis,” said Barb Bair, Director <strong>of</strong> Clinical Resource Management. “Going<br />
through the process to meet the standards forced us to look at how we were<br />
treating diabetes. It brought about improvements in so many areas.”<br />
Among the areas affected were lab work, the timing <strong>of</strong> insulin, checking<br />
blood sugars, and improved treatment and management <strong>of</strong> hypoglycemia.<br />
“This was a team effort,” explained Bair. “We pulled together different<br />
disciplines; pharmacists, dieticians, nurses and statistics pr<strong>of</strong>essionals, to<br />
make it happen.”<br />
Jelle and Bair met monthly with Melanie Mielke, Diabetes Educator at St. Clare.<br />
“Our goal was to provide patients the best possible care while in the hospital,<br />
and also to make sure that they left the hospital with the skills necessary to take<br />
care <strong>of</strong> their diabetes,” explained Mielke. “We knew that we had improved our<br />
program, but we were very honored to receive the certification.”<br />
According to Mielke, Laura Jelle had the vision, and Barb Bair did a<br />
wonderful job <strong>of</strong> organization. “St. Clare is truly a leader,” said Mielke. “We’ve<br />
already had other hospitals call and want to talk to us about the process.”<br />
St. Clare received Joint Commission certification for its Outpatient<br />
Diabetes program last spring, and was recently granted recertification for its<br />
Congestive Heart Failure (CHF) program.<br />

This year St. Clare Hospital will say goodbye to two <strong>of</strong> its long-time and valued<br />
employees, Larry Skwor, Director <strong>of</strong> Medical Imaging, and Greg Halvorson,<br />
Clinical Coordinator, Pharmacy.<br />
Larry Skwor has been at St. Clare for 38 years. “Larry has been instrumental in<br />
developing a variety <strong>of</strong> service line modalities at St. Clare,” said Laura Jelle, Vice<br />
President <strong>of</strong> Operations/Organizational Effectiveness. “He is a cornerstone <strong>of</strong> St. Clare<br />
Hospital and will be missed by the entire hospital and medical staff.”<br />
Skwor remembered when the imaging department had three x-ray rooms and a very<br />
small waiting room. “It was very basic,” recalled Skwor. “<strong>The</strong> biggest change to come<br />
about in the medical imaging department has been the implementation <strong>of</strong> the women’s<br />
program, which now includes digital mammography and stereotatic breast biopsies.”<br />
“I’ll miss the people the most,” said Skwor. “<strong>The</strong> x-ray technicians, physicians, and<br />
most <strong>of</strong> all, the patients, many <strong>of</strong> whom recognize me by name.”<br />
Greg Halvorson has called St. Clare home for over 36 years. “We went from being a<br />
hospital with little information to a hospital with the most advanced clinical electronic<br />
health record available,” said Halvorson.<br />
Halvorson has held several different positions while at St. Clare Hospital, including<br />
pharmacist, staff pharmacist, director <strong>of</strong> pharmacy, and director <strong>of</strong> pharmacy materials<br />
management. Halvorson returned to clinical pharmacy in 1999.<br />
“When I was looking for a position, I wanted to find a place that concentrated on<br />
patient care,” explained Halvorson. “<strong>The</strong> Sisters lived their mission; they had an innate<br />
ability to put together people who could fulfill the mission <strong>of</strong> the Sisters <strong>of</strong> St. Mary’s.<br />
Though there are few Sisters around St. Clare today, Halvorson says that the St. Clare<br />
administration has maintained the mission, and that it is a value-driven business.<br />
“When you care about patients and people, you naturally draw others who want to<br />
practice in that type <strong>of</strong> environment,” said Halvorson.<br />
“It’s been a pleasure to know Greg Halvorson for over 30 years,” said Peter Vrabec,<br />
Director <strong>of</strong> Pharmacy. “And a greater pleasure to have worked for and with him. He<br />
is a true pr<strong>of</strong>essional pharmacist, and has been an incredible role model for student<br />
pharmacists who had done clinical rotations at our hospital as pharmacy students,<br />
and are now practicing pharmacists in locations all over the country. But most importantly,<br />
he has always placed the care and treatment <strong>of</strong> our patients first, working<br />
many long hours and many long days, taking calls in the middle <strong>of</strong> the night, coming<br />
back to the hospital, if needed when an emergency arose, and has done so while<br />
continuing to maintain a high level <strong>of</strong> clinical expertise throughout all these<br />
years. He’s one <strong>of</strong> a kind, a true friend, an honorable colleague, and will be<br />
deeply missed by me and all in our department and at our hospital.”<br />

<strong>The</strong> St. Clare <strong>Health</strong> <strong>Care</strong> Foundation honors the memory<br />
<strong>of</strong> Golden Oak Legacy Society member <strong>The</strong>lma Balthazor.<br />
<strong>The</strong>lma had a special place in her heart for St. Clare Hospital & <strong>Health</strong> Services since<br />
1969, when her only child, David Thom, spent the last three months <strong>of</strong> his life at<br />
St. Clare, with his mother by his side around the clock. After David’s death, <strong>The</strong>lma<br />
decided to name the hospital in her estate plans. “I want to give something<br />
back to St. Clare where everyone will be able to benefit,” she<br />
said. Through her Golden Oak Legacy Society membership,<br />
<strong>The</strong>lma’s generosity will be directed to the St. Clare <strong>Health</strong><br />
<strong>Care</strong> Foundation’s endowment. <strong>The</strong>lma will be greatly<br />
