To redeem this offer:
1) Purchase between 5/1/11 and 5/31/11 KODAK PROFESSIONAL Inkjet
Photo Paper.
2) Fill out the information above.
3) Cut out the entire Universal Product Code (UPC/CAT No.) for the
KODAK PROFESSIONAL Inkjet Photo Paper.
4) No later than June 30, 2011, mail to the address shown: a) the completed
rebate redemption form, b) copies of sales receipt or internet invoice
showing proof of purchase, date of purchase (circle the items purchased)
and purchase price, and c) the UPC/CAT number cut out.

UPC barcodes must represent submitter’s actual consumer purchases. Offer is non-transferable. Copies of redemption order
form or proofs of purchase or UPC barcodes not accepted. Limit one (1) request per household, address, group or organization.
Maximum rebate value per household, address, group or organization is $100. Offer void where prohibited by law and is limited to
residents of the 50 United States and the District of Columbia (excluding Puerto Rico). Offer excludes resellers of Kodak products
and their families. Submitting original UPC barcodes, copies of sales receipts or internet invoices and completed rebate form
entitles you to receive a check without recourse to the drawer. All obligations of Kodak or its agent will be discharged by issuing
