years of age or older must sign and submit a Form HUD-9886, “Authorization for<br />
the Release of Information” and other release forms necessary for the <strong>Housing</strong><br />
<strong>Authority</strong> to verify the family’s income and other eligibility factors.<br />
a. Any activity described as follows by any household member, which<br />
did or did not result in an arrest and/or conviction, will be considered<br />
grounds for denial of assistance:<br />
1. Evidence that a household member is currently engaging in or has<br />
engaged in drug-related criminal activity; or reasonable cause to<br />
believe that illegal drug use or a pattern of illegal drug use may<br />
adversely affect the health or safety of, or the right to peaceful<br />
enjoyment of, the premises by other residents; or<br />
2. Evidence that a household member is currently engaging in or has<br />
engaged in violent criminal activity; or other criminal activity which<br />
may threaten the health or safety of, or the right to peaceful<br />
enjoyment of, the premises by other residents; or other criminal<br />
activity which may threaten the health or safety of <strong>Authority</strong><br />
employees or persons performing management functions on<br />
behalf of the <strong>Authority</strong>.<br />
3. Evidence that a household member’s abuse or pattern of abuse of<br />
alcohol interferes with the health, safety of, or the right to peaceful<br />
enjoyment of, the premises by other residents.<br />
4. Evidence that a household member has been evicted within the<br />
last five (5) years from any federal assisted housing program for<br />
drug and/or criminal related activity.<br />
5. Evidence that a household member owes rent or other amounts,<br />
or has committed fraud, bribery, or misrepresented eligibility<br />
information to SAHA, or any other PHA, or owner in connection<br />
with any assisted housing program. The applicant must satisfy in<br />
full any overdue accounts or indebtedness owned SAHA resulting<br />
from previous tenancies in any housing development or housing<br />
program administered or managed by the SAHA. There must be<br />
documentation in the tenant file and computer records supporting<br />
the applicant paid in full any outstanding debt owed to any other<br />
public housing authority as reported to SAHA via HUD’s Enterprise<br />
Income Verification (EIV) system or other database.<br />
6. The applicant’s previous lease with SAHA (if any) must not have<br />
been terminated for cause within the previous:<br />
i. 36 months for a default in the payment of rent or charges;<br />
ii.<br />
iii.<br />
60 months for a termination due to drug related criminal<br />
activity or criminal activity including crimes of physical<br />
violence to persons or property, crimes that adversely<br />
affected the health, safety or welfare of other tenants,<br />
60 months for a termination for failing to report family<br />
income or composition; or<br />
