8. The applicant must provide the complete and accurate Social Security numbers and the<br />
documentation to verify each Social Security number assigned to the applicant and to each<br />
member of the applicant’s household, regardless of age, who claim to have legal residency<br />
or citizenship status.<br />
9. The applicant, spouse, and each member of the applicant’s household who is 18 years of<br />
age or older must sign and submit a Form HUD-9886, “Authorization for the Release of<br />
Information” and other release forms necessary for the <strong>Housing</strong> <strong>Authority</strong> to verify the<br />
family’s income and other eligibility factors.<br />
D. Application Procedure<br />
1. Any family that wishes to reside in public housing must apply for admission to the program<br />
[24 CFR 1.4(b)(2)(ii), 24 CFR 960.202(a)(2)(iv), and PH Occ GB, p. 68].<br />
2. The PHA uses a two-step process in selecting families from the waiting list. The PHA<br />
initially will require families to provide only the information needed to make an initial<br />
assessment of the family’s eligibility, preference, and to determine the family’s placement<br />
on the waiting list. The family will be required to provide all of the information necessary to<br />
establish family eligibility and the amount of rent the family will pay at the Eligibility<br />
Interview.<br />
3. Families may obtain application forms from the PHA’s office during normal business hours.<br />
Families may also request – by telephone or by mail – that a form be sent to the family via<br />
first class mail. Completed pre-applications must be returned to the PHA by mail, by fax, or<br />
submitted in person during normal business hours. Pre-Applications must be complete in<br />
order to be accepted by the PHA for processing. If a pre-application is incomplete, the PHA<br />
will notify the family of the additional information required.<br />
4. At the very least, the SAHA will verify the following:<br />
a. Family size, composition, ages and identity of family members.<br />
b. Employment and student status, wages, and date of employment.<br />
c. Any condition(s) which allow an applicant a preference or determines<br />
eligibility.<br />
d. Assigned Social Security numbers for the applicant(s) and each member of<br />
the applicant’s household.<br />
e. If any member of the family is currently residing in assisted housing or if the<br />
family has outstanding debt to another public housing agency as reported in<br />
HUD’s EIV System.<br />
E. Removal of Applicant from the Waiting List<br />
1. Under any of the following conditions, applicants will be removed from the waiting list:<br />
a. The applicant has already accepted an offer of public housing from the<br />
SAHA;<br />
b. The applicant requests that their application be removed, such request must<br />
be in writing;<br />
c. The applicant failed to respond within the 15 day time period to the SAHA’s<br />
first class mail correspondence to confirm their continued interest in public<br />
housing during an update of the waiting list;<br />
d. The applicant failed to respond and attend an pre-occupancy orientation, as<br />
required for admittance into the Public <strong>Housing</strong> Program;<br />
e. The applicant has not responded to reasonable efforts to schedule an<br />
interview to complete the application or to provide clarification to information<br />
provided on their application; or<br />
f. The applicant failed to keep a scheduled interview or failed to respond to the<br />
SAHA regarding information that is necessary to process the application or to<br />
remain on the waiting list.<br />
