‘‘(7) FUNDING.—<br />
‘‘(A) IN GENERAL.—There are appropriated,<br />
from amounts in the Federal Supplementary<br />
Medical Insurance Trust Fund under<br />
section 1841 not otherwise appropriated,<br />
$2,000,000,000 for the purposes described in<br />
subparagraph (B), <strong>of</strong> which no more than 2.5<br />
percent may be used for the purpose described<br />
in clause (iii) <strong>of</strong> such subparagraph. Amounts<br />
transferred under this subparagraph shall be<br />
available until expended.<br />
‘‘(B) PURPOSES.—Amounts appropriated<br />
under subparagraph (A) shall be used for—<br />
‘‘(i) payments for items and services<br />
furnished by participating entities under<br />
