3 volved data reported under subparagraph (C)<br />
4 and such other data as deemed necessary to<br />
5 evaluate the model.<br />

‘‘(E) FINAL EVALUATION.—Not later than<br />
7 6 months after the date <strong>of</strong> completion <strong>of</strong> a dem-<br />
8 onstration program, the independent evaluation<br />
9 entity shall submit to the Secretary, the Medi-<br />
10 care Payment Advisory Commission, and the<br />
11 Chief Actuary <strong>of</strong> the Centers for Medicare &<br />
12 Medicaid Services (and make publicly available)<br />
13 a report on each model evaluated under such<br />
14 program. Such report shall include—<br />
15<br />
‘‘(i) outcomes on the clinical and<br />
16<br />
claims data received through such program<br />
17<br />
with respect to such model;<br />
18<br />
‘‘(ii) recommendations on—<br />
19<br />
‘‘(I) whether or not such model<br />
20<br />
should be implemented as an eligible<br />
21<br />
APM under this section; or<br />
22<br />
‘‘(II) whether or not the evalua-<br />
23<br />
tion <strong>of</strong> such model under the dem-<br />
24<br />
onstration program should be ex-<br />
25<br />
tended or expanded;<br />
