‘‘(I) to reduce spending under<br />
this title without reducing the quality<br />
<strong>of</strong> care; or<br />
‘‘(II) improve the quality <strong>of</strong> patient<br />
care without increasing spending;<br />
‘‘(v) The proposal for such model<br />
demonstrates—<br />
‘‘(I) the potential to successfully<br />
manage the cost <strong>of</strong> furnishing items<br />
and services under this title so as to<br />
not result in expenditures under this<br />
title for individuals participating<br />
under such APM being greater than<br />
expenditures under this title for such<br />
individuals if the APM were not implemented;<br />
‘‘(II) the ability to maintain or<br />
improve the overall patient care; and<br />
‘‘(III) the ability to maintain or<br />
improve the quality <strong>of</strong> care provided<br />
to individuals enrolled under this part<br />
who participate under such mode.<br />
‘‘(vi) The model provides for a payment<br />
arrangement—<br />
