Meaningful Use / Quality Measures

Population Health Is At The Heart Of The Emerging Health Delivery System: While
individualized care will always be important, population health management combined with individual care is the
care delivery model of the future.
New Payment models will drastically affect future reimbursement: New health care models
include capitated and partially capitated care, value based payment, and shared risk models. Defining your patient
populations’ current and future risk of disease, and understanding what population based interventions are will
most likely be effective in managing your population.
Measurement is Here To Stay: Defining your population quality requires active management of your
population. HIE assures that you are in control of your data. Use better data to update and manage
1. Quality dashboards and Quality reporting
3. Reconcile regulatory, accreditation and third party rating and evaluation systems
4. Getting credit for accurate levels of reimbursement
Participate in New Care Opportunities: Opportunities to reduce costs by reducing preventable ER visits,
admissions, readmissions, low-value, high cost procedures and medicine usage, redundant tests, and eliminate waste
and fraud have created unprecedented opportunities for the physician to take better care of patients, improve
professional satisfaction and improve personal reimbursement. Keeping abreast of these opportunities requires a
robust HIE platform to assure your population data is current, relevant and capable of identifying these opportunities.
Create a Sustainable Delivery Model: 40% of all health care may be due to wasteful activities. Such
waste offers significant opportunity for improved health management practices with benefits accruing to patients,
physicians and society alike. HIE forms a foundation for intelligence sharing, collection reporting and analysis.
Our Process
A Defined Implementation Process
Assessment & Kick Off
The first phase of our
implementation process is
designed to assess system
specifics and establish working
parameters for the team and
project.
Connections
The details of your system’s
specific needs are reviewed and
a project scope with a series of
meeting to establish project
parameters.
Testing
Functionality, data transmission,
connectivity and sample
messages are part of the testing
process to ensure your system
is integrated with SMRTNET.
Testimonials & Use Cases
Family Doctor- Care Coordination Hazardous Medications
• Story: A 76 year old female with new onset afib presents to clinic one day post hospitalization, is unaware of medications
prescribed and isn’t sure of diagnosis.
• Outcome: SMRTNET contained OHH data on d/c medications, labs, studies and diagnosis. High risks medications prescribed
and monitoring started appropriately.
• Lesson Learned: Follow up with primary care post hospitalization facilitated and referring physician satisfaction improved, time
consuming record request avoided and potential medication reaction avoided.
Testimonials & Use Cases
“The system delivers this information to me when I need it, in the way I need to see it, to provide the best care
possible for my patients. The days of waiting weeks for information or spending valuable time on phone calls,
digging through faxes and portals for the data (and often never finding it) are numbered. Now, a patient’s
updated medication and problem lists and laboratory results from a recent hospital stay or yesterday’s visit to
the specialist across town are one click away. The immediate value and impact to me and to the patient is
immeasurable.”
Dr. Brian Yeaman
“Being able to exchange data between different physicians provides the best continuity of care possible. From
a practice management standpoint this level of interoperability and the exchange of information are required
to meet Stage 2 of Meaningful Use and Stage 3 when implemented by CMS/ONC. SMRTNET is very intuitive
and straightforward to use. The information that I can access about my patients helps me be a better
healthcare provider and therefore take better care of my patients which is ultimately what we are all striving
to do.”
