Instructions:<br />
Your MUSIC USE REPORT is due upon request.<br />
The only acceptable file formats for your reports are "tab or pipe delimited .txt", "tab or<br />
pipe delimited “.csv" and ".xls". <strong>Reports</strong> submitted in any other file formats will be<br />
returned to you, and you will be asked to resubmit your reports in an acceptable format.<br />
The following formats are for your reference only; they are not the actual forms which<br />
you complete and return to us:<br />
FORM 1 – Supply data according to this format if your service provides audio only or<br />
music video.<br />
FORM 2 - Supply data according to this format if your service provides audio-visual<br />
programming such as online or mobile television, film, etc.<br />

to <strong>ASCAP</strong>NewMedia@ascap.com requesting your personal <strong>ASCAP</strong> FTP account.<br />
