Information and Descriptive Material Bidder must furnish all information requested. In addition, in the space provided on the bid, vendors shall supply at least one (1) complete set of drawings, descriptive literature and complete specifications covering the products offered. A complete set of specifications detailing the construction materials, methods, etc. for this specific ambulance must be included in this bid. This specification must be in the order of this bid document. Bids not meeting this requirement will be rejected. Does your bid comply? Yes __X__ No_____ The Purchaser recognizes Ford Motor Company’s Qualified Vehicle Modifiers (QVM) accreditation. Therefore, regardless of chassis specifications the primary manufacturer must include with this proposal their current QVM certification. The QVM certificate supplied with this proposal must be dated within eighteen (18) months of the bid due date. Because of the quality control requirements of the Ford QVM certification this applies whether the chassis is a Ford or if it is from another manufacturer. The primary manufacturer shall also submit proof that it is a ‘member in good standing’ with the Ambulance Manufacturers Division (AMD) of the National Truck Equipment Association (NTEA). To assure the Purchaser is receiving such value the primary manufacturer must submit evidence of compliance with KKK-A-1822F testing. The testing is to be performed by an independent testing facility and verified by person(s) with the standing of Professional Engineer (P.Eng.). Does your bid comply? All bid prices and conditions must be specified on the Bid Proposal Form. Bid prices shall be valid for at least 60 days from the date of the Bid Opening or as otherwise specified on the Bid Proposal form. Price shall include pre delivery service, including oil and filter if mileage exceeds 1500 miles, and delivery to Mountain Home, Idaho. Full payment will be made when the unit is received, inspected and found to comply with procurement specifications, free of damage and properly invoiced. Does your bid comply? Yes __X__ No_____ Warranty and Service Requirements The successful bidder shall provide manufacturer’s warranty coverage for the ambulance conversion. warranties claiming “Lifetime” coverage shall specify that coverage shall at a minimum meet or exceed these specific minimums: A. A Limited Warranty on the ambulance conversion: Forty-eight (48) months or 48,000 miles from date of original purchase. B. Paint: Seven (7) years from date of original purchase C. Electrical: Six (6) years or 72,000 miles from date of original purchase. D. Modular Body Structural Integrity: Fifteen (15) years from date of original purchase. E. Chassis: warranty on the chassis portion of the completed ambulance shall be the responsibility of the chassis manufacturer. F.
