In case the Tenderer is an artificial personality; tenderer shall submit Trade Register Gazette
showing the partners, members or founders and the administrative personnel of the artificial
personality according to its reference and in case the necessary information does not show up in
this gazette, he shall submit related gazettes or other necessary documents confirming this issue
and notarized Authorized Signatory List.
6.2.4. Bu şartnamenin 9 uncu maddesinin 9.1. ve 9.2. nci bentlerinde belirtilen durumlarda
olmadıklarına dair;
Proving that the Tenderer doesnt suffer from the problems underlined in Article 9.1 and Article 9.2.,
6.2.4.1. İstekli, bu şartnamenin 9.1 ve 9.2 maddelerinde belirtilen durumlarda olmadıklarına dair,
ihale tarihinden önceki üç ay içinde tacirler için ticaret sicil memurlukları, esnaf ve sanatkarlar için
esnaf ve sanatkar sicil müdürlüğü/memurluğu, serbest meslek sahipleri içinse üyesi olduğu oda
tarafından düzenlenmiş, belgenin aslı veya noter tasdikli suretini verecektir.
Tenderers shall submit an original or notarized copy of the certificate determining that they are not
in situations specified in paragraphs 9.1 and 9.2 of this specification, received 3 ( three) months
before the tender date from trade directorate for merchants, from Directorate of Tradesmen and
Craftsmen for tradesmen and craftsmen and from Chamber for the self-employed persons where
they are registered to.
6.2.4.2. İstekli, bu belgelerin düzenlenme tarihinden sonra değişikliklerin olması halinde durumun
İdareye bildirileceğine ilişkin yazılı taahhütname (LAHİKA-1) verecektir.
Tenderer shall submit written declaration (APPENDIX-1) regarding that Contracting Entity will be
informed about this situation if there are any changes after these documents were designed.
6.2.5. Şekli ve içeriği bu şartnamede belirlenen teklif fiyatını gösteren Proforma Fatura (LAHİKA-2)
The Proforma Invoice indicating the bid price, the form and content of which is provided

6.2.7. İstekli adına vekaleten iştirak ediliyorsa İsteklinin adına teklif vermeye yetkili olduğuna dair
vekaletname ile imza beyannamesi (her ikiside noter tasdikli olarak) verilecektir.
If the tender is participated in proxy on behalf of the tenderer; notarized letter of proxy and
notarized certificate of signature for bidding shall be provided..
6.2.8. İsteklinin ortak girişim olması halinde şekli ve içeriği bu şartnamede belirlenen İş Ortaklığı
Beyannamesi (LAHİKA-3),
In case the Bidder is a joint venture (If two separate companies constitute a partnership specifically
for this contract), a certificate of Business Partnership Declaration, the form and the content of
which are defined hereby (APPENDIX-3),
İsteklinin ortak girişim olması halinde, her bir ortakça 6.2.2., 6.2.3., 6.2.4. ve 6.2.14’de yer
alan belgelerin ayrı ayrı verilmesi zorunludur.
In case the Bidder is a joint venture, the documents stated in Article 6.2.2., 6.2.3., 6.2.4. and
6.2.14. shall be presented one by one.
6.2.9. İstekliler tekliflerinde, şahıs şirketi ise şirket ortaklarını, sermaye şirketi ise sermayesinin
yarısından fazlasına sahip olan ortaklarını LAHİKA-4`de örneği bulunan forma uygun olarak beyan
edecektir. Ayrıca, başka bir şahıs şirketinde ortak olmaları durumunda bu şirketi, sermaye
şirketinde ortak olmaları durumunda ise sermaye şirketinin yarısından fazlasına sahip olmaları
durumunu beyan edecektir.
The Bidder shall specify company partners according to the attached document at APPENDIX-4 if
the company is an individual company and if a stock corporation, the name of the shareholder
possessing more than half of the company’s capital. Furthermore, if the Bidder serves a partner in
another company, he shall note the name of the company and if he possesses more than half of
the capital of this, he shall underline that.
6.2.10. Gizlilik ve Güvenlik/ Confidentiality and Security:
6.2.10.1. YÜKLENİCİ, malzeme ile bağlantılı olarak elde ettiği her tür bilgi ve dokümanı özel ve
gizli tutacaktır. İmzalanacak olan sözleşmenin yerine getirilmesi amaçları dışında, İDARE’nin
HK12-A7-041 13
