Trade Register Gazette showing the partners, members or founders and the administrative
personnel of the artificial personality according to its reference. In case the necessary information
does not show up in one gazette, tenderer can submit related gazettes of other necessary
documents (if the tenderer is foreigner, he shall submit document/s equivalent to this according to
his country’s legislation) confirming this issue.
6.1.4. Bu şartnamenin 9 uncu maddesinin 9.1. ve 9.2. nci bentlerinde belirtilen durumlarda
olmadıklarına dair;
Proving that the tenderer doesn’t suffer from the problems underlined in Article 9.1 and Article 9.2.,
6.1.4.1. Bu şartnamenin 9.1 ve 9.2 maddelerinde belirtilen durumlarda olmadıklarına dair, ihale
tarihinden önceki 3 (üç) ay içinde düzenlenmiş, isteklinin uyruğunda bulunduğu veya ticari
faaliyetini sürdürdüğü şirket merkezinin kayıtlı olduğu ülke mevzuatı uyarınca bu ülkelerin ilgili
makamından alacakları, Apostille kaşeli veya ilgili ülkenin Türk Konsolosluğunca veya Türk
Dışişleri Bakanlığınca onaylanmış belgeyi verecektir.
Tenderers shall submit a document with apostille stamp or approved by the Turkish Consulate of
the related country or by Turkish Ministry of Foreign Affairs determining that they are not in
situations specified in paragraphs 9.1 and 9.2 of this specification, received 3 ( three) months
before the tender date, which shall be convenient to legislations of country where the headquarters
of company operates or tenderer has citizenship is located.
6.1.4.2. İsteklinin uyruğunda bulunduğu veya şirket merkezinin kayıtlı olduğu ülke mevzuatına
göre, söz konusu belgelerin denginin bulunamaması veya düzenlenememesi halinde; bu duruma
ilişkin yazılı beyanlarını vereceklerdir. Bu husus yabancı gerçek kişi isteklinin uyruğunda
bulunduğu veya yabancı tüzel kişi isteklinin şirket merkezinin bulunduğu ülkenin Türkiye’deki
Misyon Şefliklerine veya bu ülkelerdeki T.C.Misyon Şefliklerine teyit ettirilecektir.
If the equivalent of the document cannot be found or arranged in accordance with the legislations
of the country where the tenderer has citizenship or the headquarters of the company is registered,
they shall submit written declarations regarding this situation. A confirmation document from Chief
of Mission Department of relevant country, where the tenderer has citizenship or headquarters of
company is registered, in Turkey or Chief of Mission Department of Turkey in relevant country
shall be submitted.
6.1.4.3. İstekli, bu belgelerin düzenlenme tarihinden sonra değişikliklerin olması halinde durumun
İdareye bildirileceğine ilişkin yazılı taahhütname (LAHİKA-1) verecektir.
Tenderer shall submit written declaration (APPENDIX-1) regarding that Contracting Entity will be
informed about this situation if there are any changes after these documents were designed.
6.1.5. Şekli ve içeriği bu şartnamede belirlenen teklif fiyatını gösteren Proforma Fatura (LAHİKA-2)

6.1.7. İstekli adına vekaleten iştirak ediliyorsa isteklinin adına teklif vermeye yetkili olduğuna dair
vekaletname ile imza beyannamesi (Her iki belgede, Apostille Kaşeli veya ilgili ülkenin Türk
Konsolosluğunca veya Türk Dışişleri Bakanlığınca onaylanmış) verilecektir.
If the tender is participated in proxy on behalf of the tenderer; letter of proxy and certificate of
signature for bidding on behalf of the tenderer (each certificate shall be with Apostille cachet or
confirmed by Consulate of the Republic of Turkey in the related country or Turkish Ministry of
Foreign Affairs).
6.1.8. İsteklinin ortak girişim olması halinde şekli ve içeriği bu şartnamede belirlenen İş Ortaklığı
Beyannamesi (LAHİKA-3),
In case the Bidder is a joint venture (If two separate companies constitute a partnership specifically
for this contract), a certificate of Business Partnership Declaration, the form and the content of
which are defined hereby (APPENDIX-3),
İsteklinin ortak girişim olması halinde, her bir ortakça 6.1.2., 6.1.3., 6.1.4. ve 6.1.15’de yer
alan belgelerin ayrı ayrı verilmesi zorunludur.
