ODOMETER REPAIR OR REPLACEMENT CERTIFICATION<br />

INSTRUCTIONS:<br />

● Use this form to comply with odometer certification requirements when an odometer on any vehicle is serviced,<br />

repaired or replaced and the odometer reading changes.<br />

● If you service, repair or replace an odometer and you change the odometer reading, you must file this certification<br />

with DMV within 10 days of the date the work is completed. (See "EXEMPT VEHICLES AND PENALTIES," below.)<br />

● This form must be completed by the person who did the work on the odometer. It must be completed in full, signed<br />

and dated.<br />

The notice was placed on the vehicle in accordance with ORS 815.415 and DMV Administrative Rules. I understand that it is a crime under<br />

ORS 162.085 to certify the truth of a statement when I know the statement is not true. Such a crime is punishable by a jail sentence of up to<br />

six months, a fine of up to $1,000, or both.<br />

EXEMPT VEHICLES AND PENALTIES<br />

1) Oregon Revised Statute (ORS) 815.415 and DMV Administrative Rules state that a person commits the offense of unlawful repair of an<br />

odometer if the person services, repairs or replaces the odometer and doesn't comply with the following:<br />

a) Whenever possible, the person shall perform work on the odometer without changing the mileage reading.<br />

b) If it is not possible to perform the work without changing the mileage reading, the person must do all of the following:<br />

A) Adjust the odometer reading to zero.<br />

B) Place a permanently affixed notice on the left door frame of the vehicle that includes the following (if the vehicle does not<br />

have a door frame, we suggest that the notice be placed as near as possible to the vehicle identification number):<br />

1) Information identifying the form as an odometer repair notice.<br />

2) The name and address of the individual, agency or business that performed the work.<br />

3) The signature of the person who performed the work.<br />

4) The reading prior to the repair, replacement or service.<br />

5) The date the work was performed.<br />

6) Information stating the removal of the notice is a Class C misdemeanor.<br />

C) Submit an Odometer Repair or Replacement Certification, Form 6747, to DMV within 10 days of completing the work.<br />

2) The owner or any subsequent purchaser of a vehicle may bring an action in an appropriate court of this state against any person who<br />

violates ORS 815.415, and may recover from the person an amount of $500 or twice the actual damages caused by the violation, whichever<br />

is greater, plus costs and reasonable attorney fees. Only a single recovery is permitted under this subsection for any single violation of ORS<br />

815.415.<br />

3) A person is not subject to the requirements for work performed on vehicles that are exempt from odometer disclosure requirements.<br />

These vehicles are: ● A vehicle that is not a motor vehicle. ● A motor vehicle that is not equipped with an odometer at the time of<br />

manufacture. ● A motor vehicle 10 years old or older. ● A new motor vehicle prior to its first retail sale. ● A motor vehicle with a<br />

manufacturer's gross vehicle weight rating in excess of 16,000 pounds. ● Snowmobiles, Class 1 and Class III all-terrain vehicles.<br />
