Replacement Title Procedures
DEPARTMENT OF TRANSPORTATION

To apply for a replacement title with NO CHANGE in ownership, we must receive:
1. A completed and signed Application for Replacement Title (Form 735-515). See #1 of instructions.
2. Title fees made payable to Oregon DMV. See #2 of instructions.
INSTRUCTIONS
1. Complete the Application for Replacement Title (Form 735-515) with the following:
Line 1 & 2. Fill in the complete vehicle description.
Line 3. Complete the registered owner or lessee name, date of birth, and customer number (Oregon driver
license or Oregon ID card number) of the individual or business that is listed as owner on Oregon DMV’s
records.
Line 4-5. List the address(es) for the owner shown on Line 3.
Residence Address: If the owner listed is an individual, they must provide their true residence address (where
the owner primarily resides). If the owner listed is a business, the business address must be provided. If the
owner listed is a lessee, the lessee’s address must be shown.
Mailing Address: Complete the mailing address if you receive your mail at an address other than the
residence address.
Line 6-7. List any additional registered owners or lessees.
Line 8-9. List the mailing address for this transaction only, if different than address on Lines 4-5. Also list the
location of the vehicle or manufactured structure if different than residence address.
Line 10-15. If you owe money on the vehicle, or the vehicle is leased, complete the security interest holder
and/or lessor name and address. If the security interest holder/lessor listed is an individual, write their date of
birth and customer number in the space provided. They must also provide their true residence address (where
the owner primarily resides). If the security interest holder/lessor listed is a business, and you know the
customer number DMV assigned to them, write it in the space provided. The business address must also be
provided. If either party has a mailing address, other than the residence address, it must be provided also.
Line 16 & 17. If there is no security interest holder/lessor on DMV’s records, all registered owners must sign.
If there is a security interest holder/lessor, they must sign also.
Line 18. Signature of person who knows the disposition of the title if different than the title holders.
