Bidders may submit manually filed tax returns or tax returns filed through<br />
the Electronic Filing and Payments System (EFPS).<br />
The latest tax returns refer to the income and/or business taxes filed within<br />
one (1) year preceding the date of the Opening of Bids.<br />

ADDITIONAL REQUIREMENTS:<br />
Bidders shall submit an UNDERTAKING (BDS Form 3) or equivalent<br />
document as part of the eligibility and technical requirements, containing<br />
the following:<br />
1. That freight/forwarding costs shall be borne by the bidder; and<br />
2. That training on the proper use, care, and maintenance to the<br />
PNP personnel shall be conducted by bidder;<br />
The equivalent document aforementioned shall be subject to final<br />
determination as to their admissibility by the BAC upon recommendation<br />
of the TWG during the post-qualification.<br />

34.2 The effective date of the Contract is upon receipt of the Notice to Proceed<br />
by the Lowest Calculated and Responsive Bidder. The contract shall be<br />
duly notarized by any commissioned notary public and the notarial fee shall<br />
be paid by the winning bidder.<br />
35 Warranty Security: Upon final acceptance of the Project by the Committee<br />
on Inspection and Acceptance (CIA), the supplier shall post a special bank<br />
guarantee equivalent to at least ten percent (10%) of the contract price as<br />
warranty security pursuant to Section 62.1 of the Revised IRR of RA No.<br />
9184.<br />
