copy of the child’s birth certificate, birth registration<br />

card, passport, or report card.<br />

• A completed copy of the Commonwealth of<br />

Virginia School Entrance Health Form.<br />

• A $45.00 non-refundable registration fee per<br />

family per program.<br />

There are no exceptions! We must have all of<br />

the above items to register your child. These items<br />

are needed to comply with Virginia state licensing<br />

YOUTH AFTER SCHOOL<br />

(YAS) PROGRAM<br />

(Grades 6-8) YAS offers fun, exciting, recreational<br />

activities for middle school students including<br />

sports, games, arts & crafts, field trips, drama,<br />

tutoring, mentoring, special events and intramural<br />

competitions, as well as time for homework, relaxing,<br />

and socializing in a safe supervised environment.<br />

Students have the opportunity to help plan<br />

activities and are encouraged to explore their<br />

interests. YAS is operated in middle schools and<br />

follows the <strong>Loudoun</strong> <strong>County</strong> Public School calendar.<br />

The program begins when the final bell rings and<br />

ends at 6:15 p.m.<br />

YOUTH ADVISORY COUNCIL<br />

Teens have the opportunity to develop leadership<br />

skills, meet new people and be a voice for other<br />

<strong>Loudoun</strong> teens on the Youth Advisory Council.<br />

Members plan special events, including YouthFest<br />

<strong>2012</strong>, work on community service projects and<br />

promote events for teens. Applications are<br />
