PAYING FEES FOR STUDENT ENROLLMENT<br />
• All students who are registering online must pay required fees with a credit card.<br />
• In order to sign in for fee payment, password includes: student's ID, student's<br />
birthdate and telephone number.<br />
• If you are paying by check or cash or for those who are applying for fee waiver or<br />
fee deferment, you must register AT <strong>Hersey</strong> on one of the following days:<br />

Fee Waiver: By appointment only on Tuesday & Thursdays<br />
• If you do not have a computer at home, you may also come to <strong>Hersey</strong> on those<br />
days to use a school computer to register on one of the above times.<br />
• IF YOU DO NOT REGISTER BY JULY 31, A PROCESSING FEE OF $25.00 WILL<br />
BE ADDED TO THE REQUIRED REGISTRATION FEES.<br />
• If you have Driver's Ed. on you schedule and are not taking it in January, you<br />
must call a counselor to remove it from your schedule before you can register<br />
online.<br />

• Sophomores, juniors and senior students should come to <strong>Hersey</strong> on the following<br />
dates to pick up their packet which includes: ID, Assignment Notebook, locker<br />

FRESHMAN PHYSICALS<br />
• Per the State of Illinois, freshmen must have a current physical (dated within the<br />
last year from August, 2012) to enroll in high school. The physical and updated<br />
shot records must be mailed or dropped off to the <strong>Hersey</strong> Main Office before<br />
August 9 or your student may not attend school.<br />
• If the physical appointment is after July 31, you should still register/pay fees<br />
ONLINE BEFORE JULY 31 and mail or bring in your physical form immediately<br />
following the doctor's appointment before August 21.<br />

Helpful Information For Your Student<br />
SCHOOL DAY: Classes begin at 7:30 a.m., end at 2:50 p.m. Activity period: 2:55 - 3:20 p.m.<br />
• L.A.T.E. ARRIVAL DAYS: L.A.T.E. Days for <strong>John</strong> <strong>Hersey</strong> <strong>High</strong> <strong>School</strong> will be every Thursday with<br />
student arrival time at 8:30 a.m. (Bus pick-up 6:30 a.m.). The exceptions will be the following dates:<br />
9/12, 10/10, 3/6, 4/10, 5/15 with school start time at 9:30 a.m.(Bus pick-up 8:30 a.m.).<br />
• ELECTRONIC DEVICES PROCEDURES: Students may not use or possess electronic signaling<br />
(paging) devices or two-way radios on school property at any time, unless the building principal<br />
specifically grants permission.<br />
• CELL PHONES: The possession and use of cell phones and other electronic devices, other than<br />
paging devices and two-way radios, are subject to the following protocols:<br />
1. They may not be used in any manner that will cause disruption to the educational environment<br />
or will otherwise violate student conduct rules.<br />
2. They must remain in the “off” position and stored in a locker, backpack, purse, pocket, or<br />
other place where they are not visible during a regularly scheduled school day. A “regularly<br />
scheduled school day” is defined as the moment a student arrives on school grounds when a<br />
day school is in session, until the last class has ended for all students.<br />
3. A specific area in the school is designated where cell phones and electronic devices may be<br />
used, other than paging devices and two-way radios, during a regularly scheduled school day.<br />
• CLOTHING POLICY: No hats or heavy coats may be worn in classes or hallways. T-shirts with<br />
reference to alcohol, tobacco, sex, drugs, profanity, or obscene language are not permitted. Students<br />
shall not wear halter tops, garments with spaghetti straps, or strapless garments. Garments that are<br />
“see-through,” cut low, or expose one’s midriff are not acceptable. Skirts and shorts should not be<br />
shorter than mid-thigh. Undergarments must not be visible. All pants must fit around the waist<br />
and must be properly fastened.<br />
• BUSES: Buses are free for students living more than 1 1/2 mile from school or other designated areas.<br />
Students are required to show their I.D. cards to the bus driver starting the second week of school. There<br />
are two after school buses for students: Activity Bus at 3:50 p.m. Athletic Bus, Mon.-Thurs. at 5:45 and<br />
Friday at 5:15 p.m.<br />
• STUDENT CLASS SCHEDULES: Packets will be available for pick-up at <strong>Hersey</strong> <strong>High</strong> <strong>School</strong> on July 23<br />
& 24, 8-12 noon and July 25, 1-5 p.m. Thereafter, Mondays and Wednesdays for packet pick-up from 7:30<br />
a.m. to 3:30 p.m.. Packets include your ID, class schedule, bus route and locker combo. Freshmen will<br />
receive packets with schedules on Freshmen Orientation Day, August 15.<br />
• LUNCH - Students who wish to put money on deposit for buying food in the cafeteria must fill out the<br />
designated form found in the <strong>Hersey</strong> H.S. Main Office, or online, with a check made payable to: <strong>John</strong><br />
<strong>Hersey</strong> <strong>High</strong> <strong>School</strong>.<br />
• PHYSICAL EDUCATION SHIRTS: P.E. T-Shirts and P.E. locks will be distributed during the first week of<br />
school in P.E. class. Students are permitted to wear their own shorts in P.E. class. Additional shirts may be<br />
purchased from the gym teacher.<br />
• ATTENDANCE OFFICE telephone number is: 847-718-4801. A parent must call the Attendance Office<br />
any time a student is absent from any class. Please call on the day of the absence. Students must sign out<br />
when leaving the building during the school day for an appointment.<br />
• SCHOOL SUPPLIES: At <strong>Hersey</strong> <strong>High</strong> <strong>School</strong> we do not issue a list of school supplies. Each instructor will<br />
tell the students what is required. The first day students should bring a pen, pencil, and some paper to<br />
write on. <strong>Hersey</strong> does not have a school store.<br />

<strong>John</strong> <strong>Hersey</strong> <strong>High</strong> <strong>School</strong> will be offering the “Huskie<br />
Kickoff” as a freshmen transition experience and a great<br />
opportunity for all incoming freshmen. We believe that all<br />
freshmen participants will have a head start as they enter<br />
<strong>Hersey</strong> <strong>High</strong> <strong>School</strong>. This day will be informative and fun.<br />
Students will receive their schedule, locker number and<br />
combination, assignment notebook along with a <strong>Hersey</strong><br />
T-Shirt and other items. All freshmen should attend our<br />
“Huskie Kick-Off 2013.” No other orientation program will<br />
be available. Bus service will run at 6:45 and 11:30 a.m.<br />
PLEASE SAVE THE DATE AND PLAN TO ATTEND.<br />

<strong>Hersey</strong> Cell Phone Rules<br />
1. Cell phones are to be turned OFF, not seen or heard by staff from the time students walk into<br />
<strong>Hersey</strong> <strong>High</strong> <strong>School</strong> until 2:50 PM.<br />
2. The exception to this rule is during a Designated Time AND in a Designated Cell Phone Area.<br />
3. The Designated Times for cell phone usage are:<br />
a. BEFORE 7:25 AM<br />
b. On Orange Late Start days until 8:15 AM<br />
c. During each student’s assigned lunch period<br />
4. The Designated Cell Phone Areas are in the cafeteria and the commons area (where the vending<br />
machines and circle tables are located). The commons area does NOT include any area beyond the<br />
steps entering the commons hallway.<br />
****Cell phones are never allowed to be used, seen or heard from the time a student walks into the<br />
building until 2:50 PM in classrooms, hallways, restrooms, gymnasiums, or locker rooms.***<br />
Substance Abuse Policy<br />
Students who violate the District 214 Substance Abuse Policy may lose privileges including: open<br />
lunch, open study, enrollment in the behind the wheel component of Drivers’ Education, classes where<br />
heavy machinery is operated, campus parking, and/or overnight field trips and excursions.<br />
Reclassified Students loose the privilege of an Open Lunch<br />
Beginning with the 2012/2013 school year, students who are reclassified due to a lack of credits will<br />
lose their open lunch privileges. This will be enforced regardless of the student’s year in school. In<br />
order for a student to regain open lunch privileges, s/he must demonstrate efforts to remediate their<br />
academic situations, shown by passing all classes the following semester without major disciplinary<br />
incidents. If the student does not continue to pass all classes, or has a major disciplinary incident, the<br />
lunch will again be closed.<br />

Fall Sports Sign-Up Information<br />
2013-14<br />
Fall sports begin practice in mid-August. Before being allowed to practice/tryout, all athletes<br />
must register in person at the Athletic Office late in July. The athletic forms will be online.<br />
You must download the forms and bring them to the athletic registration properly filled out. If<br />
you can’t download the forms, they are available in the athletic office and/or the main office. All<br />
school fees must be paid prior to sign-up. Upon making a team, District 214 requires a $65<br />
participation fee to be paid before a student will be allowed to compete in any contest. The<br />
following information is essential to register.<br />
1. A current physical, signed and dated by a physician. Health history on the back page of<br />
the physical must also be filled out and signed by the parent or guardian. Physicals are<br />
good for one calendar year. Freshmen athletes must turn in a copy of their physical to the<br />
Athletic office unless they have sent one in with their fee payment materials.<br />
2. A signed and dated Athletic Permit form (must be signed by the parent and athlete).<br />
3. A concussion form (must be read and signed by the parent and athlete).<br />
4. An emergency card (fully filled out and signed by the parent and athlete).<br />
5. The <strong>John</strong> <strong>Hersey</strong> athletic card (top portion) needs to be completed.<br />

You must have a COMPLETE PACKAGE (all items mentioned above) in order to sign-up. All<br />
students are welcome and encouraged to participate in our athletic program. REMINDER - YOU<br />
MUST SIGN-UP DURING THE ABOVE DATES TO PRACTICE OR TRYOUT FOR THE FALL<br />
SEASON. We will also accept registration for those athletes who are participating in the winter<br />
and spring seasons or you may register before the winter and spring seasons begin. All athletes<br />
and a parent will be required to attend a meeting on expectations with their coaches before their<br />
first contest. Dates will be announced.<br />
Football players will be issued their equipment and have team meetings on Tuesday, August 9th<br />
in the field house. See below for more details.<br />

Chronic Health Conditions:<br />
<strong>School</strong> achievement is dependent upon effectively meeting the health needs of all the students enrolled in District 214 schools. If your<br />
student has a chronic health condition such as Asthma, Allergies, Diabetes, Seizures or Autism, it is important for the school, doctor,<br />
and family to work together to provide the best educational experience possible.<br />
In an effort to provide your student an opportunity for a successful school year, a number of health care plans are available on the<br />
www.d214.org Web site. The care plan is designed to help the school collaborate with the student’s family and medical doctor to<br />

IMMUNIZATION REQUIREMNET IN SCHOOLS<br />
Numerous outbreaks of Pertussis (whopping cough) have occurred recently among school children in<br />
Illinois. Pertussis is easily transmitted through coughing and sneezing and may cause an illness that persists<br />
for weeks to months. Petussis does not typically cause severe illness in healthy students, but can cause<br />
prolonged absences from school and extracurricular activities. In addition, Pertussis can be transmitted from<br />
healthy students to infants and individuals with chronic illnesses, for whom Pertussis can be life-threatening.<br />
Protection against Pertussis begins to wear off during grade school. This leaves<br />
pre-teens, teenagers and adults at risk for this illness.<br />
The Illinois Department of Public Health now requires that ALL students entering grades seven through<br />
twelve who have not already received a Tdap dose, regardless of the interval since the last DTap, DT, or Td<br />
dose, must receive a Tdap dose. The cases of Pertussis in Illinois have increased in 2011, which has caused<br />
these new mandates by the Illinois Department of Public Health.<br />
What does this mean to you and your student? Many students may have already received the vaccine but it<br />
has not been specified on their immunization form. Please ask your doctor f your student has received the<br />
immunization and have them provide the date it was given and a doctor’s signature. If they have not received<br />
the immunization, request it be given and provide proof of the immunization to your high school. Students<br />
without proof of Tdap vaccination or an approved medical or religious exemption on file with the school, will<br />
be subject to exclusion. All students will be required to show proof of immunizations prior to receiving their<br />
class schedules in August.<br />
New Certificate of Child Health Examination Form (Physical Exam Form)<br />
The Illinois Department of Public Health has issued a new updated form to be used for physical exams and<br />
sports physicals. Please visit the district website and click on district information, health services, forms,<br />
Certificate of Child Health Examination and print the form and give to your doctor to use for physical exams<br />
effective immediately.<br />

Class of 2014: Top 5 Things To Do This Summer<br />
To Get Ready For College Applications<br />
The summer is a great time for rising seniors to get a jumpstart on college planning and<br />
applications. Getting started now will take some of the stress out of the fall college application season,<br />
when students are also juggling classes and extracurricular commitments. Please encourage your son or<br />
daughter to:<br />
1. Finalize a college application list<br />
If your student is just beginning the college search process, there is no better way to decide what they want in<br />
a college than to step on a campus. With over 100 colleges within a one-hour drive from Arlington Heights, it<br />
is easy for them to make a visit or two, to help them decide what they are looking for in terms of size, setting,<br />
programs, and student traits. Once your student knows what traits are important, they can use Naviance and<br />
college websites to find other schools of interest and pull together a list. We ask all rising seniors to return to<br />
school in the August with a list of colleges.<br />
2. Complete a “Counselor Rec Survey”<br />
We have asked all rising seniors to complete a “Counselor Rec Survey” in Naviance before school begins in<br />
August. Answering questions and providing examples in this survey will help your student’s counselor to write<br />
a great letter of recommendation for college and scholarship applications. Students will find it on the “About<br />
Me” tab in the left-hand column.<br />
3. Prepare an activities resume<br />
Many college applications ask for a resume or list of jobs, extracurricular activities, community service, awards<br />
and honors. This resume may also come in handy for campus interviews, scholarship applications, and letters<br />
of recommendation. Naviance has a resume-writer, which is very easy to use.<br />
4. Review ACT test scores<br />
By now students should have received ACT test scores from the April Prairie State Achievement Exam (PSAE).<br />
Students should look at their scores and the admission requirements for the colleges on their list. Students<br />
should retest in September, if they want to retake the ACT for a higher score or if any of their schools require<br />
the optional ACT writing test, which was not a part of the PSAE. Be sure to sign up for this exam BEFORE<br />
August 23 to avoid a late fee. Also, remember to have your son or daughter save their ACT score reports for<br />
use when applying for scholarships next year.<br />
5. Begin college applications<br />
Most applications are available on college websites in the mid- to late summer. This is a great time to fill out<br />
demographic information or work on essays. Whatever they complete this summer, is work they won’t have to<br />
do once school and extracurricular activities begin. And if they are struggling to begin the essays, have them<br />

National Geography Challenge<br />
Every year we enter our AP students in what<br />
is called the National Geography Challenge,<br />
which is a 50 question multiple choice<br />
exam given to students nationally based<br />
on geographic concepts. This year <strong>Hersey</strong><br />
was not only the winner from the 9th grade<br />
division, but also overall winners with a total<br />
of 474 points. Jack Steines and Nick Winter<br />
were the national student leaders this year<br />
with scores of 49/50!<br />

Orchesis<br />
<strong>Hersey</strong> Orcheses ran a fundraising production<br />
and donated to the Make-A-Wish Foundation<br />
from a concert that was held on May 2, 2013.<br />
<strong>Hersey</strong> Orchesis ran the entire dance<br />
production. Dancers from Dance 1/2/3<br />
(all classes) created and performed pieces<br />
based off previous wishes, granted by the<br />
foundation, prior to this concert. Dynamite<br />
Dancers (collaboration between Orchesis<br />
and the CLS program) performed a piece<br />
at the start of the second act. Dance Fusion<br />
(created a piece all together) and opened<br />
the performance with the idea of striving<br />
for something more and accomplishing<br />
something you believe in. We had two<br />
speakers, a mother and a daughter (who are<br />
part of Make A Wish) start off our concert that<br />
night informing the audience and students<br />
about Make A Wish as well as telling a<br />
personal story about surviving cancer.<br />
Students never had the opportunity to run<br />
this production prior to taking the stage, as a<br />
whole. Lighting and sound was performed<br />
for the first time as well. Many overcame<br />
their fear of performing while others enjoyed<br />
another time to share their talents with others<br />
on stage. Dance ability levels ranged from<br />
never taking a dance class to those who have<br />
taken dance for years.<br />
I couldn't tell you how proud I am of how<br />
much everyone accomplished that night<br />
together to make something special happen<br />
not only on stage but as a whole.<br />
Due to the collaborative efforts of students<br />
and staff, <strong>Hersey</strong> Dance Program will be<br />
donating $500.00 to the Make-A-Wish<br />
Foundation this year in hopes to grant further<br />
wishes for incredible children.<br />

Chicagoland <strong>High</strong> <strong>School</strong> Film Festival<br />
When <strong>John</strong> <strong>Hersey</strong> <strong>High</strong> <strong>School</strong> seniors<br />
Jordan Graves, Franklin Barker, and<br />
Manny Roman started piecing together<br />
a film last September as part of a final<br />
project for their Contemporary American<br />
Texts class, they had no idea where this<br />
assignment would take them. "Ruin," their<br />
short post-apocalyptic thriller, is one of<br />
only twelve student films to be accepted this<br />
year at the Chicagoland <strong>High</strong> <strong>School</strong> Film<br />
Festival on May 17, where the film will be<br />
screened for an audience. The Latin <strong>School</strong><br />
sponsors the festival.<br />

and Audience Choice and for Best Directing,<br />

<strong>John</strong> <strong>Hersey</strong> <strong>High</strong> <strong>School</strong>’s Theater<br />
Department took the stage with its spring<br />
production of the 2011 revival of the<br />
well-loved musical, Godspell. With an<br />
exuberant score that ranged from folk to<br />
rock and an interwoven series of storytelling,<br />

In 1999 the Illinois General Assembly passed laws requiring that public schools notify parents/<br />
guardians and school employees at least 48 hours prior to any pesticide applications on school<br />
property. The term “pesticide” includes insecticides, herbicides, rodenticides, and fungicides.<br />
The notification requirement extends to both indoor and outdoor pesticide applications. Excluded<br />
from the notification requirement are antimicrobial agents (such as disinfectants, sanitizers, or<br />
deodorizers), insecticide baits, and rodenticide baits.<br />
<strong>John</strong> <strong>Hersey</strong> <strong>High</strong> <strong>School</strong> has therefore established a registry of people who wish to be notified.<br />
To be included in this registry, please complete the form below and submit it to the Buildings and<br />
Grounds Supervisor at your school. Any other questions you may have regarding the District’s<br />
