Chronic Health Conditions:
School achievement is dependent upon effectively meeting the health needs of all the students enrolled in District 214 schools. If your
student has a chronic health condition such as Asthma, Allergies, Diabetes, Seizures or Autism, it is important for the school, doctor,
and family to work together to provide the best educational experience possible.
In an effort to provide your student an opportunity for a successful school year, a number of health care plans are available on the
www.d214.org Web site. The care plan is designed to help the school collaborate with the student’s family and medical doctor to
insure student needs are met. Please provide these documents prior to school starting on August 21, 2013.
Care plans can be found under the Parent Resources tab and sliding down to Health Info. Once you have printed the Care Plan,
please review it with your doctor and return it to your school’s Health Office prior to the beginning of each school year.
If your student requires medication or an inhaler during the school day, these forms are also available on the www.d214.org Web site.
IMMUNIZATION REQUIREMNET IN SCHOOLS
Numerous outbreaks of Pertussis (whopping cough) have occurred recently among school children in
Illinois. Pertussis is easily transmitted through coughing and sneezing and may cause an illness that persists
for weeks to months. Petussis does not typically cause severe illness in healthy students, but can cause
prolonged absences from school and extracurricular activities. In addition, Pertussis can be transmitted from
healthy students to infants and individuals with chronic illnesses, for whom Pertussis can be life-threatening.
Protection against Pertussis begins to wear off during grade school. This leaves
pre-teens, teenagers and adults at risk for this illness.
The Illinois Department of Public Health now requires that ALL students entering grades seven through
twelve who have not already received a Tdap dose, regardless of the interval since the last DTap, DT, or Td
dose, must receive a Tdap dose. The cases of Pertussis in Illinois have increased in 2011, which has caused
these new mandates by the Illinois Department of Public Health.
What does this mean to you and your student? Many students may have already received the vaccine but it
has not been specified on their immunization form. Please ask your doctor f your student has received the
immunization and have them provide the date it was given and a doctor’s signature. If they have not received
the immunization, request it be given and provide proof of the immunization to your high school. Students
without proof of Tdap vaccination or an approved medical or religious exemption on file with the school, will
be subject to exclusion. All students will be required to show proof of immunizations prior to receiving their
class schedules in August.
New Certificate of Child Health Examination Form (Physical Exam Form)
The Illinois Department of Public Health has issued a new updated form to be used for physical exams and
sports physicals. Please visit the district website and click on district information, health services, forms,
Certificate of Child Health Examination and print the form and give to your doctor to use for physical exams
effective immediately.
If you have questions or concerns, please contact the Hersey School Nurse,
Ashley Hood, RN at 847-718-4946 or Ashley.hood@d214.org
Class of 2014: Top 5 Things To Do This Summer
To Get Ready For College Applications
The summer is a great time for rising seniors to get a jumpstart on college planning and
applications. Getting started now will take some of the stress out of the fall college application season,
when students are also juggling classes and extracurricular commitments. Please encourage your son or
daughter to:
1. Finalize a college application list
If your student is just beginning the college search process, there is no better way to decide what they want in
a college than to step on a campus. With over 100 colleges within a one-hour drive from Arlington Heights, it
is easy for them to make a visit or two, to help them decide what they are looking for in terms of size, setting,
programs, and student traits. Once your student knows what traits are important, they can use Naviance and
college websites to find other schools of interest and pull together a list. We ask all rising seniors to return to
school in the August with a list of colleges.
2. Complete a “Counselor Rec Survey”
We have asked all rising seniors to complete a “Counselor Rec Survey” in Naviance before school begins in
August. Answering questions and providing examples in this survey will help your student’s counselor to write
a great letter of recommendation for college and scholarship applications. Students will find it on the “About
Me” tab in the left-hand column.
3. Prepare an activities resume
Many college applications ask for a resume or list of jobs, extracurricular activities, community service, awards
and honors. This resume may also come in handy for campus interviews, scholarship applications, and letters
of recommendation. Naviance has a resume-writer, which is very easy to use.
4. Review ACT test scores
By now students should have received ACT test scores from the April Prairie State Achievement Exam (PSAE).
Students should look at their scores and the admission requirements for the colleges on their list. Students
should retest in September, if they want to retake the ACT for a higher score or if any of their schools require
the optional ACT writing test, which was not a part of the PSAE. Be sure to sign up for this exam BEFORE
August 23 to avoid a late fee. Also, remember to have your son or daughter save their ACT score reports for
use when applying for scholarships next year.
5. Begin college applications
Most applications are available on college websites in the mid- to late summer. This is a great time to fill out
demographic information or work on essays. Whatever they complete this summer, is work they won’t have to
do once school and extracurricular activities begin. And if they are struggling to begin the essays, have them
check out some excellent tips on essay writing here: http://owl.english.purdue.edu/owl/resource/642/01/.
Calculators
All students are strongly recommended (not required) to have a graphing calculator this year. If
you already own a graphing calculator, there is no need to purchase a new one. If you have not
yet purchased one, then we can help. Because of our ability to purchase in large quantities, we
are able to get these at a discounted price. The calculator that is recommended is the TI-84
National Geography Challenge
Every year we enter our AP students in what
is called the National Geography Challenge,
which is a 50 question multiple choice
exam given to students nationally based
on geographic concepts. This year Hersey
was not only the winner from the 9th grade
division, but also overall winners with a total
of 474 points. Jack Steines and Nick Winter
were the national student leaders this year
with scores of 49/50!
Our 10 entries were (4 tied with 46 so there is
actually 11):
Nick Wagner
Nick Winter
Luther Tenorio
Sergey Smirnov
Jack Steines
Andrew Song
Katya Gozman
Daniel Gerardi
Andrew Stomper
Alex Beric
Jason Kloud
.
Orchesis
Hersey Orcheses ran a fundraising production
and donated to the Make-A-Wish Foundation
from a concert that was held on May 2, 2013.
Hersey Orchesis ran the entire dance
production. Dancers from Dance 1/2/3
(all classes) created and performed pieces
based off previous wishes, granted by the
foundation, prior to this concert. Dynamite
Dancers (collaboration between Orchesis
and the CLS program) performed a piece
at the start of the second act. Dance Fusion
(created a piece all together) and opened
the performance with the idea of striving
for something more and accomplishing
something you believe in. We had two
speakers, a mother and a daughter (who are
part of Make A Wish) start off our concert that
night informing the audience and students
about Make A Wish as well as telling a
personal story about surviving cancer.
Students never had the opportunity to run
this production prior to taking the stage, as a
whole. Lighting and sound was performed
for the first time as well. Many overcame
their fear of performing while others enjoyed
another time to share their talents with others
on stage. Dance ability levels ranged from
never taking a dance class to those who have
taken dance for years.
I couldn't tell you how proud I am of how
much everyone accomplished that night
together to make something special happen
not only on stage but as a whole.
Due to the collaborative efforts of students
and staff, Hersey Dance Program will be
donating $500.00 to the Make-A-Wish
Foundation this year in hopes to grant further
wishes for incredible children.
Dear Parent/Guardian and Staff Members:
In 1999 the Illinois General Assembly passed laws requiring that public schools notify parents/
guardians and school employees at least 48 hours prior to any pesticide applications on school
property. The term “pesticide” includes insecticides, herbicides, rodenticides, and fungicides.
The notification requirement extends to both indoor and outdoor pesticide applications. Excluded
from the notification requirement are antimicrobial agents (such as disinfectants, sanitizers, or
deodorizers), insecticide baits, and rodenticide baits.
John Hersey High School has therefore established a registry of people who wish to be notified.
To be included in this registry, please complete the form below and submit it to the Buildings and
Grounds Supervisor at your school. Any other questions you may have regarding the District’s
pest management practices may also be directed to Brian Lichtenberger at 847 / 718-7619
Please notify me, at least 48 hours in advance, when any pesticide applications will occur
on school property.
