• where appropriate, include a chart <strong>to</strong> show how the<br />

management structure fits <strong>to</strong>gether.<br />

11. Premises, equipment and other issues: Describe any other<br />

• the ownership and/or tenancy terms, lease or purchase cost, and<br />

suitability of premises<br />

• define all the groups of people who receive benefit from your<br />

activities, directly or indirectly (eg service users and clients,<br />

volunteers)<br />

• in what specific ways will these groups benefit?<br />

• how will you record and measure whether you are achieving<br />

these benefits?<br />

14. Moni<strong>to</strong>ring and evaluation:<br />

• how will the targets be set?<br />

• how will targets be moni<strong>to</strong>red?<br />

• how will progress over time be assessed?<br />

• do you have plans for review days or social accounting?<br />
