to choose a doctor (Primary Care Provider).
We can help you continue seeing your current
doctor if you have one. If you need to choose a
doctor, we can also help you with this. So give
us a call today to start using your benefi ts at:

free), or 1-877-600-5473 TTY.
If we do not hear from you by mail or phone in
30 days we will choose a doctor for you so you
can use your benefi ts.

below. If any changes are needed give us a
call. If this is a new card, please replace your
old card with this one.

How to read your Trillium ID card
Your care needs to be
arranged by your doctor
and approved by Trillium
unless you have an
emergency or a referral.
In an emergency call your
doctor, or 911 to get the
help you need.
Then call your doctor and
Trillium in the next 24
hours so you can get help
with follow up care.
