Community & Supportive Services (CSS) for Residents • Community & Supportive Services are a critical component of HOPE VI and essential to ensuring integration of residents into the greater community and promoting economic and social self- sufficiency of the residents in the revitalized community. • 60% of Residents (19-64 yrs. old) are employed • 20% of Mulford Gardens Residents are Seniors • ALTA Consulting – CSS Program Manager • Local Case Management Organizations: • Westhab, FSSY, and Andrus • Focus is on social and economic self sufficiency and homeownership • All resident interactions documented on CSS Management system to promote coordination with services providers and relocation team.
CSS Case Management Program • 100% of Heads of Households have been contacted or interviewed • 100% of residents assigned to individual caseworkers • Approx. 400 of the 483 Heads of Households completed needs assessments • 60% of senior residents evaluated by healthcare professionals • Approx. 80% of residents (19-64 yrs. old) have completed Family/Individual Plans • Local service agencies actively involved • Relocation / Mobility Training completed
