F-520 Quality <strong>Assessment</strong> & Assurance. The facility must<br />

maintain a quality assessment and assurance committee<br />

consisting of designated key members that identify<br />

quality deficiencies, develops and implements plans of<br />

action to correct these quality deficiencies, including<br />

monitoring the effect of implemented changes and<br />

making needed revisions to the action plans.<br />

F-522 Disclosure of Ownership. The facility must comply<br />

with disclosure requirements by providing written<br />

notice to the State agency responsible for licensing the<br />

facility at the time of change, if a changes occurs in:<br />

1. Persons with an ownership or control interest.<br />

2. The officers, directors, agents, or managing<br />

employees.<br />

3. The corporation, association, or other company<br />

responsible for the management of the facility.<br />

4. The facility’s administrator or director of nursing.<br />

5. Notice must include the identity of each new<br />

individual or company.<br />
