F-512 Transportation To/From. The facility must assist the<br />
resident in making transportation arrangements to and<br />
from the source of service, if needed.<br />
F-513 X-Ray Reports in Resident File. The facility will file<br />
in the resident’s clinical record, signed and dated<br />
reports of x-rays and other diagnostic services.<br />
F-514 Clinical Records. The facility must maintain clinical<br />
records on each resident in accordance with accepted<br />
professional standards and practices that are:<br />
1. Complete;<br />
2. Accurately documented;<br />
3. Readily accessible; and<br />
4. Systematically organized.<br />
F-515 Retention of Clinical Records. The facility must retain<br />
clinical records for:<br />
1. The period of time required by State law; or<br />
2. Five years from the date of discharge when there is<br />
no requirement in State law; or<br />
3. For a minor, three years after a resident reaches legal<br />
age under State law.<br />
F-516 Safeguard Clinical Records. The facility must<br />
safeguard clinical record information against loss,<br />
destruction, or unauthorized use.<br />
F-517 Disaster & Emergency. The facility must have detailed<br />
written plans and procedures to meet all potential<br />
emergencies and disasters, such as fire, severe weather,<br />
and missing residents.<br />
F-518 Training for Emergency. The facility must train all<br />
employees in emergency procedures when they begin<br />
to work in the facility and periodically review the<br />
procedures with existing staff.<br />
F-519 Transfer Agreement. The facility must have in effect a<br />
written transfer agreement with one or more hospitals<br />
approved for participation under the Medicare and<br />
Medicaid programs, that reasonably assures timely<br />
admission to the hospital when transfer is medically<br />
