SENIOR Prom Permission Form May 19, 2012 – Sheraton, Atlantic City, 6pm-11pm
PARTICIPATION
• The Prom is a privilege (not a right) and attendance at this event will be jeopardized by a student's failure to adhere to the
Absegami High School Code of Conduct. Permission to attend is contingent on the approval by AHS administration.
• Students must be in good standing and be free of any discipline, financial or attendance obligations.
• Approval form(s) must be signed by the Absegami student, guest (if applicable) and their parents/guardians.
• Absegami Administrative approvals must be obtained before purchase of tickets. This form is due by: April 20, 2012.
ARRIVAL
• AHS STUDENTS MUST BRING THEIR SCHOOL ID and ALL GUESTS MUST HAVE PHOTO ID to be admitted.
• All students/guests must arrive at the prom by: 7:00pm. Absolutely no one will be admitted after that time.
• All students and guests will be asked to submit to a passive breathalyzer test to determine if they have consumed alcohol.
• Anyone who does not submit to this test and/or has consumed alcohol and/or is under the influence of drugs will not be admitted.
The consumption/possession of alcoholic beverages and/or the use of drugs prior to and/or during school activities will not be
tolerated and will result in immediate removal of the person or persons involved. The Galloway Township Police Department will
be contacted, and parents will be required to escort these students home.
• There will be further disciplinary action for Absegami students upon returning to school. Criminal charges may also be filed.
CODE OF CONDUCT
• All District policies will be in force for Absegami students and guests.
• If a problem arises during the event, both the AHS student and guest will be asked to leave.
• No containers of any kind or type will be allowed to be brought into the prom - No backpacks, book bags, gym bags are
allowed.
• All purses and pocketbooks will be searched.
• There will be absolutely NO SMOKING permitted on the grounds or in the building.
• Participants must stay until 10 PM.
TICKETS
• Tickets may only be purchased by student/guest who has no outstanding obligations.
• Guest must also have this permission form completed and returned before the purchase of a ticket.
• Tickets will be sold during Lunch periods, Monday, April 30 th – Friday, May 4 th .
• Price per ticket is :$70.00
GUESTS
• AHS Students are permitted to have one guest.
• All guests must be between the ages of 14 and 20. Students in middle school are not permitted to attend.
• The guest must arrive with their AHS student date. No changes to guests will be made after tickets are purchased.
• The AHS Student is responsible for their guest. If a problem arises concerning the guest, both the guest and the AHS Student will
be asked to leave.
• To initiate approval by Absegami administration the guest permission form (on the opposite side of this form) must be signed by
the guest, their parents and their high school administration. Absegami administration will review these requests as a part of the
decision to approve/deny guest participation.
