b. indicates why the plate is being used, in the appropriate check box.<br />
c. indicates the assigned operator’s name and address<br />
d. shows the vehicle’s year, make, and VIN.<br />

g. an <strong>of</strong>fice record <strong>of</strong> which temporary transport plate number has been issued<br />
h. The dealer’s copy <strong>of</strong> the form must be filed and kept at the dealership for a period<br />
<strong>of</strong> five years.<br />
i. The plates are valid for five days (120 hours).<br />
j. The dealer cannot extend the expiration <strong>of</strong> a temporary transport plate.<br />
k. The dealer cannot loan temporary transport plates to customers or other dealers.<br />
l. The dealer must issue temporary transport plates in sequence.<br />
m. The dealer must provide proper liability insurance on all temporary transport<br />
plates or is allowed to pay the uninsured motor vehicle fee.<br />
2.6.4 How to Complete the Temporary Transport Plate<br />
When the dealer fills in the information required on the face <strong>of</strong> the cardboard temporary<br />
transport license plate, use a permanent waterpro<strong>of</strong> broad point felt-tip marker. The<br />
following information is required:<br />
a. date the plates are issued<br />
b. date the plates expire (five days from issue date)<br />
c. make and model year <strong>of</strong> the vehicle<br />
d. identification number (VIN) <strong>of</strong> the vehicle<br />
e. dealership “trading as” name<br />
NOTE: DMV supplies one temporary transport plate for each vehicle (not a pair).<br />
2.6.5 How to Order Temporary Transport Plates<br />
a. To obtain an order <strong>of</strong> temporary transport plates, you should submit to<br />
DMV/MVDB or any DMV Customer Service Center.<br />
b. Completed DSD-4. “<strong>Dealer</strong> Application for Temporary Registration/Transport<br />
Plates” indicating the type and number <strong>of</strong> plates requested.<br />
c. The dealer must order temporary transport plates in multiples <strong>of</strong> 10.<br />
d. appropriate fee for the number <strong>of</strong> plates ordered Payment must be by<br />
DEALERSHIP credit card or DEALERSHIP check ONLY.<br />
e. driver license<br />
NOTE: Liability insurance or Uninsured <strong>Motor</strong> Vehicle Fee is required on all Temporary<br />
Transport Plates.<br />
