Shift supervisors have been reminded of their duty to report and address dust generating conditions, as per the Downer EDI Mining Standard Work Practice - Dust Management (SWP 1021). On Monday 07 May, operations were modified, in accordance with SWP 1021, due to dry, windy conditions. Driller noticed oil coming from rig during operation. Immediately shut down and isolated machine. Then proceeded to contain spill using provided spill kit materials. Maintenance inspection and testing was carried out by Onedrill Contractors accidentally established drill site at a location on Wambo owned land in The error arose through confusion between this site and another nearby site that had been approved. Environmental Dpt spoke with both the Geologists and Contractors to ensure correct Surface Disturbance permit process of site approval and establishment are being followed. Identified that flow of documentation was not reaching all personnel involved directly in setting up the site which allowed for the wrong site to be established. All personnel involved are now to receive a copy of relevant documents. Supervisor must physically visit the site with the personnel. A discharge to ground occurred at Wambo Coal Mine, when mine water seeped through spoil from C11Dam (the dam) onto a neighbouring mine’s land. The discharge was first noticed on the 21 May 2012. Diesel pump installed in C11 to pump down water level Farm dam on United land reinstated to catch seepage. Sediment control placed downstream. Vacuum truck used to recover seepage water from downstream of farm dam. Appendix 3: Summary of Environmental Incidents

correct operation of the dozer for the given task.
A discharge to ground occurred at Wambo Coal
Once the faulty water level sensor was noticed in the
pumping to the dam was halted (pumping from the
dam continued to transfer mine water for licenced
