Box E7.3 Bridg<strong>in</strong>g the ‘know-do’ gap<br />

Much of the discussion at the 2000 Bangkok International Conference on<br />

<strong>Health</strong> Research for Development and the 2004 Mexico M<strong>in</strong>isterial Summit<br />

on <strong>Health</strong> Research focused on the constra<strong>in</strong>ts on researchers. However,<br />

several issues about the nature and practice of research itself need to be<br />

exam<strong>in</strong>ed as well (McCoy et al. 2004):<br />

• research cultures and <strong>in</strong>centive systems have changed so that they now<br />

encourage researchers to be more concerned with publish<strong>in</strong>g their results<br />

<strong>in</strong> academic journals than with ensur<strong>in</strong>g that their research leads<br />

to better policy and practice. Revers<strong>in</strong>g this trend may require chang<strong>in</strong>g<br />

how research is evaluated and rewarded, as well as allocat<strong>in</strong>g more fund<strong>in</strong>g<br />

to academic and non-government research <strong>in</strong>stitutions <strong>in</strong> poorer<br />

countries that work closely with policy-makers, health managers, service<br />

providers and communities.<br />

• there should be more fund<strong>in</strong>g for action research that <strong>in</strong>volves service<br />

users and providers and that ensures that research is embedded <strong>in</strong><br />

ord<strong>in</strong>ary people’s day-to-day contexts and practices (<strong>in</strong>clud<strong>in</strong>g the fact<br />

that health care systems are <strong>in</strong> a state of collapse <strong>in</strong> some countries). The<br />

use of participatory research methods can also help poor communities<br />

shape health systems to meet their needs.<br />

• implementation of health research needs to be aided by a vigorous community<br />

of civil society organizations keep<strong>in</strong>g a watch on health policy<br />

development and implementation; on the use of research funds to foster<br />

civil society’s capacity to change the commission<strong>in</strong>g and priority-sett<strong>in</strong>g<br />

of research; and on the <strong>in</strong>clusion of civil society <strong>in</strong>terests <strong>in</strong> research<br />

production and partnerships with academic researchers.<br />

• the capacity of policy-makers, health managers and practiti<strong>one</strong>rs needs<br />

to be developed to appraise and make use of new <strong>in</strong>formation.<br />

reproduction. Collectively these alliances are driven by the research excellence<br />

of the North and a focus on a selected number of diseases that cause high<br />

levels of morbidity and mortality. While potentially help<strong>in</strong>g to reduce the 10/90<br />

gap, they need to be monitored to ensure that they are not biased towards the<br />

development of medical technologies produced by Northern <strong>in</strong>stitutions at<br />

the expense of, say, health systems research that would directly strengthen the<br />

capacity of health services <strong>in</strong> develop<strong>in</strong>g countries.<br />
