go hand-<strong>in</strong>-hand with the recognition that the South has produced important<br />

research (Box E7.2).<br />

Important efforts are also be<strong>in</strong>g made to strengthen national capacity <strong>in</strong><br />

research ethics and <strong>in</strong> the review of research proposals. But capacity-build<strong>in</strong>g<br />

for ethics reviews tends to be driven by a goal of speed<strong>in</strong>g up research<br />

and m<strong>in</strong>imiz<strong>in</strong>g ‘ethical problems’ when engag<strong>in</strong>g <strong>in</strong> <strong>in</strong>ternational health<br />

research. As a result, ‘just enough’ is d<strong>one</strong> to get <strong>in</strong>ternational trials reviewed<br />

but few resources are made available to generate efficient, susta<strong>in</strong>able national<br />

systems or <strong>in</strong>dependent regional mechanisms for cont<strong>in</strong>ued local capacity<br />

development.<br />

The statement issued by the 2004 Mexico Summit reaffirmed the need<br />

to strengthen national health research systems, to establish and implement<br />

national health research policies, and to support evidence-based public health<br />

and health care systems (WHO 2004). Disappo<strong>in</strong>t<strong>in</strong>gly, however, it def<strong>in</strong>ed<br />

national health research systems as ‘the people, <strong>in</strong>stitutions, and activities<br />

whose primary purpose is to generate relevant knowledge adher<strong>in</strong>g to high<br />

ethical standards, which can be used to improve the health status of populations<br />

<strong>in</strong> an equitable way’. This def<strong>in</strong>ition emphasizes ‘generators of knowledge’<br />

– researchers – while implicitly dim<strong>in</strong>ish<strong>in</strong>g the importance of those<br />

demand<strong>in</strong>g and potentially us<strong>in</strong>g research such as policy-makers, health managers<br />

and civil society. This may reflect the tendency for conferences on health<br />

research to be dom<strong>in</strong>ated by researchers and their needs. Researchers themselves<br />

have to ensure that their research is designed and managed <strong>in</strong> ways that<br />

will bridge the ‘know-do’ gap (see Box E7.3).<br />

International partnerships<br />

The second recommendation of the Commission on <strong>Health</strong> Research for<br />

Development <strong>in</strong> 1990 focused on establish<strong>in</strong>g research partnerships and<br />

networks, <strong>in</strong> the belief that the right mix of expertise, commitment, local<br />

knowledge and excellence could result <strong>in</strong> progress. Partnerships between researchers<br />

<strong>in</strong> South and North should not only be mobilized around priority<br />

health problems, but should also strengthen the health research capacity of<br />

develop<strong>in</strong>g countries.<br />

A number of so-called global alliances has been set up <strong>in</strong> recent years,<br />

<strong>in</strong>clud<strong>in</strong>g the Multilateral Initiative aga<strong>in</strong>st Malaria, the European and Develop<strong>in</strong>g<br />

Country Cl<strong>in</strong>ical Trials Partnership, the <strong>Global</strong> Alliance for Vacc<strong>in</strong>es<br />

and Immunization, and the International AIDS Vacc<strong>in</strong>e Initiative, as well as<br />

WHO programmes for research and tra<strong>in</strong><strong>in</strong>g <strong>in</strong> tropical diseases and human<br />
