But what of the other key actors? This section presents a first attempt to<br />

develop a scorecard of the past and present performance of some of them. The<br />

scores are not based on a structured appraisal but have been compiled through<br />

a consensus process, <strong>in</strong>volv<strong>in</strong>g a number of people from academia, IBFAN and<br />

WABA centrally <strong>in</strong>volved <strong>in</strong> the promotion of breastfeed<strong>in</strong>g and protection of<br />

public health. They are presented here as an example of <strong>one</strong> possible approach<br />

to monitor<strong>in</strong>g of performance of key global health <strong>in</strong>stitutions. More rigorous<br />

assessment of performance is of course needed: this is just a start<strong>in</strong>g po<strong>in</strong>t.<br />

Indicator 1: Overall promotion of breastfeed<strong>in</strong>g and support for the code<br />

Under political pressure, WHO has not played a strong leadership role <strong>in</strong> ensur<strong>in</strong>g<br />

compliance with the code. It came under significant pressure from the<br />

US adm<strong>in</strong>istration to water down its promotion of the code. Much of its effort<br />

to promote the code has been weak, and leadership on the issue <strong>in</strong> Geneva<br />

has been poor – thus los<strong>in</strong>g extrabudgetary support from two bilateral donors<br />

dur<strong>in</strong>g the 1980s and early 1990s.<br />

UNICEF has played a more significant role <strong>in</strong> promot<strong>in</strong>g breastfeed<strong>in</strong>g. It<br />

started talk<strong>in</strong>g about the importance of breastfeed<strong>in</strong>g <strong>in</strong> the early 1980s, did a<br />

lot of work on the code <strong>in</strong> the 1980s and began to fund programmes by around<br />

1990, despite threats from the US government to withdraw fund<strong>in</strong>g if it took<br />

too strong a l<strong>in</strong>e aga<strong>in</strong>st the unethical market<strong>in</strong>g practices of the baby food<br />

companies. Without its <strong>in</strong>tervention, the support of the Swedish and Dutch aid<br />

agencies and the lobby<strong>in</strong>g of <strong>in</strong>ternational NGOs, the code could have suffered<br />

an early demise. UNICEF also deserves much of the credit for the Baby-Friendly<br />

Hospital Initiative, widely successful <strong>in</strong> <strong>in</strong>creas<strong>in</strong>g exclusive breastfeed<strong>in</strong>g <strong>in</strong><br />

the early weeks of life.<br />

However, along with other UN agencies’ work on breastfeed<strong>in</strong>g, this work<br />

greatly decl<strong>in</strong>ed after 1997 as a result of the diversion of staff time and fund<strong>in</strong>g<br />

to the new UN policy on HIV and <strong>in</strong>fant feed<strong>in</strong>g. UNICEF’s pro-breastfeed<strong>in</strong>g<br />

talk has recently returned to early 1980s levels, but the fund<strong>in</strong>g is not yet back<br />

to the levels seen <strong>in</strong> the 1990s. After years of depend<strong>in</strong>g on extrabudgetary<br />

funds for a post for legal work on the code, UNICEF has recently <strong>in</strong>corporated<br />

the post <strong>in</strong>to its core budget, signall<strong>in</strong>g a commitment to cont<strong>in</strong>ue its support<br />

for implementation of the code. However, WHO’s recent performance is<br />

considered less positive. Staff <strong>in</strong> the nutrition department with responsibility<br />

for the code are considered to have d<strong>one</strong> too little to promote it and have at<br />

times even underm<strong>in</strong>ed it <strong>in</strong> public meet<strong>in</strong>gs.<br />
